Celem głównym p. jest zwiększ. dostępności usług społecz., w szczególności sąsiedzkich usług opiekuńczych dla 35 osób (21K, 14M) starszych niesamodzielnych zagrożonych ubóstwem lub wykluczeniem społecznym z obszaru GJ przez objęcie wsparciem w formie sąsiedzkich usług opiekuńczych wmiejscu zamieszkania oraz usług wykorzystuj. nowoczesne technologie informacyjno-komunikacyjne, które umożliwią 35 osobom starszym niesamodzielnym(21K, 14M) jak najdłuższe bezpieczne pozostanie w ich środowisku w okresie do 31.03.2022 r.Głównym rezultatem p. jest zwiększenie liczby miejsc/osób objętych sąsiedzkimi usługami opiekuńczymi w GJ.Grupą docelową p. jest 35 osób starszych niesamodzielnych (21K, 14M) - osoby, która ze względu na stan zdrowia lub niepełnosprawność wymagają opieki lub wsparcia w związku z niemożnością samodzielnego wykonywania co najmniej jednej z podstawowych czynności dnia codziennego. Będą to osoby zamieszkujące w rozumieniu przepisów KC gminę Jasło (gmina wiejska; woj. podkarpackie). Główne zadania: 1) Sąsiedzkie usługi opiekuńcze, 2) Teleopieka. P. przez obj. wsparciem starszych niesamodzielnych z obszaru GJ, zapewnienie im sąsiedzkich usług opiekuńcz. i usług wykorzyst. nowoczesne technologie informacyjno-komunikacyjne – Teleopieki przyczyni się do osiągnięcia celu szczegółowego RPO WP 2014-2020. Dzięki realizacji p. osoby starsze niesamodzielne poczują się bezpieczniejsze w miejscu swego zamieszk., zostaną otoczone odpowied. opieką - pomocą sąsiedzką, będą miały kontakt z otoczeniem. The main goal is to increase the availability of social services, in particular neighbourhood care services for 35 people (21K, 14M) for older dependents at risk of poverty or social exclusion from the GJ area by providing support in the form of neighbourhood care services at the place of residence and services. Modern information and communication technologies that will enable 35 elderly dependents (21K, 14M) to stay as safe as possible in their environment until 31.03.2022. The main result of which is to increase the number of places/persons covered by neighbourhood care services in GJ. They will be residents within the meaning of the provisions of the Municipality of Jasło (village; Podkarpackie voivodeship). Main tasks: 1) Neighbouring care services, 2) Telecare. P. by supporting older dependents in the GJ area, providing them with neighbouring care services and services using modern information and communication technologies – Telecare will contribute to the achievement of the specific objective of the ROP PP 2014-2020. Thanks to the implementation of the project, the elderly will feel safer in the place of their riots, they will be surrounded by an answer. care – the help of the neighbours, they will have contact with the surroundings. L’objectif principal est d’accroître la disponibilité des services sociaux, en particulier des services de proximité pour 35 personnes (21 000, 14 millions de personnes) pour les personnes âgées exposées au risque de pauvreté ou d’exclusion sociale de la région de GJ en fournissant un soutien sous la forme de services de proximité sur le lieu de résidence et de services. Les technologies modernes de l’information et de la communication qui permettront à 35 personnes âgées à charge (21K, 14M) de rester aussi sûres que possible dans leur environnement jusqu’au 31.03.2022, dont le principal résultat est d’augmenter le nombre de places/personnes couvertes par les services de proximité en GJ. Ils seront résidents au sens des dispositions de la municipalité de Jasło (village; Voïvodie de Podkarpackie). Principales tâches: 1) Services de soins de proximité, 2) Télésoins. P. en soutenant les personnes âgées dépendantes dans la région de GJ, en leur fournissant des services de soins et des services voisins utilisant les technologies modernes de l’information et de la communication — Telecare contribuera à la réalisation de l’objectif spécifique du PP ROP 2014-2020. Das Hauptziel besteht darin, die Verfügbarkeit sozialer Dienstleistungen, insbesondere Nachbarschaftspflegeleistungen für 35 Personen (21K, 14M) für ältere Menschen, die von Armut oder sozialer Ausgrenzung bedroht sind, aus dem GJ-Gebiet zu erhöhen, indem sie Unterstützung in Form von Nachbarschaftsbetreuungsdiensten am Wohnort und Dienstleistungen anbieten. Moderne Informations- und Kommunikationstechnologien, die es 35 älteren Menschen ermöglichen (21K, 14M), bis zum 31.03.2022 in ihrem Umfeld so sicher wie möglich zu bleiben. Sie werden Einwohner im Sinne der Bestimmungen der Gemeinde Jasło (Dorf; Woiwodschaft Podkarpackie). Hauptaufgaben: 1) benachbarte Pflegedienste, 2) Telecare. P. durch die Unterstützung älterer Angehöriger im GJ-Bereich und die Bereitstellung von benachbarten Pflegediensten und Dienstleistungen unter Nutzung moderner Informations- und Kommunikationstechnologien – Telecare wird zur Verwirklichung des spezifischen Ziels des ROP PP 2014-2020 beitragen. Dank der Umsetzung des Projekts werden sich ältere Menschen an der Stelle ihrer Unruhen sicherer fühlen, sie werden von einer Antwort umgeben sein. Het belangrijkste doel is de beschikbaarheid van sociale diensten, met name buurtzorgdiensten voor 35 personen (21K, 14 miljoen) voor ouderen die met armoede of sociale uitsluiting uit het GJ-gebied worden bedreigd, te vergroten door steun te verlenen in de vorm van buurtzorgdiensten op de woonplaats en diensten. Moderne informatie- en communicatietechnologieën die 35 afhankelijke ouderen (21K, 14M) in staat zullen stellen zo veilig mogelijk in hun omgeving te blijven tot 31.03.2022. Het belangrijkste resultaat daarvan is het verhogen van het aantal plaatsen/personen dat onder buurtzorgdiensten in GJ valt. Zij zullen ingezetenen zijn in de zin van de bepalingen van de gemeente Jasło (dorp; Podkarpackie woiwodeschap). Belangrijkste taken: 1) Aangrenzende zorgdiensten, 2) Telecare. P. door oudere afhankelijken in het GJ-gebied te ondersteunen door hen naburige zorgdiensten en -diensten te verlenen met behulp van moderne informatie- en communicatietechnologieën — Telecare zal bijdragen tot de verwezenlijking van de specifieke doelstelling van het ROP PP 2014-2020.
