Głównym celem projektu jest zwiększenie dostępności do usług opiekuńczych dla 90osób (75K,15M) na terenie Gminy Lubraniec (woj. kujawsko-pomorskie) w terminie 01.05.2019-30.04.2021, poprzez utworzenie 60 nowych miejsc świadczenia dziennych usług opiekuńczych w nowo powstałych Klubach Seniora oraz 1 miejsca świadczenia usług opiekuńczych w miejscu zamieszkania w ramach zatrudnienia 1 opiekuna. Cel ten przyczyni się do realizacji celu szczegółowego tj. Zwiększenie dostępności usług społecznych w szczególności usług środowiskowych, opiekuńczych oraz usług wsparcia rodziny i pieczy zastępczej dla osób zagrożonych ubóstwem lub wykluczeniem społecznym (Cel szczegółowy 3). Grupa docelowa: - 90 os.niesam., zagrożonych ubóstwem lub wykluczeniem społ. (75K) - osób niesamodzielnych, mieszk. w rozum. KC na terenie woj.kujawsko-pom.Zaplanowane zadania:1. Utworzenie i bieżące funkcjonowanie 15 miejsc świadczenia dziennych usług op.w formie Klubu Seniora w Siemnówku 2. Utworzenie i bieżące funkcjonowanie 15 miejsc świadczenia dziennych usług opiek. w formie Klubu Seniora w Ossowie3. Utworzenie i bieżące funkcjonowanie 30 miejsc świadczenia dziennych usług op. w formie Klubu Seniora w Lubrańcu 4. Zwiększenie liczby miejsc świad. usług opiek. w miejscu zamiesz. oraz zapewnienie teleopieki na terenie Gminy Lubraniec.5. Realizacja wyjazdów kulturalnych dla 40 seniorów z Klubów Seniora w Gminie Lubraniec. Główne założenie/efekty:-Utworzenie 60 nowych miejsc Klubie Seniora w Gminie Lubraniec.-Zwiększenia liczby miejsc świadczenia usług społecznych w społeczności lokalnej poprzez zatrudnienie 1 opiekuna oraz liczby osób objętych usługami świadczonymi w społeczności lokalnej (5 os. w tym 5 k) na terenie Gminy Lubraniec, - Wsparcie 5 (K) osób zagrożonych ubóstwem lub wykluczeniem społecznym w ramach dziennych usług opiekuńczych. -Wsparcie 30 osób w postaci teleopieki. The main objective of the project is to increase the availability of care services for 90 persons (75K,15M) in the municipality of Lubraniec (Kujawsko-Pomorskie Voivodeship) within the period 01.05.2019-30.04.2021, by creating 60 new places for providing day care services in newly established Senior Clubs and 1 places of care services in the place of residence within the framework of 1 caregiver employment. This objective will contribute to the specific objective, i.e. Increase the availability of social services in particular environmental, care and family support and foster care services for those at risk of poverty or social exclusion (Specific Objective 3). Target group: — 90 people at risk of poverty or exclusion. (75K) – dependants, in mind. KC on the territory of the Kujawsko-pom voivodship. Creation and current functioning of 15 places of daily services op. in the form of the Senior Club in Siemnów 2. Creation and current operation of 15 places to provide day care services in the form of Senior Club in Ossow3. Creation and current operation of 30 places of daily services op. in the form of the Senior Club in Lubraniec 4. Increase the number of places of witness care services in the place of the riot and provide telecare in the municipality of Lubraniec.5. Realisation of cultural trips for 40 seniors from the Senior Clubs in Lubraniec. Main assumption/effects:-Creating 60 new places of the Senior Club in Lubraniec.-Increasing the number of places to provide social services in the local community by employing 1 guardians and the number of people covered by services provided in the local community (5 people including 5 k) in the municipality of Lubraniec, – Support 5 (K) of those at risk of poverty or social exclusion as part of daily care services. —Supporting 30 people in the form of telecare. L’objectif principal du projet est d’accroître la disponibilité des services de soins pour 90 personnes (75K,15M) dans la commune de Lubraniec (province de Kujawsko-Pomorskie) entre le 01.05.2019-30.04.2021, en créant 60 nouveaux lieux de prestation de soins quotidiens dans les clubs seniors nouvellement créés et un lieu de soins dans le lieu de résidence dans le cadre de l’emploi d’une personne soignante. Cet objectif contribuera à la réalisation de l’objectif spécifique, à savoir: Accroître la disponibilité des services sociaux, en particulier les services d’aide à l’environnement, de protection de la famille et de placement familial pour les personnes exposées au risque de pauvreté ou d’exclusion sociale (objectif spécifique 3). Création et fonctionnement actuel de 30 places offrant des services quotidiens op. sous forme de Senior Club à Lubrańcu 4. Augmenter le nombre de lieux de garde des témoins sur le lieu de résidence et assurer la téléassistance à Lubraniec Commune.5. Mise en œuvre de voyages culturels pour 40 aînés provenant de clubs seniors de la municipalité de Lubraniec. Principaux objectifs/effets:-Créer 60 nouveaux sièges du Club senior dans la municipalité de Lubraniec.- Augmenter le nombre de places pour fournir des services sociaux dans la communauté locale en employant 1 personne soignante et le nombre de personnes couvertes par les services fournis dans la communauté locale (5 personnes dont 5 k) dans la municipalité de Lubraniec, — Soutien à 5 (K) personnes exposées au risque de pauvreté ou d’exclusion sociale dans le cadre des services de soins quotidiens. —Soutien de 30 personnes sous forme de télésoins. Einrichtung und laufender Betrieb von 15 Plätzen der täglichen Betreuung in Form von Senior Club in Ossów3. Schaffung und aktueller Betrieb von 30 Plätzen, die täglich Dienstleistungen anbieten, op. in Form von Senior Club in Lubrańcu 4. Erhöhung der Zahl der Orte der Zeugenbetreuung am Wohnort und Bereitstellung von Telecare in Lubraniec Commune.5. Durchführung von Kulturreisen für 40 Senioren aus Senior Clubs in der Gemeinde Lubraniec. Hauptziel/Effekte: Schaffung von 60 neuen Senior Club-Sitzen in der Gemeinde Lubraniec.-Erhöhung der Zahl der Plätze für soziale Dienstleistungen in der lokalen Gemeinschaft durch die Beschäftigung von 1 Betreuer und die Zahl der Personen, die in der Gemeinde Lubraniec von Dienstleistungen erbracht werden (5 Personen, davon 5 k) in der Gemeinde Lubraniec, – Unterstützung von 5 (K) Menschen, die von Armut oder sozialer Ausgrenzung im Rahmen der täglichen Betreuungsleistungen bedroht sind.
