Celem głównym projektu jest zwiększona dostępności do wysokiej jakości świadczeń zdrowotnych realizowanych w ramach podstawowej opieki zdrowotnej (POZ) przez NZOZ "Zdrowie" s.c. Izabela Dorota Anasiewicz-Kostrzewa, Maria Kokoszka w Janowie Lubelskim. Przedmiot projektu obejmuje rozszerzenie i uzupełnienie zakresu diagnostyki obrazowej realizowanej w POZ poprzez zakup przenośnego ultrasonografu wraz z niezbędnym osprzętem komputerowym (laptop, przenośna drukarka). Katalog wydatków uzupełniają: opracowanie Studium Wykonalności i działania promocyjne. Realizacja inwestycji pozwoli na wykonywanie badań USG przez lekarza POZ podczas wizyty w domu pacjenta. Ma to kluczowe znaczenie dla tych osób, które ze względu na stan zdrowia, niepełnosprawność czy podeszły wiek (grupa osób zależnych) nie mają możliwości skorzystania ze stacjonarnych form diagnostyki. Inwestycji wykazuje spójność z założeniami Narzędzia 14 Policy Paper dla ochrony zdrowia jak również zapisami map potrzeb zdrowotnych w obszarze chorób: nowotworowych, kardiologicznych, gruczołów wydzielania wewnętrznego, cukrzycy, nowotworów niezłośliwych, metabolicznych, układu moczowo - płciowego u mężczyzn i kobiet, wątroby i trzustki. Kluczowe wskaźniki realizacji projektu to: Ludność objęta ulepszonymi usługami zdrowotnymi (CI36) - 12 138 osób; Liczba wspartych podmiotów leczniczych - 1szt.; Nakłady inwestycyjne na zakup aparatury medycznej - 91 666,67 zł. Grupę docelową projektu stanowią pacjenci NZOZ. Projekt ma charakter nieinfrastrukturalny (w rozumieniu procedur OOŚ). (Polish)

The main objective of the project is to increase the availability of high-quality health care provided by NZOZ “Zdrowie” s.c. Izabela Dorota Anasiewicz-Kostrzewa, Maria Kokoszka in Janów Lubelski. The subject of the project includes extending and supplementing the scope of image diagnostics carried out in GE by purchasing a portable ultrasound with the necessary hardware (laptop, portable printer). The expenditure catalogue shall complete: development of the Feasibility Study and promotional activities. The implementation of the investment will allow the GP to perform ultrasound tests during a visit to the patient’s home. This is crucial for those who, due to their health, disability or old age (a group of dependent persons) are not able to benefit from stationary forms of diagnostics. The investment is consistent with the objectives of the Health Tools 14 Policy Paper as well as the records of health needs maps in the field of diseases: neoplasms, cardiology, internal secretion glands, diabetes mellitus, non-malignant, metabolic, ureteral and genital cancers in men and women, liver and pancreas. Key indicators for the implementation of the project are: Population covered by improved health services (CI36) – 12,138 people; Number of medicinal entities supported – 1 pieces; Investment outlays for the purchase of medical equipment – PLN 91 666.67. The target group of the project is NZOZ patients. The project is non-infrastructured (within the meaning of EIA procedures). (English)

L’objectif principal du projet est d’améliorer l’accès à des services de santé de haute qualité fournis dans le cadre des soins de santé primaires (POZ) par NZOZ «Santé» s.c. Izabela Dorota Anasiewicz-Kostrzewa, Maria Kokoszka à Janów Lubelski. L’objet du projet comprend l’extension et le complément du champ d’application des diagnostics d’imagerie réalisés dans le POZ en achetant une échographie portable avec le matériel nécessaire (ordinateur portable, imprimante portable). La liste des dépenses est complétée par: élaboration de l’étude de faisabilité et activités promotionnelles. La mise en œuvre de l’investissement vous permettra d’effectuer des examens échographiques par un médecin POZ lors d’une visite chez le patient. C’est essentiel pour ceux qui, en raison de leur état de santé, de leur handicap ou de leur vieillesse (groupe de personnes à charge) ne sont pas en mesure d’utiliser des diagnostics fixes. L’investissement est conforme aux hypothèses du 14 Policy Paper for Health and Health Needs Maps dans le domaine des maladies: cancer, cardiaque, glandes de sécrétion interne, diabète, système bénin, métabolique, génito-urinaire chez les hommes et les femmes, foie et pancréas. Les principaux indicateurs de mise en œuvre des projets sont les suivants: Population couverte par l’amélioration des services de santé (CI36) — 12 138; Nombre d’entités médicinales soutenues — 1 poste; Dépenses d’investissement pour l’achat de matériel médical — 91 666,67 PLN. Hauptziel des Projekts ist es, den Zugang zu hochwertigen Gesundheitsdienstleistungen im Rahmen der primären Gesundheitsversorgung (POZ) durch NZOZ „Health“ s.c. Izabela Dorota Anasiewicz-Kostrzewa, Maria Kokoszka in Janów Lubelski zu verbessern. Gegenstand des Projekts ist die Erweiterung und Ergänzung des Anwendungsbereichs der bildgebenden Diagnostik im POZ durch den Kauf eines tragbaren Ultraschalls mit der erforderlichen Hardware (Laptop, tragbarer Drucker). Die Liste der Ausgaben wird ergänzt durch: Entwicklung der Machbarkeitsstudie und Werbemaßnahmen. Mit der Durchführung der Investition können Sie Ultraschalluntersuchungen durch einen POZ-Arzt während eines Besuchs im Haus des Patienten durchführen. Dies ist von entscheidender Bedeutung für diejenigen, die aufgrund ihres Gesundheitszustands, ihrer Behinderung oder ihres Alters (Gruppe der abhängigen Personen) nicht in der Lage sind, stationäre Diagnosen zu verwenden. Die Investition steht im Einklang mit den Annahmen des 14 Policy Paper for Health and Health Needs Maps im Bereich der Krankheiten: Krebs, Herz, innere Sekretionsdrüsen, Diabetes, gutartig, metabolisches, genitourinäres System bei Männern und Frauen, Leber und Bauchspeicheldrüsen.
