Głównym celem projektu jest zaktywizowanie społeczno–zawodowe 45 osób wykluczonych z terenu Powiatu Ostrołęckiego do 30.09.2018r. Grupę docelową stanowić będzie 33 wychowanków pieczy zastępczej(WPZ) i 12 osób z niepełnosprawnością (OzN). W ramach projektu realizowane będą zadania: 1. Aktywna integracja: a) rekrutacja,diagnoza potrzeb i predyspozycji UP oraz skonstruowanie dla każdego UP indywidualnej ścieżki reintegracji b) aktywizacja społeczna, aktywizacja zdrowotna, aktywizacja zawodowa - ścieżka samodzielności dla osób z niepełnosprawnościami (OzN) i ścieżka dorosłości dla wychowanków pieczy zastępczej (WPZ). 2. Praca socjalna - bezpośrednie wsparcie pracowników socjalnych i koordynatorów rodzinnej pieczy zastępczej. 3. Świadczenia pieniężne - wypłata świadczeń na rzecz UP w okresie trwania projektu. Główne wskaźniki projektu: Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem w programie-45 Liczba osób z niepełnosprawnościami objętych wsparciem w programie-12 Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu-26; Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, poszukujących pracy po opuszczeniu programu-10; Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek)-5; Odsetek osób, które osiągnęły minimalny poziom efektywności społeczno - zatrudnieniowej - 56% (dla osób zagrożonych ubóstwem lub wykluczeniem społecznym)/ 46% (dla osób o znacznym stopniu niepełnosprawności, osób z niepełnosprawnością intelektualną oraz osób z niepełnosprawnościami sprzężonymi). Odsetek osób, które osiągnęły minimalny poziom efektywności zatrudnieniowej - 22% (dla osób zagrożonych ubóstwem lub wykluczeniem społecznym)/ 12% (dla osób o znacznym stopniu niepełnosprawności, osób z niepełnosprawnością intelektualną oraz osób z niepełnosprawnościami sprzężonymi). The main objective of the project is to activate the socio-professional 45 people excluded from the area of Ostrołęcki County until 30.09.2018. L’objectif principal du projet est l’activation socioprofessionnelle de 45 personnes exclues du district d’Ostrołęcki jusqu’au 30 septembre 2018. Le groupe cible sera de 33 étudiants en famille d’accueil (CDH) et de 12 personnes handicapées (OZN). Dans le cadre du projet, les tâches suivantes seront exécutées: 1. Intégration active: a) le recrutement, le diagnostic des besoins et des prédispositions de l’UP et la construction d’un parcours de réinsertion individuelle pour chaque UP; b) l’activation sociale, l’activation sanitaire, l’activation professionnelle — chemin d’indépendance pour les personnes handicapées (OZN) et le parcours adulte pour les étudiants en famille d’accueil (HR). 2. Travail social — soutien direct aux travailleurs sociaux et aux coordonnateurs des familles d’accueil. 3. Prestations en espèces — versement de prestations à l’organisme payeur pendant la durée du projet. Principaux indicateurs du projet: Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale bénéficiant d’un soutien au titre du programme 45 Nombre de personnes handicapées bénéficiant d’un soutien au titre du programme-12 Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale ayant obtenu une qualification après avoir quitté le programme-26; Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale à la recherche d’un emploi après avoir quitté le programme-10; Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale dans l’emploi post-programme (y compris les travailleurs indépendants)-5; Proportion de personnes ayant des performances socioprofessionnelles minimales — 56 % (pour les personnes exposées au risque de pauvreté ou d’exclusion sociale)/46 % (pour les personnes présentant un handicap grave, les personnes ayant un handicap intellectuel et les personnes handicapées associées).
