С реализацията на проектното предложение (ПП) ще се удовлетвори идентифицираната в Плана за действие за периода 2018-2021г. за изпълнение на Нац. стратегия за дългосрочна грижа, необходимостта от изграждане на инфраструктура за разкриване на Център за грижа за хора с увреждания и възрастни хора (лица с психични разстройства, умствена изостаналост, различни форми на деменция и възрастни хора в невъзможност за самообслужване).Целеви групи:-възрастни хора и хора с увреждания, които чакат за настаняване в социални услуги от резидентен тип, в т.ч. с психични разстройства, умствена изостаналост и деменция-възрастни с психични разстройства, трайно настанени (над 1г.) в държ. психиатрични болници и които нямат нужда от активно лечение, но не могат да бъдат изписани поради липсата на жилище, близки или последните не желаят да се грижат за тях след изписването им от болницата. -пълнолетни лица с умствена изостаналост и множествени увреждания-лица с различни форми на деменция-роднини и близки на възрастни хора, хора с увреждания, деменция и др., които полагат грижи за тях-широката общественост.Основна цел: изграждане на инфраструктура за разкриване на социална услуга в общността за възрастни хора и хора с увреждания, съгл. Картата на резидентните услуги и Картата на услугите за подкрепа в общността. Очакван резултат: В изградената инфраструктура да се предоставя качествена социална услуга за възрастни и хора с увреждания. ПП ще допринесе за постигане на интегриран подход при грижата за възрастните хора и хората с увреждания, с ясен социален ефект и въздействие, чрез гарантираната допълняемост, интегрираност и устойчивост на мерките, заложени по ОПРЧР 2014-2020г. Основни дейности: организация и управление, мерки за информиране и комуникация, изготвяне на инвестиционен проект, изпълнение на СМР, осъществяване на АН, строителен надзор с инвеститорски функции и оценка на съответствието на инв.проект, въвеждане в експлоатация, обзавеждане и оборудване.
