Gł.celem projektu jest zwiększenie dostępu do usług społecznych w zakresie wsparcia rodziny oraz osób niesamodzielnych i niepełnosprawnych poprzez zbudowanie zintegrowanego systemu pomocy dla 80 osób (44K i 36M)zagrożonych wykluczeniem społecznym, zamieszkujących zdegradowany obszar miasta Nowy Dwór Gdański. Objęcie wsparciem danej grupy wynika z diagnozy przeprowadzonej w ramach GPR dla Gminy Nowy Dwór Gdański,zatwierdzonego uchwałą Rady Miejskiej 260/XXX/2017 z dn.16.03.2017r. Cele szczegółowe: -wzmocnienie więzi społecznych u min.50% uczest.; -rozwój społeczno-kulturalno-edukacyjny u co najmniej 80%dzieci i młodzieży objętych proj; -wzrost umiejętności rozwiązywania problemów wychowawczych i społecznych u min.50%rodziców objętych proj, -wzrost wiedzy w zakresie opieki nad os.niesamodzielnymi i niepełnosprawnym u min.25% uczest.; Projekt zakłada:1.Utworzenie Miejsca Aktywności Lokalnej(MAL)w ramach Centrum Rozwoju Rodziny(CRR): -utworzenie 2grup samopomocowych:Akademia Fajnego Rodzica oraz Ruch sąsiedzki, -organizacja warsztatów rodzicielskich, -wyjazdy studyjne dla gr.samopomocowych, -zatrudnienie koordynatora MAL oraz animatora społ., -zakup wyposażenia sali warszt. 2.Utworzenie Placówki wsparcia dziennego w ramach CRR,tzw.Kluboświetlica: -warsztaty wpływające na rozwój dzieci i młodzieży, -warsztaty zdrowego żywienia,plenerowe, -wycieczki edukacyjno-rozwojowe, -zatrudnienie kierownika i wychowawcy oraz zakup wyposażenia. 3.Utworzenie Punktu Konsult.w zakresie poradnictwa specjal.:psycholog,prawnik i terapeuta. 4.Wsparcie osób niesamodzielnych i niepełnosprawnych: -warsztaty dla opiekunów, -zatrudnienie asystenta os.niepełnosp.(AON)oraz opiekuna rodzin, -utworzenie Centrum Wolontariatu(CW)i zatrudnienie koordynatora CW. Gł.efektem proj.będzie wzrost aktywności społ.u min.80%uczestników proj.poprzez zwiększenie liczby m-c świadczenia usług społ.w stosunku już realizowanych przez gminę. (Polish)

The aim of the project is to increase access to social services in the field of family support and dependent and disabled persons by building an integrated aid system for 80 people (44K and 36M) at risk of social exclusion, living in the degraded area of Nowy Dwór Gdański. The inclusion of support for a given group results from the diagnosis carried out under the GPR for Nowy Dwór Gdański commune approved by the City Council resolution 260/XXX/2017 dated 16.03.2017r. Specific objectives: —strengthening social ties in at least 50 % of participation; — socio-cultural and educational development in at least 80 % of children and young people involved in the project; —increase the ability to solve educational and social problems in at least 50 % of the projected parents, -increasing knowledge of care for dependent and disabled persons in min.25 % participation; The project assumes:1.Creation of Local Activity Facility(MAL) within the framework of the Centre for Family Development(CRR): — creation of 2 self-help groups: Cool Parent Academy and Neighbourhood Movement, -organisation of parental workshops, -study trips for self-help games, -employment of MAL coordinator and social animator, -purchase of garage equipment. 2.Creating a day support facility within the framework of the CRR, so-called Club Lighting: —workshops affecting the development of children and young people, -workshops of healthy nutrition, background, -education and development trips, -employment of the manager and educator and purchase of equipment. 3.Creating a Consultation Point in the field of special counseling:psychologist, lawyer and therapist. 4.Support for dependent and disabled persons: —workshops for carers, -employment of part-time assistant (AON) and family caregiver, -creation of the Volunteer Centre(CW) and employment of CW coordinator. The effect of the project will be an increase in social activity of at least 80 % of participants in the project by increasing the number of services provided by the community in relation to the already implemented by the commune. L’objectif de ce projet est d’améliorer l’accès aux services sociaux dans le domaine de l’aide aux familles et des personnes dépendantes et handicapées en mettant en place un système intégré d’assistance à 80 personnes (44K et 36M) exposées au risque d’exclusion sociale, vivant dans la zone dégradée de Nowy Dwór Gdański. Le soutien du groupe est basé sur le diagnostic effectué dans le cadre du RPG pour la municipalité de Nowy Dwór Gdański, approuvé par la résolution 260/XXX/2017 du conseil municipal du 16 mars 2017. Objectifs spécifiques: —renforcer les liens sociaux chez au moins 50 % des participants; développement socioculturel et éducatif d’au moins 80 % des enfants et des jeunes couverts par le projet; —accroître la capacité de résoudre les problèmes éducatifs et sociaux chez au moins 50 % des parents projetés, — accroître la connaissance des soins aux personnes dépendantes et handicapées chez au moins 25 % des participants; Projet zakłada:1.Utworzenie Site d’activités locales(MAL) au sein du Centre de développement de la famille (CRR): —création de 2 groupes de samopomocowych:Akademia Cool Parent and Neighbouring Movement, -organisation d’ateliers parentaux, — voyages d’étude pour l’auto-assistance, -emploi de coordinateur MAL et animateur social, — achat de matériel de salle d’atelier. 2.Création d’une facilité de soutien journalier dans le cadre du CRR, dite Club Lights: —ateliers affectant le développement des enfants et des jeunes, -ateliers de nutrition saine, cours arrières, voyages éducatifs et de développement, — emploi d’un gestionnaire et d’un éducateur et achat d’équipements. 3.Création d’un point de consultation dans le domaine du conseil specialty:psychologist, avocat et thérapeute. 4.Soutien aux personnes à charge et aux personnes handicapées: —ateliers pour aidants, — emploi d’un assistant (AON) et aidant familial, création d’un centre de bénévolat (CW) et emploi d’un coordinateur de la CW. Le principal effet du projet sera une augmentation de l’activité sociale d’au moins 80 % des participants au projet en augmentant le nombre de mois de prestation de services à la société dans le rapport déjà fourni par la municipalité.
