Notre opération se compose d'une seule action dénommée : Accompagnement renforcé personnalisé Référent de parcours PLIE A partir d'un diagnostic préalable pourra être proposé au participant : \- Une intégration dans un parcours PLIE proposition d'intégration qui sera validée par le Comité de suivi PLIE du territoire. \- Une réorientation vers une structure sociale ou de soins du territoire ou vers Pôle emploi si la personne ne relève pas d'un accompagnement renforcé L'accompagnement socio professionnel se mettra en place consécutivement à la proposition d'intégration dans un parcours PLIE, après validation par le Comité de suivi PLIE du territoire. La mise en place du parcours est contractualisée avec le participant (engagement) en prenant en compte le diagnostic réalisé et partagé avec celui- ci. A partir du diagnostic et lors de la contractualisation : des premiers objectifs sont fixés avec le participant. Objectifs qui seront renégociés et réajusté en fonction de l'avancée du participant dans son parcours et au travers la réalisation d'actions, d'étapes. (French)

The implementation of the course is contracted with the participant (commitment) taking into account the diagnosis made and shared with the participant. From the diagnosis and at the time of contracting: initial objectives are set with the participant. Unsere Operation besteht aus einer einzigen Aktion mit der Bezeichnung: Erweiterte persönliche Begleitung PLIE Kursreferenz Ab einer Vordiagnose kann dem Teilnehmer angeboten werden: \- Eine Integration in einen PLIE-Plan Vorschlag zur Integration, der vom PLIE Begleitausschuss des Gebiets bestätigt wird. \- Eine Umorientierung auf eine Sozial- oder Pflegestruktur des Gebiets oder auf den Arbeitsmarkt, wenn die Person nicht unter eine verstärkte Begleitung fällt. Die soziale und berufliche Begleitung erfolgt nach dem Vorschlag für die Integration in einen PLIE-Begleit nach Validierung durch den PLIE-Begleitausschuss des Gebiets. Die Durchführung des Kurses wird mit dem Teilnehmer (Verpflichtung) unter Berücksichtigung der durchgeführten Diagnose vereinbart und mit ihm geteilt. Aus der Diagnose und bei Vertragsabschluss: erste Ziele werden mit dem Teilnehmer festgelegt. Ziele, die neu verhandelt und entsprechend dem Fortschritt des Teilnehmers auf seinem Weg und durch die Durchführung von Aktionen und Etappen angepasst werden. (German)

Onze operatie bestaat uit één enkele actie genaamd: Verbeterde gepersonaliseerde ondersteuning Cursusreferentie PLIE Van een voorafgaande diagnose kan de deelnemer worden aangeboden: \- Integratie in een PLIE-integratievoorstel dat zal worden gevalideerd door het regionale PLIE-toezichtcomité. Een heroriëntatie naar een sociale of zorgstructuur op het grondgebied of naar het arbeidsbureau als de persoon niet onder verhoogde steun valt De sociaal-professionele ondersteuning zal worden ingevoerd na het voorstel voor integratie in een PLIE-traject, na validatie door het PLIE-toezichtcomité van het grondgebied. De uitvoering van de cursus wordt gecontracteerd met de deelnemer (verbintenis) rekening houdend met de diagnose gesteld en gedeeld met de deelnemer. Uit de diagnose en op het moment van contractering: de initiële doelstellingen worden vastgesteld met de deelnemer. Doelstellingen waarover opnieuw zal worden onderhandeld en aangepast aan de voortgang van de deelnemer in zijn/haar reis en door de uitvoering van acties, fasen. (Dutch)
