Projekt obejmuje rozwój opieki koordynowanej w przychodni miejskiej „Remedium”, zarządzanej przez Centrum Medyczne Sp. z o.o. i zakłada wykonanie remontu infrastruktury budynku, celem zwiększenia dostępności do opieki zdrowotnej osób starszych oraz osób niepełnosprawnych w ramach POZ i AOS oraz zakup wyposażenia niezbędnego do udzielania świadczeń medycznych. Realizacja projektu przyczyni się do zwiększenia dostępności i jakości usług podstawowej opieki zdrowotnej i ambulatoryjnej opieki specjalistycznej realizowanych w zakresie opieki koordynowanej. Projekt ma na celu realizację następujących zadań: • Prace budowlane • Zakup wyposażenia- sprzęt medyczny • Zakup wyposażenia- sprzęt informatyczny i oprogramowanie. Realizacja projektu pozwoli na osiągnięcie następujących wskaźników: - Liczba wspartych podmiotów leczniczych– 1 szt. - Nakłady inwestycyjne na zakup aparatury medycznej –282 183,10zł - Liczba leczonych w podmiotach leczniczych objętych wsparciem – 8575os. - Ludność objęta ulepszonymi usługami zdrowotnymi (CI36) – 8618os. - Liczba obiektów dostosowanych do potrzeb osób z niepełnosprawnościami – 1szt. Przedmiotowy projekt skierowany jest do mieszkańców powiatu zgierskiego, w szczególności będących pacjentami CM Sp. z o.o. Wszelkie działania związane z realizacją projektu jak i jego trwałością, będą prowadzone w myśl projektowania uniwersalnego. (Polish)

The project includes the development of integrated care in the “Remedium” municipal clinic, managed by Centrum Medyczne Sp. z o.o. and involves repairing the building’s infrastructure, in order to increase accessibility to health care of elderly people and people with disabilities within the framework of POZ and AOS and purchase of equipment necessary to provide medical services. The implementation of the project will contribute to increasing the availability and quality of primary healthcare services and outpatient specialist care provided in the area of integrated care. The aim of the project is to carry out the following tasks: • Construction works • Purchase of equipment- medical equipment • Purchase of equipment- IT equipment and software. The implementation of the project will allow the following indicators to be achieved: — Number of supported medicinal entities – 1 pcs. — Investment outlays on the purchase of medical equipment – PLN 282 183.10 PLN – Number of treated in medical entities supported – 8575os. — Population covered by improved health services (CI36) – 8618person. — Number of facilities tailored to the needs of people with disabilities – 1pcs. This project is addressed to residents of the Zgierski county, in particular patients of CM Sp. z o.o. All activities related to the implementation of the project and its durability will be carried out in accordance with universal design. (English)

Le projet comprend le développement des soins coordonnés dans la clinique municipale «Remedium», gérée par Centrum Medicale Sp. z o.o., et comprend la rénovation de l’infrastructure du bâtiment, afin d’améliorer l’accès aux soins de santé pour les personnes âgées et les personnes handicapées dans le cadre de POZ et AOS et d’acheter l’équipement nécessaire à la prestation de services médicaux. La mise en œuvre du projet contribuera à accroître la disponibilité et la qualité des services spécialisés de soins primaires et ambulatoires fournis dans le domaine des soins coordonnés. Le projet vise à réaliser les tâches suivantes: • Travaux de construction • achat d’équipement- matériel médical • achat d’équipements- matériel informatique et logiciels. La mise en œuvre du projet permettra d’atteindre les indicateurs suivants: — Nombre d’entités médicinales prises en charge — 1 pcs. — Dépenses d’investissement pour l’achat de matériel médical — 282 183.10 PLN — Nombre de traitements dans les entités médicinales bénéficiaires — 8575 personnes. — Population couverte par l’amélioration des services de santé (CI36) — 8618 personnes. — Nombre d’installations adaptées aux besoins des personnes handicapées — 1 pcs. Le projet s’adresse aux résidents du district de Zgierski, en particulier à ceux qui sont patients de CM Sp. z o.o. Toutes les activités liées à la mise en œuvre du projet et à sa durabilité seront menées dans le respect de la conception universelle. (French)

Das Projekt umfasst die Entwicklung der von Centrum Medicale Sp. z o.o. geleiteten Klinik „Remedium“ und umfasst die Renovierung der Gebäudeinfrastruktur, um den Zugang zur Gesundheitsversorgung für ältere Menschen und Menschen mit Behinderungen im Rahmen von POZ und AOS zu verbessern und die für die Bereitstellung medizinischer Dienstleistungen notwendige Ausrüstung zu erwerben. Die Durchführung des Projekts wird dazu beitragen, die Verfügbarkeit und Qualität der primären und ambulanten spezialisierten Betreuungsdienste im Bereich der koordinierten Versorgung zu verbessern. Ziel des Projekts ist es, folgende Aufgaben zu erfüllen: • Bauarbeiten • Kauf von Geräten- medizinische Ausrüstung • Kauf von Geräten- IT-Ausrüstung und Software. Durch die Durchführung des Projekts können folgende Indikatoren erreicht werden: — Anzahl der unterstützten medizinischen Einrichtungen – 1 Stk. — Investitionsausgaben für den Kauf von medizinischer Ausrüstung – 282 183,10 PLN – Anzahl der Behandlungen in den begünstigten medizinischen Einrichtungen – 8575 Personen. — Bevölkerung mit verbesserter Gesundheitsversorgung (CI36) – 8618 Personen. — Anzahl der an die Bedürfnisse von Menschen mit Behinderungen angepassten Einrichtungen – 1 Stk. Das Projekt richtet sich an Bewohner des Bezirks Zgierski, insbesondere an Patienten der CM Sp. z o.o.. Alle Aktivitäten im Zusammenhang mit der Umsetzung des Projekts und seiner Nachhaltigkeit werden nach universeller Gestaltung durchgeführt. (German)
