Проектът представлява интегриран модел на предоставяне на здравни грижи и социални услуги в дома за възрастни хора, хронично -болни и лица с увреждания, свързан с преодоляване на затрудненията от здравен характер и подобряване на техния психологически и социален статус. Услугите се предоставят директно в дома и включват обгрижване, помощ при самообслужване и медицински дейности според утвърдения здравен пакет. Дейностите са насочени изцяло към възрастните хора над 65 години с ограничения или невъзможност за самообслужване, както и към хора с увреждания и техните семейства, хора които нямат близки и за които има социална необходимост от почасови услуги. Голяма част от хората над 65 г. възраст запазили способността си за самостоятелен живот, страдат от съпътстващи ги здравословни проблеми и имат нужда от медицинско наблюдение. Домашните грижи ще се оказват от мобилни екипи, подпомагащи лицата при справяне с ежедневните им дейности от бита, поддържане хигиената в дома, хигиена на тялото, хранене, пазаруване, придружаване при необходимост до лекуващ лекар или разходки. Услугите ще се предоставят до 2 часа на ден на лице, според идентифицираните нужди за период от 12 календарни месеца. The project is an integrated model of care provision and social services in the home for the elderly, chronically ill and disabled, related to overcoming health difficulties and improving their psychological and social status. Services are provided directly at home and include care, self-care and medical activities according to the approved health package. The activities are aimed entirely at older people over 65 years of age, with limitations or inability to self-service, as well as people with disabilities and their families, people with no relatives and for whom there is a social need for hourly services. A large proportion of people over the age of 65 have retained their ability to live independently, suffer from accompanying health problems and need medical supervision. Home care will be provided by mobile teams to assist individuals in coping with their daily activities from the household, maintaining home hygiene, body hygiene, eating, shopping, accompanying if necessary to a treating physician or walks. The services will be provided up to 2 hours per day per person, according to the identified needs over a period of 12 calendar months. Mobile teams will include domestic orderlies, medical professionals and social workers. (English)
