Celem proj jest wzrost liczby miejsc świadczenia usług zdrow. w zakresie opieki paliat. i działań wspierających o 8 miejsc woj. łódzkiego w okresie 2019-04-01–2020-10-31 Proj. zostanie objętych 90 os niesamodzielnych chorujących na nieuleczalne,postępujące,ograniczające życie choroby nowotworowe i nienowotworowe zgodnie z Zał. nr 1 do Rozp. MZ z dnia 29.10.2013r. w sprawie świadczeń gwarantowanych z zakresu opieki paliat. i hospic. Projektem zostanie objętych 90 op. faktycznych, ucz. w szkoleniu. Łączna l. ucz. proj. wyniesie 180os. W ramach proj. zostaną zrealizowane zadania z zakresu: 1.domowej opieki paliat. dla 90 ucz. proj. Zadanie będzie realizowane przez 18 m-cy w warunkach domowych. Świadczenia opieki paliat.i hospic. to wszechstronna,całościowa opieka i leczenie objawowe chorych na nieuleczalne,niepoddające się leczeniu przyczynowemu,postępujące,ograniczające życie choroby. Opieka będzie świadczona 365 dni w roku. Opieka paliatywna zostanie uzupełniona przez opiekuna medycznego, którego zadaniem będzie wsparcie chorego w codziennych czynnościach. 2.szk. indywidualne z zakresu uszanowania chorego i jego woli decyzyjnej jako sposób przeciwdziałania uporczywej terapii. Szkol. usprawni proces skutecznej komunikacji klinicznej,ukierunkowanej na uszanowanie woli chorego i przedyskutowanie przedterminowych poleceń. Jest to jeden z warunków przeciwdziałania uporczywej terapii(obok wczesnego wdrożenia podejścia paliatywnego,skoordynowania opieki zdrow. i społ.,urealniania celów leczenia). W szkoleniach udział wezmą opiekunowie wszystkich uczestników projektu. Proj. pozwoli na stworzenie kompleksowej usługi medyczno-opiekuńczej jaką jest domowa opieka paliatywna wraz ze wsparciem opiekuna medycznego, dzięki czemu każdy z uczestników projektu będzie miał zapewnioną opiekę przez 7 dni w tyg., co przyczyni się do realnego zwiększenia bezpieczeństwa i będzie miało bezpośredni ma wpływ na poprawę jakości życia zarówno podopiecznego, jak i opiekuna fakty (Polish)

MZ dated 29.10.2013 on guaranteed benefits in the field of paliat care and hospic. The project will be covered by 90 op. factual, learner in training. The total l. student project will be 180 people. As part of the project, the following tasks will be carried out: 1.domestic care palat. for 90 learner. project. The task will be carried out by 18 months at home. The care provision of paliat.i hospic. is a comprehensive, comprehensive care and symptomatic treatment of patients with incurable, untreated causal treatment, progressive, life-limiting disease. Care will be provided 365 days a year. Palliative care will be supplemented by a medical caregiver whose task will be to support the patient in everyday activities. 2. the individual in respect of the patient and his decision-making will as a means of counteracting persistent therapy. The school will improve the process of effective clinical communication, aimed at respecting the patient’s will and discussing early instructions. This is one of the conditions for counteracting persistent therapy (besides early implementation of palliative approach, coordination of health care. and community, achievement of treatment objectives). The training will be attended by carers of all participants of the project. The project will allow the creation of a comprehensive medical and care service, which is home palliative care together with the support of a medical caregiver, so that each of the project participants will be provided care for 7 days in a week, which will contribute to a real increase in safety and will have a direct impact on improving the quality of life of both the patient and the caregiver facts. L’objectif du projet est d’augmenter le nombre de places fournissant des services de santé dans le domaine des soins palliatifs et des activités de soutien de 8 places de la voïvodie de Łódź au cours de la période 2019-04-01-2020-10-31 Proj. seront couverts par 90 personnes dépendantes souffrant d’un cancer incurable, en progression et limitant la vie, ainsi que de maladies non cancéreuses, conformément à l’annexe 1 du règlement. MZ du 29.10.2013 sur les prestations garanties dans le domaine du paliat. et de l’hospice. Le projet sera couvert par 90 op. de facto, l’enseignement en formation. Le total de l. enseigner. proj. sera 180o. Dans le cadre du projet, les tâches suivantes seront exécutées: 1. soins à domicile palais. pour l’enseignement 90. proj. La tâche sera exécutée d’ici 18 mois dans les conditions de la maison. La fourniture de paliat.i hospic. est un traitement complet, global et symptomatique des patients atteints de maladies incurables, non-causionales, progressistes, limitant la vie. Les soins seront dispensés 365 jours par an. Les soins palliatifs seront complétés par un soignant qui aura pour tâche de soutenir le patient dans ses activités quotidiennes. 2. Le verre individuel dans le domaine du respect du patient et de sa prise de décision sera un moyen de contrecarrer la thérapie persistante. L’école améliorera le processus de communication clinique efficace, dans le but de respecter la volonté du patient et de discuter des commandements précoces. C’est l’une des conditions pour lutter contre le traitement persistant (à côté de la mise en œuvre précoce de l’approche palliative, de la coordination des soins de santé et des soins sociaux, de la réalisation des objectifs du traitement). La formation sera suivie par les superviseurs de tous les participants au projet. Le projet permettra de créer un service médical et de soins complet, c’est-à-dire des soins palliatifs à domicile, avec le soutien d’un soignant, grâce auquel chacun des participants au projet recevra des soins pendant 7 jours par semaine, ce qui contribuera à une réelle augmentation de la sécurité et aura un effet direct sur l’amélioration de la qualité de vie de l’aidant et du soignant des faits (French)

Ziel des Projekts ist es, die Zahl der Plätze, die Gesundheitsdienstleistungen im Bereich der Palliativversorgung und Unterstützungsmaßnahmen anbieten, um 8 Orte der Woiwodschaft Łódź im Zeitraum 2019-04-01-2020-10-31 Proj. zu erhöhen, die gemäß Anhang 1 der Verordnung von 90 abhängigen Personen abgedeckt werden, die an unheilbaren, fortschreitenden, lebensbegrenzenden Krebs- und Krebserkrankungen leiden. MZ vom 29.10.2013 zu garantierten Leistungen im Bereich Paliat und Hospic. Das Projekt wird durch 90 op. de facto, Unterricht in der Ausbildung abgedeckt werden. Die Gesamtzahl l. unterrichten. proj. wird 180os sein. Im Rahmen des Projekts werden folgende Aufgaben durchgeführt: 1. Hauspflege Gaumen. für 90 Unterricht. proj. Die Aufgabe wird um 18 Monate in den häuslichen Bedingungen ausgeführt. Die Bereitstellung von paliat.i hospic. ist eine umfassende, umfassende Pflege und symptomatische Behandlung von Patienten mit unheilbaren, nicht kausionalen,progressiven,lebensbegrenzenden Erkrankungen. Pflege wird 365 Tage im Jahr zur Verfügung gestellt. Die Palliativversorgung wird durch einen Arzt ergänzt, dessen Aufgabe es ist, den Patienten bei alltäglichen Aktivitäten zu unterstützen. 2. Individuelles Glas im Bereich der Achtung vor dem Patienten und seiner Entscheidungsfindung wird als Mittel zur Bekämpfung der anhaltenden Therapie dienen. Die Schule wird den Prozess der effektiven klinischen Kommunikation verbessern, um den Willen des Patienten zu respektieren und frühzeitige Befehle zu diskutieren. Dies ist eine der Bedingungen für die Bekämpfung der anhaltenden Therapie (neben der frühzeitigen Umsetzung des palliativen Ansatzes, Koordinierung von Gesundheitsversorgung und Sozialfürsorge, Verwirklichung der Ziele der Behandlung). An der Schulung nehmen die Vorgesetzten aller Projektteilnehmer teil. Das Projekt ermöglicht es, einen umfassenden medizinischen und Pflegedienst zu schaffen, der die häusliche Palliativversorgung zusammen mit der Unterstützung eines medizinischen Pflegepersonals ist, dank dem jeder der Projektteilnehmer für 7 Tage in der Woche betreut wird, was zu einer wirklichen Erhöhung der Sicherheit beiträgt und sich unmittelbar auf die Verbesserung der Lebensqualität sowohl der Pfleger als auch der Betreuer der Fakten auswirkt. Doel van het project is het aantal plaatsen dat gezondheidsdiensten op het gebied van palliatieve zorg en ondersteunende activiteiten verleent, te verhogen met acht plaatsen van het woiwodschap Łódź in de periode 2019-04-01-2020-10-31 Proj. zal worden gedekt door 90 afhankelijke personen die lijden aan ongeneeslijke, voortschrijdende, levensbeperkende kanker- en niet-kankerziekten overeenkomstig bijlage 1 bij de verordening. MZ van 29.10.2013 over gegarandeerde uitkeringen op het gebied van paliat. en hospic. Het project zal worden gedekt door 90 op. de facto, onderwijs in opleiding. De totale l. teach. proj. zal 180o zijn. In het kader van het project worden de volgende taken uitgevoerd: 1. thuis zorg gehemelte. voor 90 onderwijs. proj. De taak zal binnen 18 maanden in huis worden uitgevoerd. De verstrekking van paliat.i hospic. is een uitgebreide, algemene zorg en symptomatische behandeling van patiënten met ongeneeslijke, niet-causional,progressieve, levensbeperkende ziekten. De zorg wordt 365 dagen per jaar verstrekt. Palliatieve zorg zal worden aangevuld met een medische verzorger, wiens taak zal zijn om de patiënt te ondersteunen bij alledaagse activiteiten. 2. Individueel glas op het gebied van respect voor de patiënt en zijn besluitvorming zal als een manier om aanhoudende therapie tegen te gaan. De school zal het proces van effectieve klinische communicatie verbeteren, gericht op het respecteren van de wil van de patiënt en het bespreken van vroege commando’s. Dit is een van de voorwaarden voor het tegengaan van aanhoudende therapie (naast de vroege implementatie van palliatieve aanpak, coördinatie van gezondheidszorg en sociale zorg, het realiseren van de doelstellingen van de behandeling). De opleiding wordt bijgewoond door de begeleiders van alle projectdeelnemers. Project zal het mogelijk maken om een uitgebreide medische en zorgdienst te creëren, die thuis palliatieve zorg is, samen met de ondersteuning van een medische verzorger, waardoor elk van de deelnemers aan het project 7 dagen per week zorg krijgt, wat zal bijdragen tot een echte verhoging van de veiligheid en een direct effect zal hebben op de verbetering van de levenskwaliteit van zowel de verzorger als de verzorger van feiten (Dutch)

L'obiettivo del progetto è aumentare il numero di posti che forniscono servizi sanitari nel settore delle cure palliative e delle attività di sostegno di 8 posti del voivodato di Łódź nel periodo 2019-04-01-2020-10-31 Proj. sarà coperto da 90 persone non autosufficienti che soffrono di cancro incurabile, progressivo, limitante la vita e malattie non tumorali, conformemente all'allegato 1 del regolamento. MZ del 29.10.2013 sulle prestazioni garantite nel settore paliat. e hospic. Il progetto sarà coperto da 90 op. de facto, insegnamento in formazione. Il totale l. insegnare. proj. sarà 180os. Nell'ambito del progetto saranno svolti i seguenti compiti: 1.home care palato. per 90 insegnamento. proj. Il compito sarà effettuato entro 18 mesi in condizioni di casa. La fornitura di paliat.i ospic. è una cura completa e generale e un trattamento sintomatico di pazienti con malattie incurabili, non cauzionali, progressive e limitanti la vita. Le cure saranno fornite 365 giorni all'anno. Le cure palliative saranno integrate da un'assistenza medica, il cui compito sarà quello di sostenere il paziente nelle attività quotidiane. 2. Il vetro individuale nel campo del rispetto per il paziente e il suo processo decisionale sarà un modo per contrastare la terapia persistente. La scuola migliorerà il processo di comunicazione clinica efficace, finalizzata al rispetto della volontà del paziente e alla discussione dei primi comandi. Questa è una delle condizioni per contrastare la terapia persistente (accanto all'attuazione precoce dell'approccio palliativo, al coordinamento dell'assistenza sanitaria e dell'assistenza sociale, alla realizzazione degli obiettivi del trattamento). Alla formazione parteciperanno i supervisori di tutti i partecipanti al progetto. Il progetto consentirà di creare un servizio medico e assistenziale completo, che è l'assistenza palliativa domiciliare insieme al supporto di un medico assistenziale, grazie al quale ciascuno dei partecipanti al progetto sarà fornito con assistenza per 7 giorni in una settimana, che contribuirà ad un reale aumento della sicurezza e avrà un effetto diretto sul miglioramento della qualità della vita sia del caregiver che del caregiver dei fatti (Italian)
