Celem projektu jest zwiększenie dostępności usług społecznych dla 65 osób (35K, 30M) z 15 rodzin zamieszkałych na terenie gminy Rozogi, zagrożonych ryzykiem ubóstwa i/lub wykluczenia społecznego poprzez objęcie ich w okresie 01.10.2017 - 30.09.2018 usługami społecznymi w postaci zadań: indywidualnej diagnozy rodziny, poradnictwa indywidualnego, zajęć grupowych, zajęć wyjazdowych, pracy socjalnej, prowadzącymi do aktywnego udziału w życiu społecznym oraz powrocie na rynek pracy. W ramach projektu realizowane będą następujące zadania: indywidualna diagnoza rodziny, poradnictwo indywidualne, zajęcia grupowe, zajęcia wyjazdowe, praca socjalna. Uczestnikami projektu będzie 65 (35K, 30M) osób z 15 rodzin zamieszkałych na terenie gminy Rozogi w rozumieniu Kodeksu Cywilnego. Uczestnikami projektu, są osoby zagrożone ryzykiem ubóstwa i/lub wykluczenia społecznego (zgodnie z definicją określoną w horyzontalnych, krajowych wytycznych ministra właściwego ds. rozwoju regionalnego dotyczących realizacji CT 9 stworzoną na podstawie Programu Przeciwdziałania Ubóstwu i Wykluczeniu Społecznemu), w tym osoby bezrobotne, które zgodnie z ustawą o promocji zatrudnienia i instytucjach rynku pracy znajdą się w trzeciej grupie osób (tzw. oddalonych od rynku pracy) mieszkające poza Miejskim Obszarem Funkcjonalnym Olsztyna. Wsparciem obejmowane będą rodziny wieloproblemowe. Dzięki realizacji projektu zwiększy się dostępność usług społecznych na terenie gminy Rozogi. (Polish)

The aim of the project is to increase the availability of social services for 65 people (35K, 30M) from 15 families living in Rozogi commune, at risk of poverty and/or social exclusion by extending them to social services during the period 01.10.2017-30.09.2018 in the form of tasks: individual diagnosis of family, individual counseling, group activities, away activities, social work, leading to active participation in social life and returning to the labour market. The project will carry out the following tasks: individual family diagnosis, individual counseling, group activities, away activities, social work. The participants of the project will be 65 (35K, 30M) people from 15 families living in Rozogi commune within the meaning of the Civil Code. The participants of the project are people at risk of poverty and/or social exclusion (as defined in the horizontal national guidelines of the Minister responsible for regional development for the implementation of CT 9 based on the Programme against Poverty and Social Exclusion), including unemployed persons who, in accordance with the Law on the Promotion of Employment and Labour Market Institutions, will be included in the third group of persons (so-called remote from the labour market) living outside the City Function Area. Support will include multi-problem families. The implementation of the project will increase the availability of social services in Rozogi commune. (English)

L’objectif du projet est d’accroître la disponibilité des services sociaux pour 65 personnes (35K, 30M) de 15 familles vivant dans la municipalité de Rozogi exposées au risque de pauvreté et/ou d’exclusion sociale en leur fournissant des services sociaux au cours de la période 01.10.2017-30.09.2018 sous forme de tâches: diagnostic individuel de la famille, conseil individuel, activités de groupe, activités sortantes, travail social, conduisant à une participation active à la vie sociale et à la réinsertion sur le marché du travail. Le projet s’acquittera des tâches suivantes: diagnostic individuel de famille, counseling individuel, classes de groupe, cours à l’extérieur, travail social. Les participants au projet seront 65 (35K, 30M) de 15 familles résidant dans la commune de Rozogi au sens du Code civil. Les participants au projet sont les personnes exposées au risque de pauvreté et/ou d’exclusion sociale (telles que définies dans les lignes directrices nationales horizontales du ministre chargé du développement régional concernant la mise en œuvre de la CT 9, créée sur la base du programme de lutte contre la pauvreté et l’exclusion sociale), y compris les chômeurs qui, conformément à la loi sur la promotion de l’emploi et des institutions du marché du travail, figureront parmi les troisièmes groupes de personnes (appelées éloignées du marché du travail) vivant en dehors de la zone fonctionnelle municipale d’Olsztyn. Les familles à problèmes multiples seront soutenues. Grâce à la mise en œuvre du projet, la disponibilité des services sociaux dans la municipalité de Rozogi sera augmentée. (French)

Ziel des Projekts ist es, die Verfügbarkeit von Sozialleistungen für 65 Personen (35K, 30M) von 15 Familien, die in der Gemeinde Rozogi leben, zu erhöhen, die von Armut und/oder sozialer Ausgrenzung bedroht sind, indem ihnen im Zeitraum 01.10.2017-30.09.2018 soziale Dienstleistungen in Form von Aufgaben zur Verfügung gestellt werden: individuelle Diagnose der Familie, individuelle Beratung, Gruppenaktivitäten, ausgehende Tätigkeiten, Sozialarbeit, die zur aktiven Teilhabe am gesellschaftlichen Leben und zur Wiedereingliederung in den Arbeitsmarkt führt. Im Rahmen des Projekts werden folgende Aufgaben durchgeführt: individuelle Familiendiagnose, individuelle Beratung, Gruppenklassen, Auswärtsklassen, Sozialarbeit. Die Teilnehmer des Projekts werden 65 (35K, 30M) Personen aus 15 Familien mit Wohnsitz in der Gemeinde Rozogi im Sinne des Bürgerlichen Gesetzbuchs sein. Teilnehmer des Projekts sind Personen, die von Armut und/oder sozialer Ausgrenzung bedroht sind (wie in den horizontalen, nationalen Leitlinien des für die regionale Entwicklung zuständigen Ministers für die Umsetzung von CT 9, das auf der Grundlage des Programms zur Bekämpfung von Armut und sozialer Ausgrenzung geschaffen wurde), einschließlich Arbeitslose, die gemäß dem Gesetz über die Förderung von Beschäftigungs- und Arbeitsmarktinstitutionen zu der dritten Gruppe von Personen gehören (sogenannte „Abgelegenheit vom Arbeitsmarkt“), die außerhalb des städtischen Funktionsgebiets von Olsztyn leben. Familien mit mehreren Problemen werden unterstützt. Dank der Umsetzung des Projekts wird die Verfügbarkeit von Sozialdienstleistungen in der Gemeinde Rozogi erhöht. (German)

Doel van het project is de beschikbaarheid van sociale diensten voor 65 personen (35K, 30M) van 15 gezinnen in de gemeente Rozogi die met armoede en/of sociale uitsluiting worden bedreigd, te vergroten door hen in de periode 1.10.2017-30.09.2018 sociale diensten te verlenen in de vorm van taken: individuele diagnose van het gezin, individuele begeleiding, groepsactiviteiten, uitgaande activiteiten, maatschappelijk werk, actieve deelname aan het maatschappelijk leven en re-integratie op de arbeidsmarkt. Het project voert de volgende taken uit: individuele gezinsdiagnostiek, individuele begeleiding, groepslessen, afstandsklassen, sociaal werk. De deelnemers aan het project zijn 65 (35K, 30M) mensen uit 15 gezinnen die in de gemeente Rozogi wonen in de zin van het Burgerlijk Wetboek. Deelnemers aan het project zijn mensen die risico lopen op armoede en/of sociale uitsluiting (zoals gedefinieerd in de horizontale, nationale richtsnoeren van de minister die verantwoordelijk is voor regionale ontwikkeling met betrekking tot de uitvoering van CT 9, opgericht op basis van het programma ter bestrijding van armoede en sociale uitsluiting), met inbegrip van werklozen die, overeenkomstig de wet ter bevordering van instellingen voor werkgelegenheid en arbeidsmarkt, behoren tot de derde groep mensen (zogenaamd ver van de arbeidsmarkt) die buiten het gemeentelijke functionele gebied Olsztyn wonen. Families met meerdere problemen zullen worden ondersteund. Dankzij de uitvoering van het project zal de beschikbaarheid van sociale diensten in de gemeente Rozogi worden verhoogd. (Dutch)
