L'opération se compose d'une seule action. L'opération prend la forme d'un accompagnement individualisé de demandeurs d’emploi présentant un cumul de difficultés professionnelles et sociales qui nécessite de mobiliser les expertises dans ces deux domaines, de manière simultanée et coordonnée. Cet accompagnement sur le champ professionnel sera mis en ½uvre par 4 conseillers dédiés correspondant à 3.6 ETP pour une année entière. Conformément au partenariat élaboré avec le conseil départemental du Tarn et Garonne, le dispositif est déployé sur le territoire comme suit : * 3.4 ETP au 01/10/2014, date de démarrage du dispositif, dont : * 3 conseillers dédiés répartis sur les 2 agences Pôle emploi de Montauban (3 * 0.8 ETP) * 1 conseiller dédié sur l’agence Pôle emploi de Castelsarrazin (1 ETP) A noter : au 01/11/2014, l’un des conseiller de Montauban a augmenté sa quotité de temps de travail en passant à 0.9ETP Ainsi la présente demande, pour la période considérée (du 01/10/2014 au 31/12/2016) portera sur 7.87 ETP L’action prévoit les phases suivantes : * La détection des candidats à l'accompagnement ; qui constitue la phase amont de l’action * Le diagnostic ; * Le bilan professionnel, la stratégie de recherche d'emploi et la définition du plan d'action ; * La mise en ½uvre du plan d'action avec des contacts personnalisés, des MER, le bénéfice de prestations collectives (courtes type atelier CV, atelier entretien), la promotion de candidatures auprès d’entreprises, … * En continu, l’articulation avec l’offre d’action sociale du département * Le suivi dans l'emploi ou la formation ; qui constitue la phase aval de l’action. L'amont et l'aval de l'accompagnement font partie intégrante de l'action. Le conseiller dédié pourra moduler ces phases et leur contenu en fonction du besoin de chaque demandeur d'emploi suivi dans le cadre de l'accompagnement global. Cet accompagnement intensif sera principalement individuel. Après 12 mois d’accompagnement, si le demandeur d’emploi n’a pas trouvé de solution d’insertion professionnelle, sa situation est réexaminée conjointement par le Conseil départemental et Pôle emploi (regard croisé différent du binôme conseiller Pôle emploi/travailleur social) pour acter ou non la poursuite de l’accompagnement global par le conseiller Pôle emploi. The operation consists of a single action. The operation takes the form of individualised support for jobseekers with multiple occupational and social difficulties, which requires simultaneous and coordinated mobilisation of expertise in these two areas. This professional support will be implemented by 4 dedicated advisers corresponding to 3.6 FTEs for a full year. In accordance with the partnership developed with the Tarn and Garonne Departmental Council, the facility is deployed on the territory as follows: * 3.4 FTE at 01/10/2014, starting date of the device, of which: * 3 dedicated advisers spread across the 2 agencies Pôle emploi de Montauban (3 * 0.8 FTE) * 1 dedicated adviser on the employment department of CASTELSARRAZIN (1 FTE) Please note: as of 01/11/2014, one of Montauban’s advisers increased its working time quota to 0.9 FTE. Thus the present application for the period concerned (from 01/10/2014 to 31/12/2016) will cover 7.87 FTEs The action foresees the following phases: * The detection of candidates for accompaniment; which constitutes the upstream phase of the action * Diagnosis; * The job review, the job search strategy and the definition of the action plan; * Implementation of the action plan with personalised contacts, SRMs, collective benefits (short CV workshop type, interview workshop), promotion of applications to companies, etc. * Ongoing, articulation with the department’s social action offer * Monitoring in employment or training; which constitutes the downstream phase of the action. Upstream and downstream accompaniment are an integral part of the action. The dedicated advisor will be able to modulate these phases and their content according to the needs of each job seeker followed as part of the overall support. This intensive support will be mainly individual. After 12 months of support, if the jobseeker has not found a solution for occupational integration, his/her situation is reviewed jointly by the Departmental Council and the Employment Department (a different view from the employment/social worker advisory group) in order to carry out or not the continuation of the overall support by the employment department adviser. Die Operation besteht aus einer einzigen Aktion. Die Maßnahme erfolgt in Form einer individuellen Betreuung von Arbeitsuchenden mit kumulativen beruflichen und sozialen Schwierigkeiten, die die gleichzeitige und koordinierte Mobilisierung von Fachwissen in diesen beiden Bereichen erfordert. Diese Begleitung auf dem Berufsfeld wird ½ von 4 engagierten Beratern durchgeführt, die für ein ganzes Jahr 3,6 VZÄ entsprechen. Im Einklang mit der Partnerschaft mit dem Bezirksrat von Tarn und Garonne wird das System auf dem Gebiet wie folgt eingesetzt: * 3.4 VZÄ zum 1.1.2014, Startdatum der Einrichtung, davon: * 3 engagierte Berater verteilt auf die 2 Agenturen Jobpol von Montauban (3 * 0.8 VZÄ) * 1 engagierter Berater auf der Agentur Pole Job von CASTELSARRAZIN (1 VZÄ) Bemerkenswert: zum 1.1.2014 hat einer der Montauban-Berater seine Arbeitszeitquote auf 0.9 VZÄ erhöht. So wird sich der vorliegende Antrag für den betrachteten Zeitraum (1/10/2014 bis 31.12.2016) auf 7.87 VZÄ beziehen. Die Maßnahme sieht folgende Phasen vor: * Die Erkennung von Bewerbern für die Begleitung; das ist die vorgelagerte Phase der Aktion * Diagnose; * Die berufliche Bilanz, die Strategie für die Arbeitssuche und die Festlegung des Aktionsplans; * Umsetzung des Aktionsplans mit personalisierten Kontakten, MEER, kollektiven Leistungen (kurze Art Workshop CV, Workshop Interview), Förderung von Bewerbungen bei Unternehmen,... * Kontinuierlich die Verknüpfung mit dem Angebot von Sozialmaßnahmen der Abteilung * Follow-up in Beschäftigung oder Ausbildung; dies ist die nachgelagerte Phase der Aktion. Der Vor- und der Abschluss der Begleitung sind integraler Bestandteil der Aktion. Der spezielle Berater kann diese Phasen und ihren Inhalt entsprechend dem Bedarf jedes Arbeitsuchenden, der im Rahmen der allgemeinen Begleitung begleitet wird, differenzieren. Diese intensive Begleitung erfolgt in erster Linie individuell. Hat der Arbeitsuchende nach 12 Monaten keine Lösung für die berufliche Eingliederung gefunden, so wird seine Situation vom Conseil départemental et Pôleemploi (Kreuzblick, der sich vom Doppelberater Pôleemploi/Sozialarbeiter unterscheidet) gemeinsam überprüft, um die Fortsetzung der allgemeinen Begleitung durch den Berater zu begreifen.
