В рамките на проекта се предвижда осигуряване на достъп до интегрирани услуги, за социално включване в общността или в домашна среда, в т.ч. подкрепящи и интегрирани между секторни услуги, според специфичните потребности на човека в неравностойно положение въз основа на индивидуална социална оценка чрез подкрепа и разширяване на дейността на съществуващото звено за почасово предоставяне на услуги, създадено по проект "Помощ в дома". В звеното ще се представят по иновативен начин на нуждаещи се лица, живеещи на територията на община Копривщица три типа почасови услуги в домашна среда – за лична помощ, за социална подкрепа и помощ при комунално-битови дейности, както и допълнителни здравни и социални услуги в среда различна от домашната, а именно: здравни услуги, рехабилитационни услуги, мотивационни и/или психологически услуги на нуждаещите се лица за срок от 18 месеца. Към него ще бъдат назначени по трудови договори 24 души персонал, които ще предоставят услуги за 38 потребители. / В това число персонала и потребителите от проект "Нови алтернативи", които ще бъдат включени след 01.03.2016год/.Настоящето проектно предложение на община Копривщица отговаря на общата и специфичните цели на схема „Независим живот” по следния начин:-представителите на целевата група ще пулучат достъп до дългосрочна здравно-социална грижа, вкл. и достъп до заетост, според специфичните потребности на човека в неравностойно положение на основата на индивидуална оценка на потребностите;- създава се устойчив децентрализиран модел на предоставяне на социални услуги и осигуряване на условия за достоен живот за лица, които са или се намират в риск от социална изолация;-предоставя възможности за връщането на реалния пазар на труда на лицата, които полагат грижи за близките си с увреждания. The project envisages providing access to integrated services, for social inclusion in the community or in the home environment, including supporting and integrated between sectoral services, according to the specific needs of disadvantaged people on the basis of an individual social assessment by supporting and expanding the activities of the existing unit for hourly service delivery established under the project “Help at home”. The unit will present in an innovative way to deprived persons living on the territory of the municipality of Koprivshtitsa three types of hourly services in the home environment — for personal assistance, for social support and assistance in utility activities, as well as additional health and social services in an environment other than the home environment, namely: health services, rehabilitation services, motivational and/or psychological services to people in need for a period of 18 months. It will be recruited under employment contracts 24 staff who will provide services to 38 consumers. /Including the staff and users of the project “New Alternatives”, which will be included after 01.03.2016/.The present project proposal of the Municipality of Koprivshtitsa meets the general and specific objectives of the “Independent Life” scheme in the following way:-the representatives of the target group will have access to long-term health-social care, including access to employment, according to the specific needs of the disadvantaged person on the basis of an individual assessment of needs;- creates a sustainable decentralised model of provision of social services and decent living conditions for persons who are or are at risk of social exclusion;- provide opportunities for the return of the real labour market to the persons taking care of their close disabilities.
