Cel główny: upowszechnienie dostępu, usprawnienie i poprawa jakości świadczeń usług społecznych poprzez stworzenie niezbędnej infrastruktury i aktywizacja społeczno-zawodowa mieszkańców. Cele szczegółowe: zwiększenie zakresu usług społecznych o nowe formy wsparcia; zwiększenie liczby miejsc świadczenia usług; wzrost integracji społecznej; wsparcie zrównoważonego rozwoju gminy w sferze gospodarczej i społecznej poprzez rozwój infrastruktury i aktywizację społeczno-zawodową mieszkańców. Kluczowe rezultaty: 1.rzeczywista liczba użytkowników usług aktywizacji społeczno-zawodowej świadczonych we wspartych obiektach-110 osób/rok; 2.liczba przebudowanych obiektów, w których realizowane są usługi aktywizacji społeczno-zawodowej-1 szt.; 3.liczba obiektów dostosowanych do potrzeb osób z niepełnosprawnościami-1szt.; 4.liczba wspartych instytucji świadczących środowiskowe formy wsparcia w miejscu zamieszkania-1szt.; 5.liczba projektów, w których sfinansowano koszty racjonalnych usprawnień dla osób z niepełnosprawnościami-1 szt. Grupa docelowa to samorząd gminy, uczestnicy wsparcia w ramach ŚDS, lokalni przedsiębiorcy i instytucje umożliwiające realizację projektu. Przebudowa spichlerza w Podzamczu ma na celu przystosowanie go do dziennej opieki i aktywizacji osób upośledzonych umysłowo w ramach Środowiskowego Domu Samopomocy. Na parterze budynku będą mieścić się pomieszczenia ogólnodostępne i administracyjne oraz kuchnia z jadalnią i taras. Piwnica: sala doświadczania świata, sala rehabilitacji ruchowej oraz pomieszczenia gospodarcze. I piętro: sale zajęciowe, pokój psychologa i pielęgniarki, zaplecze sanitarne. Poddasze: hol, wyjście na taras widokowy, wentylatornia; 4 mieszkania treningowe 1-osobowe, 2 dwuosobowe pokoje interwencyjne, pomieszczenie gospodarcze. Zostanie wykonane zagospodarowanie terenu wraz z parkingiem i zjazdem z drogi wojewódzkiej oraz infrastruktura techniczna: przyłącza wod.-kan., gazowe, energetyczne, instalacja monitoringu, portal www projektu. Main objective: increasing access, improving and improving the quality of social services services through the creation of the necessary infrastructure and social and occupational activation of residents. Specific objectives: increasing the scope of social services by new forms of support; increasing the number of places to provide services; increasing social inclusion; support for the sustainable development of the municipality in economic and social spheres through infrastructure development and socio-occupational activation of residents. Key results: 1.the actual number of users of socio-occupational activation services provided in supported facilities-110 persons/year; 2. number of reconstructed buildings in which socio-occupational activation services are carried out – 1 pieces; 3.number of facilities tailored to the needs of people with disabilities-1pieces; 4. number of institutions supporting environmental support at the place of residence-1 pieces; 5.number of projects which financed the costs of rational improvements for people with disabilities-1 pcs. The target group is the municipal self-government, participants of support within the framework of the SDS, local entrepreneurs and institutions enabling the implementation of the project. Reconstruction of the granary in Podzamcze is aimed at adapting it to day care and activation of mentally impaired people within the Environmental Self-Help Home. On the ground floor of the building there will be public and administrative spaces, as well as a kitchen with dining area and a terrace. Cellar: world Experience Room, Rehabilitation Room and Economic Room. And the first floor: classrooms, psychologist’s room and nurses’ room, sanitary facilities. Attic: lobby, exit to the observation deck, VENTILATION; 4 training apartments 1-bed, 2 double intervention rooms, farm room. The area will be developed along with parking and exit from the voivodship road as well as technical infrastructure: connects water.-channel, gas, energy, monitoring installation, web portal of the project. Objectif principal: accroître l’accès, l’amélioration et l’amélioration de la qualité des services sociaux par la création des infrastructures nécessaires et l’activation socioprofessionnelle des résidents. Objectifs spécifiques: élargir la portée des services sociaux grâce à de nouvelles formes de soutien; l’augmentation du nombre de places pour fournir des services; accroître l’inclusion sociale; soutenir le développement durable de la municipalité dans le domaine économique et social par le développement des infrastructures et l’activation socioprofessionnelle des résidents. Principaux résultats: 1.Un nombre réel d’utilisateurs de services d’activation socioprofessionnelle fournis dans des installations bénéficiant d’un soutien: 110 personnes/an; 2.Nombre d’installations reconstruites dans lesquelles les services d’activation socio-professionnelle sont effectués-1 pièces; 3.Nombre d’installations adaptées aux besoins des personnes handicapées-1pcs; 4.Nombre d’institutions soutenues fournissant un soutien environnemental dans le lieu de résidence-1 pcs; 5.Nombre de projets qui ont financé le coût des aménagements raisonnables pour les personnes handicapées-1 pcs. Le groupe cible est l’autonomie gouvernementale de la municipalité, les participants à l’appui dans le cadre de la SDD, les entrepreneurs locaux et les institutions permettant la mise en œuvre du projet. La reconstruction du grenier de Podzamcz vise à l’adapter aux garderies et à l’activation des personnes souffrant de handicaps mentaux au sein de la maison d’auto-assistance environnementale. Au rez-de-chaussée du bâtiment il y aura des salles publiques et administratives, ainsi qu’une cuisine avec salle à manger et terrasse. Sous-sol: salle d’expérience mondiale, salle de réhabilitation de mouvement et chambres économiques. 1er étage: Salles de travail, salle de psychologues et d’infirmières, sanitaires. Grenier: hall d’entrée, sortie vers le pont d’observation, ventilateur; 4 appartements de formation 1 lit, 2 salles d’intervention doubles, buanderie. L’aménagement du terrain avec stationnement et sortie de la route provinciale et de l’infrastructure technique sera réalisé: connexions eau-canal, gaz, énergie, installation de surveillance, portail web du projet.
