Celem projektu będzie podniesienie dostępu do usług opieki i specjalist. wsparcia dla dzieci i młodzieży szkolnej w wieku do 18 r.ż. oraz rodzin zagrożonych dysfunkcjami lub przeżywających trudności opiekuńczo wychowawcze z terenu Gminy Zawoja GZ w okresie od 01.01.2018 31.12.2020.Projekt ma na celu organizację czasu wolnego dzieci i młodzieży z terenu GZ z uwzględnieniem działań edukacyjnych, sportowych, kulturalnych, rekreacyjnych oraz animacyjnych rozwijających indywidualne talenty i pasje młodego człowieka. Projekt będzie dążył do poprawy funkcjonowania oraz propagowania wartości rodziny w oparciu o indywidualne potrzeby wychowanka i jego rodziny określone w Indywidualnym Planie Wsparcia. Realizacja projektu uwzględnia wsparcie istniejących placówek wsparcia dziennego PWD w GZ poprzez zwiększenie liczby miejsc w tych PWD oraz rozszerzenie oferty wsparcia PWD będą prowadzone w formie opiekuńczej, i specjalistycznej w dni robocze oraz dni wolne od pracy w wakacje i ferie. PWD będą prowadzone zgodnie z Ustawą o wspieraniu rodziny. Pobyt dziecka w PWD będzie nieodpłatny w trakcie projektu i okresie trwałości. Projekt zakładać będzie wsparcie dla osób i rodzin zagrożonych ubóstwem lub wykluczeniem społecznym, doświadczające wielokrotnego wykluczenia społecznego. Wsparciem objęte zostaną osoby legitymujące się znacznym lub umiarkowanym stopniem niepełnosprawności oraz dzieci z orzeczoną niepełnosprawnością. Mając na uwadze również system pomocy społecznej działaniami zostaną objęte również osoby i rodziny korzystające i uprawnione do PO PŻ. PWD będą realizowały interdyscyplinarny model pracy, angażując do współpracy przedstawicieli innych lokalnych instytucji działających na rzecz rodziny w tym oświaty i pomocy społecznej. Należy podkreślić również, że PWD znajdują się na obszarze zdegradowanym i objęte są programem rewitalizacji. Projekt został wpisany do lokalnego programu rewitalizacji. (Polish)

The project will aim to improve the functioning and promotion of family values based on the individual needs of the pupil and his family defined in the Individual Support Plan. The implementation of the project takes into account the support of existing PWD day support facilities in GZ by increasing the number of seats in these PWDs and extending the PWD support offer will be carried out in the form of care, and specialist on working days and holidays during holidays and holidays. PWD will be conducted in accordance with the Law on Family Support. The child’s stay in PWD will be free of charge during the project and durability period. The project will support people and families at risk of poverty or social exclusion who experience multiple social exclusion. Support will be given to persons with a significant or moderate degree of disability and children with a disability. Also taking into account the social assistance system, the measures will also cover persons and families benefiting from and entitled to PO GH. PWD will implement an interdisciplinary work model, involving representatives of other local institutions working for the family, including education and social assistance. L’objectif du projet sera d’améliorer l’accès aux services de soins et de soutien spécialisé pour les enfants et les jeunes scolarisés jusqu’à l’âge de 18 ans et les familles exposées à des dysfonctionnements ou à des difficultés de soins depuis la commune de Zawoja GZ au cours de la période allant du 01.01.2018 au 31.12.2020. Le projet vise à organiser des loisirs pour les enfants et les jeunes de la zone GZ, y compris des activités éducatives, sportives, culturelles, récréatives et d’animation développant des talents individuels et des passions des jeunes. Le projet visera à améliorer le fonctionnement et à promouvoir les valeurs de la famille en fonction des besoins individuels de l’éducateur et de sa famille tels que définis dans le plan de soutien individuel. La mise en œuvre du projet tient compte de l’appui apporté aux structures d’appui quotidiennes existantes en faveur des personnes handicapées à GZ en augmentant le nombre de places dans ces personnes et en élargissant l’offre d’aide à la personne handicapée sous la forme de soins, et spécialisée sur les jours de travail et les jours fériés. La DDP sera exécutée conformément à la loi sur l’aide à la famille. Le séjour de l’enfant dans la PWD sera gratuit pendant le projet et la période de durabilité. Le projet soutiendra les personnes et les familles exposées au risque de pauvreté ou d’exclusion sociale, qui sont confrontées à de multiples exclusions sociales. Les personnes ayant un degré élevé ou modéré d’incapacité et les enfants handicapés bénéficieront d’un soutien. Compte tenu également du système d’assistance sociale, les personnes et les familles bénéficiant du PAP et ayant droit à celui-ci seront également couvertes. Die Arbeitsunterlage wird gemäß dem Familienhilfegesetz durchgeführt. Der Aufenthalt des Kindes in der Arbeitsunterlage ist während des Projekts und der Dauerhaftigkeit kostenlos. Mit dem Projekt sollen Menschen und Familien unterstützt werden, die von Armut oder sozialer Ausgrenzung bedroht sind und mehrfache soziale Ausgrenzung erfahren. Menschen mit einem erheblichen oder mittleren Grad an Behinderung und Kinder mit Behinderungen werden unterstützt. Auch unter Berücksichtigung des Sozialhilfesystems werden Personen und Familien, die Anspruch auf das PAP haben und Anspruch auf das PAP haben, ebenfalls abgedeckt. Het doel van het project is de toegang tot zorgdiensten en gespecialiseerde ondersteuning voor kinderen en schooljongeren tot 18 jaar en gezinnen met een risico op disfunctie of zorgproblemen uit de gemeente Zawoja GZ in de periode van 01.01.2018 31.12.2020 te verbeteren. Het project heeft tot doel vrije tijd te organiseren voor kinderen en jongeren uit het GZ-gebied, met inbegrip van educatieve, sport-, culturele, recreatie- en animatieactiviteiten die individuele talenten en passies van jongeren ontwikkelen. Het project heeft tot doel de werking en bevordering van de waarden van het gezin te verbeteren op basis van de individuele behoeften van de opvoeder en zijn gezin, zoals gedefinieerd in het individuele ondersteuningsplan. Bij de uitvoering van het project wordt rekening gehouden met de ondersteuning van de bestaande dagelijkse ondersteuningsfaciliteiten van gehandicapten in GZ door het aantal plaatsen in deze gehandicapten te verhogen en door het aanbod van steun aan de gehandicapten in de vorm van zorg uit te breiden, en gespecialiseerd in werkdagen en vakanties. De PWD zal worden uitgevoerd in overeenstemming met de Wet gezinsondersteuning. Het verblijf van het kind in de PWD is gratis tijdens het project en de periode van duurzaamheid. Het project zal mensen en gezinnen ondersteunen die het risico lopen op armoede of sociale uitsluiting en die te maken hebben met meervoudige sociale uitsluiting. Mensen met een significante of matige mate van handicap en kinderen met een handicap zullen worden ondersteund. Rekening houdend met het stelsel van sociale bijstand zullen ook personen en gezinnen die profiteren van en recht hebben op het PAP, onder de regeling vallen. De gehandicaptenorganisaties zullen een interdisciplinair werkmodel ten uitvoer leggen, waarbij vertegenwoordigers van andere voor het gezin werkende lokale instellingen betrokken zijn, met inbegrip van onderwijs en sociale bijstand. Er zij ook op gewezen dat de PMP’s zich in een aangetast gebied bevinden en onder een revitaliseringsprogramma vallen. Het project is opgenomen in het lokale revitaliseringsprogramma. (Dutch)
