Cel: Reintegracja społeczna i zawodowa 36 (24K;12M) os. zagrożonych ubóstwem lub wykluczeniem społ. (w tym 6 os. bezrob. należących do III profilu), które w 1-szej kolejności wymagają aktywizacji społ.- będące os. korzystając. ze świadczeń z pomocy społ. zg. z ustawą o pomocy społ. lub kwalifik. się do objęcia wsparciem pomocy społ., zamieszk. na terenie gm. Koluszki - w okresie 01.09.2019- 31.08.2021 poprzez realiz. działań ukierunk. na aktywizację społeczną, edukac. i zawodową. Grupa docelowa: 36 (24K;12M) os. fizycznych, zagrożonych ubóstwem lub wyklucz. społ., które w 1-szej kolejności wymag. aktywizacji społ.: będące os. korzystaj. ze świadczeń z pomocy społ. zg. z ustawą z 12.03.2004 r. o pomocy społ. lub kwalifik. się do objęcia wsparciem pomocy społ., tj. spełn. co najm. 1 z przesłanek określ. w art. 7 ww. ust., zamieszk. na terenie gm. Koluszki (WŁ zg z KC), kwalifik. się do objęcia wsparciem CIS zg.z Ustawą o zatrud. soc., w tym: * min. 6os. (4K,2M) to os. bezrob. zarejestr. w PUP, dla których ustalono III profil (wsparcie realiz. jest na podst. kontraktu socj. z obowiązk. zastos. instrumentów aktywnej integr. o charakterze zawod.) * pozostali UP- os. bierne, niezarejest. w PUP (Proj. nie obejm. os. bezrob., zarejestr. w PUP i zakwalifik. do I i II profilu pomocy, m.in. ze wzgl. na wsparcie aktywiz. otrzym. z PUP) Działania w proj. ukierunkow. są na aktywizację: *społ. (praca socjalna, wyznaczenie ścieżki reintegracji, poradn. spec., trening radzenia sobie z negatywnymi emocjami i stresem, wsparcie arteterapeutyczne), *edukacyjną (kursy komputerowe), *zawodową (udział w CIS, IPD, szkolenia zawod., praktyki i pośredn. pracy). Objective: Social and occupational reintegration 36 (24K; 12M) people at risk of poverty or social exclusion. (including 6 persons belonging to the third profile) which in the 1st order require the activation of the community- those who benefit from the social assistance benefits in accordance with the Social Aid Act or eligible for community aid, residents on the premises of Koluszki – in the period 01.09.2019-31.08.2021 through the implementation of actions aimed at social activation, education and professional activities. Target group: 36 (24K; 12M) physical persons at risk of poverty or exclusion, which in the 1st order of activation of the community: those persons benefit from the benefits of social aid in accordance with the Act of 12.03.2004 on social aid or eligible for the support of the social aid, i.e. the rent. 1 of the conditions specified in Article 7 above, the inhabitant on the premises of Koluszki (WŁZ z z KC), eligible for CIS support under the Law on Employment Soc., including: * min. 6os. (4K,2M) is a person registered in the PUP, for which the third profile has been established (the support of the implementation is on the next contract of socij. with obligatory use of active instruments integrated.) * the remaining UP-passive, is not registered in PUP (Proj. do not include. It’s a team. (social work, determination of the path of reintegration, special counselling, training for dealing with negative emotions and stress, art therapy support), *educational (computer courses), *professional (participation in CIS, IPD, occupational training, practice and mid-work).
