Το έργο θα υλοποιηθεί μέσω ενός υποέργου με τη μέθοδο της υλοποίησης με ίδια μέσα. Ο τίτλος του υποέργου ταυτίζεται με τον τίτλο της Πράξης, ήτοι: «Ολοκληρωμένη Κοινοτική Θεραπεία και Κατ οίκον Νοσηλεία από την Κινητή Μονάδα Ψυχικής Υγείας Λακωνίας της ΑΠΟΣΤΟΛΗ». Το υποέργο θα χωρίζεται σε δύο Πακέτα Εργασίας. Συγκεκριμένα, στο Πακέτο Εργασίας 1: Διαδικασίες Έναρξης και Προετοιμασία της Πράξης και στο Πακέτο Εργασίας 2: Λειτουργία Τμήματος Κατ Οίκον Νοσηλείας στη Λακωνία . Το Πακέτο Εργασίας 1 περιλαμβάνει την προετοιμασία για την έναρξη υλοποίησης της Πράξης, όπως την πρόσκληση εκδήλωσης ενδιαφέροντος για την πρόσληψη του προβλεπόμενου προσωπικού (1 Ψυχιάτρου, 1 Ψυχολόγου, 1 Κοινωνικού λειτουργού και 1 Νοσηλευτή), την πρόσκληση για μακροχρόνια μίσθωση αυτοκινήτου κα. Επίσης, περιλαμβάνει την προετοιμασία υλικού δημοσιότητας, ενημέρωσης και πληροφόρησης της κοινότητας. Το Πακέτο Εργασίας 2 αφορά στην λειτουργία του Τμήματος Κατ οίκον Νοσηλείας της Κινητής Μονάδας Ψυχικής Υγείας στην Λακωνία με την πλήρη ανάπτυξη των επιμέρους ψυχιατρικών υπηρεσιών.Κύριος στόχος είναι η παροχή υπηρεσιών ψυχικής υγείας και η βελτίωση της ποιότητας ζωής σε ψυχικά ασθενείς, ενήλικες, ηλικιωμένους, άτομα με αναπηρίες κ.α., τα οποία χρειάζονται ιδιαίτερη φροντίδα καθώς είτε δεν εξυπηρετούνται πλήρως, είτε διαβιούν μοναχικά ή είναι εγκαταλελειμμένα είτε δεν έχουν επαρκείς οικονομικούς πόρους, χωρίς να απομακρυνθούν από το κοινωνικό τους περιβάλλον.Επιμέρους Στόχοι: Η πρόληψη, η διάγνωση στις περιπτώσεις που δεν είχαν τύχει πρωθύστερης διάγνωσης Η αντιμετώπιση - θεραπεία Η εκτίμηση και η παρακολούθηση της φαρμακευτικής αγωγής Η συμβουλευτική Η αποστιγματοποίηση Η συμμετοχή στην κοινότητα. Η πολυκλαδική ομάδα του Τμήματος Κατ οίκον Νοσηλείας θα αναπτύξει την δράση της σ ολόκληρο τον Νομό Λακωνίας με την παροχή κατ οίκον εξειδικευμένων υπηρεσιών σε ψυχικά ασθενείς τόσο στην έδρα της στην πόλη της Σπάρτης, όσο και στην υπόλοιπη επικράτεια της περιφερειακής ενότητας, με την πραγματοποίηση επισκέψεων από τα μέλη της πολυκλαδικής ομάδας, όπου απαιτείται, και με συνεργασία με τα τρία μεγάλα Κέντρα Υγείας του Νομού Λακωνίας, και συγκεκριμένα το Κ.Υ. Νεάπολης, το Κ.Υ. Αρεόπολης και το Κ.Υ. Γυθείου καθώς και με τα περιφερειακά ιατρεία. (Greek)

The project will be implemented through a subproject using the method of implementation by own means. The title of the subproject is the same as the title of the Act, namely: “Integrated Community Treatment and Home Nursing by the Mobile Mental Health Unit of Laconia mission”. The subproject will be divided into two Work Packages. Specifically, in the Work Package 1: Opening and Preparing Procedures for the Act and in the Work Package 2: Operation of Department of Nursing in Laconia. The Work Package 1 includes the preparation for the start of implementation of the Act, such as the call for expressions of interest for the recruitment of the intended staff (1 Psychiatrist, 1 Psychologist, 1 Social Officer and 1 Nurse), the call for long-term car hire etc.It also includes the preparation of publicity material, information and information of the community. The Work Package 2 concerns the operation of the Department of Home Nursing of the Mobile Mental Health Unit in Lakonia with the full development of individual psychiatric services.The main objective is to provide mental health services and improve the quality of life for mental patients, adults, elderly, people with disabilities etc., who need special care as they are either not fully served, whether they live alone or are abandoned or have not sufficient financial resources to: Prevention, diagnosis in cases where they had not received a prior diagnosis The management – treatment The assessment and monitoring of medication The counseling The diabetic The participation in the community. The multidisciplinary team of the Department of Home Hospital will develop its activities throughout the prefecture of Laconia by providing home-specific services to mental patients both at its headquarters in the city of Sparta, and in the rest of the territory of the regional unit, by making visits by the members of the multidisciplinary group, where required, and in collaboration with the three major Health Centres of Laconia Prefecture, namely the K.Y.A. Le projet sera mis en œuvre dans le cadre d’un sous-projet utilisant la méthode d’auto-exécution. Le titre du sous-projet est identique au titre de la Loi, à savoir: «Traitement communautaire intégré et soins infirmiers à domicile de l’Unité mobile de santé mentale de Laconia d’Apostoli». Le sous-projet sera divisé en deux paquets de travail. Plus précisément, dans le module de travail no 1: Procédures de démarrage et préparation de la loi et de l’ensemble de travail 2: Exploitation du département des maisons de soins infirmiers à Laconia. Le module de travail 1 comprend la préparation du début de la mise en œuvre de l’opération, comme l’appel à manifestation d’intérêt pour le recrutement de personnel (1 psychiatre, 1 psychologue, 1 assistante sociale et 1 infirmière), l’appel à location de voitures à long terme, etc. Il comprend également la préparation de matériel de publicité, d’information et d’information pour la communauté. Le module de travail no 2 concerne le fonctionnement du Département des soins infirmiers à domicile de l’Unité mobile de santé mentale de Laconia, qui a pour objectif principal de fournir des services de santé mentale et d’améliorer la qualité de vie des malades mentaux, des adultes, des personnes âgées, des personnes handicapées, etc., qui ont besoin de soins spéciaux parce qu’ils ne sont pas pleinement servis, vivent seuls ou sont abandonnés ou ne disposent pas de ressources financières suffisantes, sans quitter leur environnement social.
