Създаване на Звено за почасово предоставяне на услуги за социално включване в общността или в домашна среда в Община Карлово. Чрез реализацията на проектното предложение ще се предоставят интегрирани услуги за хората с невъзможност за самообслужване и за хората с увреждане, като ще се съчетават комплексни действия в посока осигуряване на дългосрочна грижа, включително и социални здравни услуги в общността или в домашна среда. Целева група по проекта: 1. Хора с увреждания и техните семейства. 2. Хора над 65 г. с ограничения или в невъзможност за самообслужване.Проектът ще се реализира чрез дейности и мерки, които ще бъдат изпълнявани в партньорство с организация доставчик на социални услуги СОНИК СТАРТ ООД, за да се постигнат заложените резултати в проектното предложение, както следва: 1. Организация и управление на проекта. Подбор и напасване на потребители и доставчици на социални услуги. 2. Предоставяне на почасови социални услуги в общността и в домашна среда. Осигуряване на достъп до интегрирани услуги, според специфичните потребности на човека в неравностойно положение въз основа на индивидуална социална оценка, чрез подкрепа на дейността на Звено за почасово предоставяне на услуги.3. Обучение на персонала за предоставяне на услугите.4. Предоставяне на психологическа подкрепа и/или консултиране и/или супервизия на персонала, предоставящ социални услуги.5. Мотивационна и/или психологическа или друг тип подкрепа за потребителите на услугите, според тяхната индивидуална потребност.6. Информация и публичност.В резултат на изпълнение на дейностите ще се подобри качеството на живот на възрастните и лицата с увреждания чрез създаване на условия и подкрепа за ефективно упражняване правото им на независим живот и социално включване, при зачитане на правата им, съобразяване с техните възможности и специфични потребности. Establishment of a Unit for hourly provision of services for social inclusion in the community or in a home environment in Karlovo Municipality. Through the implementation of the project proposal, integrated services will be provided for people with inability to self-service and for people with disabilities, combining complex actions towards long-term care, including social health services in the community or in the home environment. Project target group: 1. People with disabilities and their families. 2. People over 65 years old with restrictions or inability to self-service.The project will be implemented through activities and measures, which will be implemented in partnership with a social service provider organisation SONIC START Ltd. in order to achieve the expected results in the project proposal as follows: 1. Organisation and management of the project. Selection and matching of users and social service providers. 2. Provision of hourly social services in the community and in the home environment. Ensuring access to integrated services, according to the specific needs of disadvantaged people on the basis of an individual social assessment, by supporting the activities of the Unit for hourly provision of services.3. Training of staff in the provision of services.4. Provision of psychological support and/or counselling and/or supervision of staff providing social services.5. Motivational and/or psychological or other type of support to service users according to their individual need.6. Information and publicity. As a result of the implementation of the activities, the quality of life of adults and persons with disabilities will be improved by creating conditions and supporting the effective exercise of their right to independent life and social inclusion, while respecting their rights, taking into account their capabilities and specific needs. Création d’une unité de prestation horaire de services d’inclusion sociale dans la communauté ou dans un environnement familial dans la municipalité de Karlovo. Grâce à la mise en œuvre de la proposition de projet, des services intégrés seront fournis aux personnes incapables de se rendre en libre-service et aux personnes handicapées, en combinant des actions complexes en faveur des soins de longue durée, y compris des services de santé sociale dans la collectivité ou à domicile. Groupe cible du projet: 1. Les personnes handicapées et leurs familles. 2. Personnes âgées de plus de 65 ans avec des restrictions ou une incapacité au libre-service.Le projet sera mis en œuvre par le biais d’activités et de mesures, qui seront mises en œuvre en partenariat avec un organisme de services sociaux SONIC START Ltd. afin d’obtenir les résultats escomptés dans la proposition de projet comme suit: 1. Organisation et gestion du projet. Sélection et mise en correspondance des utilisateurs et des prestataires de services sociaux. 2. Prestation de services sociaux horaires dans la collectivité et dans l’environnement familial. Garantir l’accès à des services intégrés, en fonction des besoins spécifiques des personnes défavorisées sur la base d’une évaluation sociale individuelle, en soutenant les activités de l’unité pour la prestation de services horaires.3. Formation du personnel à la prestation de services.4. Fourniture d’un soutien psychologique et/ou d’un conseil et/ou d’une supervision du personnel fournissant des services sociaux.5. Soutien motivé et/ou psychologique ou autre type de soutien aux utilisateurs de services en fonction de leurs besoins individuels.6. Information et publicité: Grâce à la mise en œuvre des activités, la qualité de vie des adultes et des personnes handicapées sera améliorée en créant des conditions et en soutenant l’exercice effectif de leur droit à la vie indépendante et à l’inclusion sociale, tout en respectant leurs droits, en tenant compte de leurs capacités et de leurs besoins spécifiques. Einrichtung einer Einheit für die Erbringung von Dienstleistungen für die soziale Eingliederung in der Gemeinde Karlovo in der Gemeinde Karlovo. Durch die Umsetzung des Projektvorschlags werden integrierte Dienstleistungen für Menschen mit Selbstbedienungsunfähigkeit und für Menschen mit Behinderungen bereitgestellt, die komplexe Maßnahmen zur Langzeitpflege, einschließlich der sozialen Gesundheitsdienste in der Gemeinschaft oder im häuslichen Umfeld, kombinieren. Projektzielgruppe: 1. Menschen mit Behinderungen und ihre Familien. 2. Personen über 65 Jahre mit Einschränkungen oder Unfähigkeit zur Selbstbedienung.Das Projekt wird durch Aktivitäten und Maßnahmen durchgeführt, die in Partnerschaft mit einer Sozialdienstleisterorganisation SONIC START Ltd. durchgeführt werden, um die erwarteten Ergebnisse des Projektvorschlags wie folgt zu erreichen: 1. Organisation und Verwaltung des Projekts. Auswahl und Abstimmung von Nutzern und Sozialdienstleistern. 2. Erbringung von stundenlangen Sozialdiensten in der Gemeinschaft und im häuslichen Umfeld. Gewährleistung des Zugangs zu integrierten Dienstleistungen entsprechend den besonderen Bedürfnissen benachteiligter Menschen auf der Grundlage einer individuellen sozialen Bewertung durch Unterstützung der Tätigkeiten des Referats für die Dienstleistungen pro Stunde.3. Ausbildung des Personals bei der Erbringung von Dienstleistungen.4. Bereitstellung von psychologischer Unterstützung und/oder Beratung und/oder Überwachung des Personals, das soziale Dienstleistungen anbietet.5. Motivation und/oder psychologische oder andere Art von Unterstützung für die Servicenutzer entsprechend ihrer individuellen Bedürfnisse.6. Information und Publizität. Als Ergebnis der Durchführung der Maßnahmen wird die Lebensqualität von Erwachsenen und Menschen mit Behinderungen verbessert, indem Bedingungen geschaffen und die wirksame Ausübung ihres Rechts auf unabhängiges Leben und soziale Inklusion unterstützt werden, wobei ihre Rechte unter Berücksichtigung ihrer Fähigkeiten und spezifischen Bedürfnisse gewahrt werden.
