Cel główny projektu: Poprawiona dostępność i jakość usług społecznych dla 26 osób (23K, 3M) wykluczonych lub zagrożonych ubóstwem i wykluczeniem społecznym z Gminy Kodeń poprzez usługi społeczne w społeczności lokalnej w formie 3 Klubów Seniora w Kodniu, Kostomłotach i Zabłociu do 31.12.2020 r. Główne rezultaty: Liczba wspartych w programie miejsc świadczenia usług społecznych istniejących po zakończeniu projektu – 26 Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami społecznymi świadczonymi w interesie ogólnym w programie – 26 Grupa docelowa: Projekt skierowany jest do 26 os (23K i 3M) wykluczonych, lub zagrożonych ubóstwem i wykluczeniem społecz, (jako najb potrz kompleksowego wsparcia zg. z diagn), które spełniają warunki: - zamieszkują tereny wiejskie (kryt. Premiuj nr 5), - są mieszkańcami Gminy Kodeń: Kostomłoty – 8 os (5K, 3M), Zabłocie – 8os (8K, 0M), Kodeń – 10 os (10K, 0M) - mają ukończone 60 lat. Klub Seniora będzie miejscem spotkań osób starszych, w nieformalnej atmosferze, z jak największym spontanicznym zaangażowaniem samych seniorów (uczestnicy kluby podejmują odpowiedzialność/decyzyjność w sprawach klubu). Działania klubu będą integrować seniorów i wspierać aktywność osób starszych, ale przede wszystkim zapewniać im dobre towarzystwo, w pobliżu miejsca zamieszkania. Klub pozwoli na kształtowanie wśród seniorów nawyków do czynnego, kreatywnego spędzania wolnego czasu, rozwoju umiejętności, nauki radzenia sobie w nowych sytuacjach oraz dostosowania się do realiów obecnego świata. Uczestnicy mogą spędzać czas w klubie w pełnym wymiarze lub brać udział w wybranych zajęciach/aktywnościach. Zadania: 1. Main objective of the project: Improved accessibility and quality of social services for 26 people (23K, 3M) excluded or at risk of poverty and social exclusion from the commune of Kodeń through social services in the local community in the form of 3 Senior Clubs in Kodń, Kostomłoty and Zabłocie until 31.12.2020. Number of post-project social services supported by the programme – 26 Number of people at risk of poverty or social exclusion covered by social services of general interest in the programme – 26 Target group: The project is aimed at 26 people (23K and 3M) excluded, or at risk of poverty and exclusion, (as the first time comprehensive support from Diagn) which meets the following conditions: — inhabit rural areas (crit). The Senior Club will be a meeting place for the elderly, in an informal atmosphere, with the greatest possible spontaneous involvement of the seniors themselves (the club participants take responsibility/decision in club matters). The activities of the club will integrate seniors and support the activity of the elderly, but above all provide them with good company, close to their place of residence. The club will allow to shape among seniors habits of active, creative leisure time, development of skills, learning to cope in new situations and adapt to the reality of the present world. Objectif principal du projet: Amélioration de l’accessibilité et de la qualité des services sociaux pour 26 personnes (23K, 3M) exclues ou menacées de pauvreté et d’exclusion sociale de la commune de Kodeń grâce aux services sociaux de la communauté locale, sous la forme de 3 clubs seniors à Kodń, Kostomłoty et Zabłocie d’ici au 31.12.2020. Nombre de sites de prestation de services sociaux post-projet soutenus par le programme — 26 Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale couvertes par les services sociaux d’intérêt général du programme — 26 Groupe cible: Le projet s’adresse à 26 personnes (23 000 et 3 millions d’habitants) exclues ou menacées de pauvreté et d’exclusion sociale (grâce à un soutien global selon le diagn), qui remplissent les conditions suivantes: — ils vivent dans des zones rurales (critique. Le Senior Club sera un lieu de rencontre pour les personnes âgées, dans une atmosphère informelle, avec la plus grande implication spontanée des aînés eux-mêmes (les participants du club prennent la responsabilité/décision dans les affaires du club). Les activités du club intégreront les seniors et favoriseront le vieillissement actif, mais surtout leur fourniront une bonne compagnie, à proximité de leur lieu de résidence. Le club permettra aux aînés de développer leurs habitudes de loisirs actifs et créatifs, de développer leurs compétences, d’apprendre à faire face à de nouvelles situations et de s’adapter aux réalités du monde actuel. Hauptziel des Projekts: Verbesserung der Zugänglichkeit und Qualität der sozialen Dienstleistungen für 26 Personen (23K, 3M), die bis zum 31.12.2020 von der Gemeinde Kodeń ausgeschlossen oder von Armut und sozialer Ausgrenzung bedroht sind, durch soziale Dienstleistungen in der lokalen Gemeinschaft in Form von 3 Senior Clubs in Kodń, Kostomłoty und Zabłocie. Zahl der im Rahmen des Programms unterstützten Einrichtungen zur Erbringung sozialer Dienstleistungen nach dem Projekt – 26 Anzahl der von Armut oder sozialer Ausgrenzung bedrohten Personen, die von sozialen Dienstleistungen von allgemeinem Interesse im Rahmen des Programms abgedeckt sind – 26 Zielgruppe: Das Projekt richtet sich an 26 Personen (23K und 3M), die von Armut und sozialer Ausgrenzung (als Ergebnis einer umfassenden Unterstützung nach dem Diagn) ausgeschlossen sind oder von Armut und sozialer Ausgrenzung bedroht sind und folgende Bedingungen erfüllen: — sie leben in ländlichen Gebieten (kritisch. Bonus Nr. 5), – sind Einwohner der Gemeinde Kodeń: Kostomłoty – 8 Personen (5K, 3M), Zabłocie – 8os (8K, 0M), Koden – 10 os (10K, 0M) – sind mindestens 60 Jahre alt. Der Senior Club wird ein Treffpunkt für ältere Menschen in einer informellen Atmosphäre sein, unter möglichst spontaner Beteiligung der Senioren selbst (die Teilnehmer des Vereins übernehmen Verantwortung/Entscheidung in den Angelegenheiten des Vereins). Die Aktivitäten des Vereins werden Senioren integrieren und aktives Altern fördern, aber vor allem bieten ihnen ein gutes Unternehmen, nahe an dem Ort, wo sie leben. Der Verein wird es Senioren ermöglichen, ihre Gewohnheiten für aktive, kreative Freizeit, Kompetenzentwicklung, das Lernen zu entwickeln, neue Situationen zu bewältigen und sich an die Realitäten der heutigen Welt anzupassen. Die Teilnehmer können Zeit im Club auf Vollzeitbasis verbringen oder an ausgewählten Aktivitäten/Aktivitäten teilnehmen. Aufgaben: 1.
