Проект "Помогни на възрастни хора и лица с увреждания чрез патронажна грижа в община Карлово" е насочен изцяло към възрастните хора над 65 години с ограничения или в невъзможност за самообслужване, както и към хора с увреждания и техните семейства, хора, които нямат близки, за които има социална необходимост от почасови услуги в обичайната им среда.Освен грижи в дома, за тези лица ще се развият и услуги в общността, ориентирани към предотвратяване на самотата и изолацията и адаптирани към основните нужди на възрастните хора, за да бъдат те независими и да останат възможно най-дълго време в своите домове. За лицата с увреждания ще се осигурят специализирани медицински грижи в домашна среда, като това ще спомогне изграждането и утвърждаването на модел за патронажни грижи и предоставяне на почасови мобилни интегрирани здравно-социални услуги. The project “Help elderly people and people with disabilities through patronage care in Karlovo Municipality” is aimed entirely at elderly people over 65 years old with restrictions or inability to self-service, as well as people with disabilities and their families, people who have no relatives, for whom there is a social need for hourly services in their usual environment. Besides home care, these people will develop community services, oriented to prevent loneliness and isolation and adapted to the basic needs of older people, in order to be independent and stay as long as possible in their homes. Specialised home-based medical care will be provided for persons with disabilities, helping to build and validate a model of patronage care and the provision of hourly mobile integrated health-social services. Le projet «Aide aux personnes âgées et aux personnes handicapées par des soins de mécénat dans la municipalité de Karlovo» s’adresse entièrement aux personnes âgées de plus de 65 ans avec des restrictions ou à l’incapacité de s’auto-service, ainsi qu’aux personnes handicapées et à leur famille, aux personnes qui n’ont pas de famille, pour lesquelles il existe un besoin social de services horaires dans leur environnement habituel. Outre les soins à domicile, ces personnes développeront des services communautaires, axés sur la prévention de la solitude et de l’isolement et adaptés aux besoins fondamentaux des personnes âgées, afin d’être indépendants et de rester le plus longtemps possible dans leur foyer. Des soins médicaux spécialisés à domicile seront dispensés aux personnes handicapées, ce qui contribuera à l’élaboration et à la validation d’un modèle de soins de mécénat et à la fourniture de services sociaux intégrés et mobiles horaires. Das Projekt „Hilfe für ältere Menschen und Menschen mit Behinderungen durch Schirmherrschaft in der Gemeinde Karlovo“ richtet sich ausschließlich an ältere Menschen über 65 Jahre mit Einschränkungen oder Unfähigkeit zur Selbstbedienung, sowie Menschen mit Behinderungen und deren Familien, Menschen ohne Verwandte, für die es einen sozialen Bedarf an Stundendiensten in ihrem gewohnten Umfeld gibt. Neben der häuslichen Betreuung werden diese Menschen Gemeinschaftsdienste entwickeln, die darauf ausgerichtet sind, Einsamkeit und Isolation zu verhindern und an die Grundbedürfnisse älterer Menschen anzupassen, um unabhängig zu sein und so lange wie möglich in ihren Häusern zu bleiben. Für Menschen mit Behinderungen wird eine spezialisierte häusliche medizinische Versorgung bereitgestellt, die zum Aufbau und zur Validierung eines Modells der Betreuung der Schirmherrschaft und zur Erbringung von fahrmobilen integrierten Gesundheits- und Sozialdiensten beiträgt.
