POLSKIE STOWARZYSZENIE NA RZECZ OSÓB Z NIEPEŁNOSPRAWNOŚCIĄ INTELEKTUALNĄ KOŁO W JAROSŁAWIU

Celem głównym projektu (P) jest zwiększenie dostępności usług społecznych w szczególności całodziennych usług opiekuńczych i świadczonych w lokalnej społeczności (w tym aktywizacji w sferze fizycznej, intelektualnej, społecznej) dla 10 osób z upośledzeniem umysłowym, z orzeczonym stopniem niepełnosprawności (OUU), zamieszkujących na terenie powiatu jarosławskiego do 31 grudnia 2021 roku. P. realizowany będzie w okresie od 01.01.2019 r. do 31.12.2021 r. Grupą docelową P. będzie 10 niesamodzielnych osób (5K,5M) z upośledzeniem umysłowym, z orzeczonym stopniem niepełnosprawności (OUU), zamieszkujących na terenie powiatu jarosławskiego, w wieku powyżej 18 roku życia – wymagających wsparcia w formie usług opiekuńczych. Z doświadczenia Wnioskodawca (W.) wynika, jak również biorąc pod uwagę przedstawioną analizę sytuacji problemowej, że z P. skorzystają głównie osoby z upośledzeniem umysłowym znacznym lub głębokim z orzeczonym znacznym stopniem niepełnosprawności (OUUzZSN). W ramach P. planuje się rozszerzenie i działalność Dziennego Domu Pomocy (DDP) dla łącznie 10 uczestników projektu (UP). Organizacja placówki oraz zakres i poziom świadczonych w niej usług uwzględnia wolność, intymność, godność i poczucie bezpieczeństwa uczestników zajęć oraz stopień ich fizycznej i psychicznej sprawności. W ramach pobytu w DDP UP dostarczane będą usługi opiekuńcze oraz aktywizacja w sferze fizycznej, intelektualnej oraz społecznej, które świadczone będą przez kierownika DDP, opiekunów osób niepełnosprawnych, psychologa, fizjoterapeutę i terapeutę. Główne rezultaty P: 1. Liczba wspartych w programie miejsc świadczenia usług społecznych, istniejących po zakończeniu projektu – 10 2. The main objective of the project (P) is to increase the availability of social services in particular all-day care services and services in the local community (including activation in the physical, intellectual and social spheres) for 10 people with mental disabilities, with a degree of disability (Ou) residing in the district of Jaroslawski until 31 December 2021. P. will be implemented in the period from 01.01.2019 to 31.12.2021. The target group of P. will be 10 dependable persons (5K,5M) with mental impairment, with determined degree of disability (Ouu), living in the district of Jaroslawski, aged over 18 – requiring support in the form of care services. From the experience of the applicant (W.) it follows, as well as taking into account the presented analysis of the problem situation, that P will benefit mainly persons with significant or profound mental impairment with a significant degree of disability (OUZSN). Under P. plans to extend and operate the Daily Assistance House (DDP) for a total of 10 participants of the project (UP). The organisation of the facility and the scope and level of services provided take into account the freedom, intimacy, dignity and security of participants, as well as the degree of their physical and mental fitness. As part of the stay in the DDP UP will provide care services and activation in the physical, intellectual and social spheres, which will be provided by the DDP manager, caregivers of persons with disabilities, psychologist, physiotherapist and therapist. L’objectif principal du projet (P) est d’accroître la disponibilité des services sociaux, en particulier des services de soins et des services de garde tout au long de la journée fournis dans la communauté locale (y compris l’activation physique, intellectuelle et sociale) pour 10 personnes souffrant d’un handicap mental, présentant un degré de handicap (OUu), résidant dans le district de Jarosław jusqu’au 31 décembre 2021. P. sera mis en œuvre au cours de la période allant du 01.01.2019 au 31.12.2021. Le groupe cible P. sera de 10 personnes à charge (5K,5M) souffrant d’un handicap mental, avec un degré de handicap (OUu), résidant dans le district de Jarosław, âgées de plus de 18 ans, nécessitant un soutien sous la forme de services de soins. L’expérience de la requérante (W.) montre, tout en tenant compte de l’analyse de la situation problématique présentée, que P. bénéficiera principalement aux personnes souffrant d’un handicap mental grave ou grave (OUUZZSN). P. prévoit d’étendre et d’exploiter le Daily Help House (DDP) pour un total de 10 participants au projet (UP). L’organisation de l’institution ainsi que l’étendue et le niveau des services qui y sont fournis tiennent compte de la liberté, de l’intimité, de la dignité et de la sécurité des participants et du degré de leur aptitude physique et mentale. Dans le cadre du séjour au DDP UP, des services de soins et d’activation dans les sphères physique, intellectuelle et sociale seront fournis, qui seront assurés par le directeur de DDP, les aidants des personnes handicapées, psychologue, physiothérapeute et thérapeute. Hauptziel des Projekts (P) ist es, die Verfügbarkeit sozialer Dienstleistungen, insbesondere Ganztagesdienste und Dienstleistungen, die in der örtlichen Gemeinschaft (einschließlich physischer, geistiger und sozialer Aktivierung) für 10 Menschen mit psychischen Behinderungen (OUu) erbracht werden, bis zum 31. Dezember 2021 im Bezirk Jarosław zu erhöhen. P. wird in der Zeit vom 01.01.2019 bis zum 31.12.2021 umgesetzt. Die Zielgruppe P. wird 10 unterhaltsberechtigte Personen (5K,5M) mit geistiger Behinderung sein, mit einem Grad an Behinderung (OUu), die im Bezirk Jarosław im Alter von über 18 Jahren wohnhaft sind und Unterstützung in Form von Betreuungsleistungen benötigen. Die Erfahrung des Antragstellers (W.) zeigt neben der Analyse der dargestellten Problemsituation, dass P. vor allem Menschen mit schwerer oder schwerer psychischer Behinderung (OUUZZSN) zugute kommt. P. plant die Erweiterung und den Betrieb des Daily Help House (DDP) für insgesamt 10 Projektteilnehmer (UP). Die Organisation der Einrichtung sowie der Umfang und der Umfang der dort erbrachten Dienstleistungen berücksichtigen die Freiheit, Intimität, Würde und Sicherheit der Teilnehmer sowie den Grad ihrer körperlichen und geistigen Eignung. Im Rahmen des Aufenthaltes an der DDP UP werden Pflegedienste und Aktivierungen im physischen, intellektuellen und sozialen Bereich angeboten, die vom Manager von DDP, Betreuern von Menschen mit Behinderungen, Psychologen, Physiotherapeuten und Therapeuten bereitgestellt werden.
