Wsparciem w ramach Projektu zostaną objęte osoby wykluczone i zagrożone wykluczeniem społecznym z powodu niepełnosprawności, ubóstwa, bezrobocia, nieporadności społecznej, zawodowej z terenu miasta Radomia. GRUPA DOCELOWA: 25 os. (11K/14M) bezrobotne/bierne zawodowo w wieku aktywności zawodowej, korzystające ze świadczeń pomocy społecznej zg. z ustawą o pomocy społecznej lub kwalifikujące się do objęcia wsparciem pomocy społecznej. CEL GŁ.projektu jest integracja społeczna oraz aktywizacja zawodowa os.wykluczonych i zagrożonych wykluczeniem społ.poprzez zwiększenie dostępności do usług społeczno-zawodowych do końca XII 2019 roku. Uczestnicy/czki projektu zostaną objęci ścieżką reintegracji: aktywizacją społeczną, edukacyjną i zawodową. Indywidualna ścieżka reintegracji, będzie planem działania, wg którego uczestnik/czka zostanie poprowadzony/a w dalszych etapach projektu. ZADANIA: 1. Wypracowanie indywidualnej ścieżki reintegracji: • Diagnoza indywidualnych potrzeb 25 uczestników/czek projektu. Przeprowadzenie diagnoz psychospołecznych i funkcjonalnych w celu przygotowania i realizacji wsparcia w oparciu o ścieżkę reintegracji 2. Działania w ramach aktywizacji społecznej: • specjalistyczne poradnictwo: psychologiczne, prawne, • trening umiejętności społecznych • warsztaty rozwoju osobistego • wydarzenia kulturalno- integracyjne: wyjście do kina, teatru, muzeum 3. Działania w ramach aktywizacji edukacyjnej: • szkolenia/kursy dające nowe kompetencje zawodowe (katalog 10 kursów/szkoleń do wyboru zgodnie ze ścieżką reintegracji i IPD) 4. Działania w ramach aktywizacji zawodowej: • doradztwo zawodowe • pośrednictwo pracy • warsztaty z zakresu umiejętności poszukiwania pracy i wiedzy na temat rynku pracy • indywidualne zajęcia z coach-em kariery • indywidualne zajęcia z trenerem pracy WSKAŹNIKI: opis w punkcie C2.1.1 Wszystkie zakupy realizowane z koncepcją uniwersalnego projektowania. The project will be supported by people excluded and at risk of social exclusion due to disability, poverty, unemployment, social incorrigence and occupational disabilities from the city of Radom. TARGET GROUP: 25 wasps. (11K/14M) unemployed/passive in working age benefiting from social assistance in accordance with the Social Assistance Act or eligible for social assistance support. The aim of the project is to social inclusion and professional activation of people excluded and at risk of exclusion by increasing access to socio-occupational services by the end of XII 2019. Participants/participants of the project will be included in the reintegration path: social, educational and professional activation. The individual path of reintegration will be an action plan according to which the participant will be guided and in the further stages of the project. TASKS: 1. Developing an individual path of reintegration: • Diagnosis of individual needs of 25 participants/pieces of the project. Performing psychosocial and functional diagnosis to prepare and implement support based on the path of reintegration 2. Social activation activities: • specialised advice: psychological, legal, • social skills training • personal development workshops • cultural and integration events: exit to the cinema, theatre, museum 3. Activities within the framework of educational activation: • training/courses giving new professional competences (directory of 10 courses/training courses to choose according to the path of reintegration and IPD) 4. Le projet soutiendra les personnes exclues et menacées d’exclusion sociale en raison du handicap, de la pauvreté, du chômage, de l’incapacité sociale et professionnelle de la ville de Radom. GROUPE CIBLE: 25 personnes (11K/14M) chômeurs/employés en âge de travailler, recevant des prestations d’assistance sociale en vertu de la loi sur l’assistance sociale ou pouvant bénéficier d’une aide sociale. L’objectif du projet est l’intégration sociale et l’activation professionnelle des exclus et des menaces d’exclusion sociale en améliorant l’accès aux services socioprofessionnels d’ici la fin décembre 2019. Les participants/femmes du projet seront inclus dans le parcours de réintégration: activation sociale, éducative et professionnelle. Le parcours individuel de réintégration sera un plan d’action selon lequel le participant sera guidé dans les étapes ultérieures du projet. TÂCHES: 1. Développement d’un parcours de réinsertion individuelle: • Diagnostic des besoins individuels de 25 participants/vérifications du projet. Réalisation d’un diagnostic psychosocial et fonctionnel afin de préparer et de mettre en œuvre un soutien basé sur la voie de réintégration 2. Mesures d’activation sociale: • conseil spécialisé: psychologique, juridique, • formation des compétences sociales • ateliers de développement personnel • événements culturels et d’intégration: sortie au cinéma, théâtre, musée 3. Activités éducatives d’activation: • formation/cours donnant de nouvelles compétences professionnelles (catalogue de 10 cours/formation à sélectionner en fonction du parcours de réinsertion et de la DPI) 4. Mesures d’activation: • orientation professionnelle • placement d’emploi • compétences et connaissances en recherche d’emploi sur le marché du travail • cours individuels d’entraîneurs de carrière • classes individuelles avec formateur d’emploi WSKAZENIKI: description au point C2.1.1 Tous les achats effectués avec le concept de conception universelle. Das Projekt wird Menschen unterstützen, die aufgrund von Behinderung, Armut, Arbeitslosigkeit, sozialer und beruflicher Arbeitsunfähigkeit aus der Stadt Radom ausgeschlossen und von sozialer Ausgrenzung bedroht sind. ZIELGRUPPE: 25 Personen (11K/14M) Arbeitslose/erwerbstätige Personen im erwerbsfähigen Alter, die Sozialhilfeleistungen nach dem Sozialhilfegesetz erhalten oder Sozialhilfe erhalten. Ziel des Projekts ist die soziale Integration und die berufliche Aktivierung ausgegrenzter und bedrohter sozialer Ausgrenzung, indem der Zugang zu sozialen und beruflichen Dienstleistungen bis Ende Dezember 2019 verbessert wird. Die Teilnehmer/Frauen des Projekts werden in den Wiedereingliederungspfad einbezogen: soziale, pädagogische und berufliche Aktivierung. Der individuelle Wiedereingliederungspfad ist ein Aktionsplan, nach dem der Teilnehmer in den folgenden Phasen des Projekts geführt wird. AUFGABEN: 1. Entwicklung eines individuellen Wiedereingliederungspfads: • Diagnose der individuellen Bedürfnisse von 25 Teilnehmern/Kontrollen des Projekts. Durchführung psychosozialer und funktioneller Diagnosen zur Vorbereitung und Umsetzung der Unterstützung auf der Grundlage des Wiedereingliederungspfads 2. Maßnahmen zur sozialen Aktivierung: • Fachberatung: psychologische, rechtliche, • Ausbildung sozialer Kompetenzen • persönliche Entwicklungsworkshops • Kultur- und Integrationsveranstaltungen: Ausgang zum Kino, Theater, Museum 3. Aktivitäten zur Aktivierung des Bildungswesens: • Ausbildung/Kurse, die neue berufliche Kompetenzen vermitteln (Katalog von 10 Kursen/Ausbildungen, die nach dem Wiedereingliederungspfad und IPD ausgewählt werden sollen) 4.
