Celem gł. proj. jest przetestowanie i wdrożenie kompleksowego i zindywidualizowanego modelu środowiskowego wsparcia 1025 dorosłych os. z niepełnosprawnością intelektualną, osób z całościowymi zaburzeniami rozwoju, z niepełnosprawnością sprzężoną (OzNI) oraz ich rodzin/opiekunów/otoczenia, opracowanego w ramach I etapu konkursu, w okresie trwania projektu.Model wdrażany będzie w 4-ch dzielnicach m.st. Warszawy: Bemowo, Białołęka, Wilanów, Wola w zakresie wsparcia OzNI a także pracy ze społecznościami lokalnymi oraz na poziomie ogólnomiejskim, w zakresie koordynacji systemu wsparcia, wymiany informacji, monitoringu i wewnętrznej ewaluacji wsparcia.Celem Modelu jest koordynacja usług dostępnych na obszarze m.st.Warszawy i utworzenie systemu wsparcia dla OzNI i ich rodzin, który dzięki wykorzystaniu ich zasobów umożliwi im samodzielne życie na miarę ich możliwości i realizację w życiu prywatnym, społecznym i zawodowym.W ramach testowania Modelu zostaną m.in. utworzone:- Centrum koordynacji Punkt Informacyjno-Koordynacyjny dla Osób z Niepełnosprawnością Intelektualną (PIKONI);- nowe stanowisko -Asystent rodziny OzNI przy ośrodkach pomocy społecznej (OPS-ach);- nowe stanowisko -Organizator społeczności lokalnej na rzecz OzNI przy OPS-ach;- 4 mieszkania wspomagane dla różnych grup odbiorców (jedno mieszkanie interwencyjno-wytchnieniowe oraz po jednym dla OzNI w stopniu lekkim, z całościowymi zaburzeniami rozwoju i dla OzNI sprzężoną).- wsparcie w mieszkaniu prywatnym OzNI,W ramach Modelu duży nacisk postawiono też na współpracę z użytkownikami Modelu jakimi są podmioty, które działają na rzecz odbiorców zarówno na poziomie ogólnomiejskim jak i dzielnicowym.Proj. jest ogólnodostępny, w przypadku zakwalifikowania do projektu OzNI o specjalnych potrzebach funkcjonalnych przewidywana jest możliwość uruchomienia mechanizmu racjonalnych usprawnień (np. dostos. akustycznego, asystenta tłumaczącego na język łatwy, tłumacza języka migowego, przewodnika dla osób słabowidzących,itp). L’objectif du projet est de tester et de mettre en œuvre un modèle de soutien environnemental complet et individualisé pour 1025 adultes souffrant de handicaps intellectuels, les personnes souffrant de troubles du développement globaux, les personnes souffrant de handicaps associés (Ozni) et leurs familles/tuteurs/extérieurs, développé dans le cadre de la première étape du concours, pendant la durée du projet. Le modèle sera mis en œuvre dans 4 districts de Varsovie: Bemowo, Białołęka, Wilanów, Wola dans le domaine du soutien d’Ozni, ainsi qu’avec les communautés locales et au niveau municipal, dans le domaine de la coordination du système de soutien, de l’échange d’informations, du suivi et de l’évaluation interne de l’aide. qui, grâce à l’utilisation de leurs ressources, leur permettra de vivre de manière autonome en fonction de leurs capacités et de leur mise en œuvre dans la vie privée, sociale et professionnelle.Une partie de l’essai modèle sera créée, entre autres:- un centre de coordination pour le Centre d’information et de coordination pour les personnes handicapées intellectuelles (PIKONI); un nouveau poste — Assistant de la famille Ozni dans les centres d’assistance sociale (OPS-ach);- nouveau poste — organisateur local d’Ozni à Ozni à l’OPS;-- 4 appartements assistés pour différents groupes de bénéficiaires (une intervention et un appartement pour Ozni dans un degré léger, avec des troubles de développement globaux et pour Ozni couplé).- le soutien dans un appartement privé Ozni, le modèle met également l’accent sur la coopération avec les utilisateurs du modèle, qui sont des entités qui agissent au bénéfice du public à la fois au niveau urbain et au niveau du district. Le projet est ouvert au public, dans le cas de la qualification pour le projet Ozni avec des besoins fonctionnels spéciaux, la possibilité de lancer un mécanisme d’aménagement raisonnable (par exemple adaptation acoustique, interprète facile en langue des signes, guide pour les personnes ayant une déficience visuelle, etc.) est envisagée. Ziel des Projekts ist es, ein umfassendes und individualisiertes Umweltförderungsmodell für 1025 Erwachsene mit geistiger Behinderung, Menschen mit allgemeinen Entwicklungsstörungen, mit damit verbundenen Behinderungen (Ozni) und deren Familien/Hüter/Außenseite zu testen und umzusetzen, das während der Laufzeit des Projekts im Rahmen der ersten Phase des Wettbewerbs entwickelt wurde. Das Modell wird in 4 Bezirken von Warschau umgesetzt: Bemowo, Białołęka, Wilanów, Wola im Bereich der Ozni-Unterstützung sowie die Zusammenarbeit mit lokalen Gemeinschaften und auf kommunaler Ebene im Bereich der Koordinierung des Unterstützungssystems, des Informationsaustauschs, der Überwachung und der internen Evaluierung der Unterstützung. Ziel des Modells ist es, die im Bereich Warschau verfügbaren Dienste zu koordinieren und ein Unterstützungssystem für Ozni und ihre Familien zu schaffen, das dank der Nutzung ihrer Ressourcen wird es ihnen ermöglichen, unabhängig von ihren Fähigkeiten und der Umsetzung im privaten, sozialen und beruflichen Leben zu leben.Ein Teil der Modelltests wird unter anderem geschaffen:- ein Koordinierungszentrum für Informations- und Koordinierungszentrum für Menschen mit geistiger Behinderung (PIKONI); neue Position – Assistent der Ozni Familie in Sozialhilfezentren (OPS-ach);- neue Position – Lokale Gemeindeorganisator für Ozni bei OPS;- 4 unterstützte Wohnungen für verschiedene Gruppen von Empfängern (eine Intervention und eine Wohnung für Ozni in einem leichten Grad, mit allgemeinen Entwicklungsstörungen und für Ozni gekoppelt).- Unterstützung in einer Privatwohnung Ozni, das Modell legt auch großen Wert auf die Zusammenarbeit mit Benutzern des Modells, die Einrichtungen sind, die für die Öffentlichkeit sowohl auf städtischer als auch auf Bezirksebene handeln. Das Projekt ist offen für die Öffentlichkeit, im Falle der Qualifikation für das Ozni-Projekt mit besonderen funktionalen Bedürfnissen, die Möglichkeit, einen Mechanismus der angemessenen Unterkunft (z. B. akustische Anpassung, leichtsprachige Dolmetscher, Gebärdensprachdolmetscher, Leitfaden für sehbehinderte Menschen, etc.) zu starten. (German)
