Celem głównym proj jest dokonanie postępu w procesie aktywizacji społeczno- zatrudnieniowej przez min. 34% spośród 45 osób (25K, 20 M) zagrożonych ubóstwem lub wykluczeniem społecznym, zamieszkujących w myśl KC woj. zachodniopomorskim w Powiatach choszczeńskim, gryfińskim, pyrzyckim stargardzkim i myśliborskim poprzez udział w kompleksowym wsparciu do 31.12.2019. W proj. założono osiągnięcie wskaźników: • Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem – 45. • Liczba osób z niepełnosprawnościami objętych wsparciem – 6. • Liczba osób zagrożonych ubóstwem lub wykluczeniem społ., które uzyskały kwalifikacje– 14 (min. 31%). • Liczba osób zagrożonych ubóstwem lub wykl. społ. poszukujących pracy – 12 (min. 25%). • Liczba osób zagrożonych ubóstwem lub wykluczeniem społ. pracujących – 7(min.15%). • Liczba os. zagrożonych ubóstwem lub wykl. społ., które nabyły kompetencje– 18 Grupę docelową stanowi 45 osób (25 K,20 M) zamieszkałych w Powiecie choszczeńskim, gryfińskim, pyrzyckim stargardzkim, myśliborskim zagrożonych ubóstwem lub wykluczeniem społ. Do proj zostaną przyjęte osoby spełniające min. 1 przesłankę zagrożenia ubóstwem lub wykluczeniem społecznym: - os bierne ZAW - 27 UP (15 K, 12 M), - os bezrobotne – 18 UP (10K, 8 M), - os niepełnosprawne – 6 UP (4K), w tym 5 o znacznym/umiarkowanym stopniu niepełnospr., z niepełn. sprzężoną oraz os z zaburz. psych., w tym os z niepełnospr. intelektualną i os z całościowymi zaburzeniami rozw. W ramach projektu ostaną zrealizowane takie zadania, jak: 1. Działalność Klubu Aktywnych 2. Aktywizacja społeczna 3. Aktywizacja edukacyjna 4. Aktywizacja zawodowa. (Polish)

The main aim of the project is to make progress in the process of social and employment activation by at least 34 % of the 45 people (25K, 20 M) at risk of poverty or social exclusion, living in the West Pomeranian Voivodeship in the districts of Choszcze, Gryfin, Pyrzyce Stargardzki and Hunting, by participating in comprehensive support up to 31.12.2019. The project assumes the achievement of indicators: • Number of people at risk of poverty or social exclusion supported – 45. • Number of persons with disabilities supported – 6. • Number of people at risk of poverty or social exclusion who are qualified – 14 (min. 31 %). • Number of people at risk of poverty or job seekers – 12 (min. 25 %). • Number of people at risk of poverty or social exclusion – 7 min.15 %). • The number of people at risk of poverty or population who have acquired competences – 18 target group consists of 45 people (25 K,20 M) residing in the Poviat of Choszczen, Gryfin, Pyrzyce Stargardzki, Huntsberg, at risk of poverty or exclusion of the community. Persons fulfilling at least 1 conditions of at risk of poverty or social exclusion will be admitted to the project: — passive wass ZAW – 27 UP (15 K, 12 M), – wasp unemployed – 18 UP (10K, 8 M), – wasp disabled – 6 UP (4K), including 5 with a significant/moderate degree of disability, with a partial coupled and wasps with mental disturbances. The project will provide such tasks as: 1. Activity of the Activity Club 2. Social Activity 3. Educational Activity 4. It’s a professional activisation. (English)

L’objectif principal du projet est de progresser dans le processus d’activation sociale et d’emploi d’au moins 34 % des 45 personnes (25 000, 20 millions de personnes) menacées de pauvreté ou d’exclusion sociale, résidant dans les voïvodies de Zachodniopomorskie selon le KC de Choszczeń, de gryfiński, de Pyrzycki Stargard et de Huntorski en participant à un soutien complet jusqu’au 31.12.2019. Le projet suppose la réalisation d’indicateurs: • Le nombre de personnes menacées de pauvreté ou d’exclusion sociale bénéficiant d’un soutien — 45. • Nombre de personnes handicapées bénéficiant d’un soutien — 6. • Le nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale ayant obtenu des qualifications — 14 (min. 31 %). • Le nombre de personnes exposées au risque de pauvreté ou de faute professionnelle — 12 (min. 25 %). • Le nombre de personnes menacées de pauvreté ou d’exclusion dans l’emploi — 7 (min.15 %). • Le nombre de personnes exposées au risque de pauvreté ou de saignements qui ont acquis des compétences — le groupe cible de 18 personnes est composé de 45 personnes (25 K.20 M) vivant dans les régions de Choszczeń, Gryfin, Pyrzycki Stargarddzki, terrains de chasse exposés au risque de pauvreté ou d’exclusion sociale. Les personnes remplissant au moins une condition de risque de pauvreté ou d’exclusion sociale seront admises au proj: — passive OS ZAW — 27 UP (15 K, 12 M), — chômeurs — 18 UP (10K, 8 M), — Personnes handicapées — 6 UP (4K), dont 5 sont gravement/modérément incomplets, avec un couplage incomplet et une guêpe avec troubles mentaux, y compris la guêpe avec des troubles intellectuels incomplets et la guêpe avec des troubles du développement total. Dans le cadre du projet, les tâches suivantes seront exécutées: 1. Activité de l’Active Club 2. Activation sociale 3. Activation éducative 4. Activation professionnelle. (French)
