Namen projekta je izboljšanje pogojev za primarno zdravstveno oskrbo na območju Upravne enote Trebnje in sicer z investiranjem v ustrezne kapacitete, ki bodo omogočale krepitev javnozdravstvene vloge zdravstvenega doma Trebnje, implementacijo nadgradnje preventivnih pregledov za otroke in mladostnike, izvajanje preventivnih obravnav za ogrožene otroke in mladostnike, izvajanje prenovljene in nadgrajene preventive kroničnih bolezni, vključevanje ranljivih oseb v preventivno zdravstveno varstvo ter uvajanje modela skupnostnega pristopa za krepitev zdravja in zmanjševanje neenakosti v zdravju na območju Upravne enote Trebnje. Vse navedene aktivnosti se bodo izvajale preko vzpostavitve integriranih centrov za krepitev zdravja (CKZ) kot samostojnih enot v zdravstvenem domu Cilj projekta je zagotovitev prostorskih in materialnih pogojev za izvajanje dejavnosti centra za krepitev zdravja. The purpose of the project is to improve the conditions for primary health care in the area of Trebnje Administrative Unit by investing in appropriate capacities that will enable the strengthening of the public health role of the Trebnje health centre, the implementation of the upgrading of preventive examinations for children and adolescents, the implementation of preventive treatments for children and adolescents at risk, the implementation of renewed and upgraded prevention of chronic diseases, the inclusion of vulnerable persons in preventive healthcare and the introduction of a Community approach model for health promotion and reduction of health inequalities in the area of the Trebnje Administrative Unit. All the above activities will be carried out through the establishment of integrated health promotion centres (CKZ) as separate units in the health centre The objective of the project is to provide spatial and material conditions for the implementation of the activities of the Centre for Health Promotion. Ce projet vise à améliorer les conditions des soins de santé primaires dans le domaine de l’unité administrative de Trebnje en investissant dans des capacités appropriées qui permettront de renforcer le rôle du centre de santé de Trebnje en matière de santé publique, la mise en œuvre de l’amélioration des examens préventifs pour les enfants et les adolescents, la mise en place de traitements préventifs pour les enfants et les adolescents à risque, la mise en œuvre d’une prévention renouvelée et améliorée des maladies chroniques, l’inclusion des personnes vulnérables dans les soins de santé préventifs et l’introduction d’un modèle communautaire de promotion et de réduction des inégalités en matière de santé dans le domaine de l’unité administrative de Trebnje. Toutes les activités ci-dessus seront menées par la création de centres intégrés de promotion de la santé (CKZ) en tant qu’unités distinctes dans le centre de santé. L’objectif du projet est de fournir des conditions spatiales et matérielles pour la mise en œuvre des activités du Centre de promotion de la santé. (French)

Ziel des Projekts ist es, die Bedingungen für die primäre Gesundheitsversorgung im Bereich der Verwaltungseinheit Trebnje zu verbessern, indem in geeignete Kapazitäten investiert wird, die die Stärkung der Rolle der öffentlichen Gesundheit des Gesundheitszentrums Trebnje, die Durchführung von Präventivuntersuchungen für Kinder und Jugendliche, die Einführung von präventiven Behandlungen für gefährdete Kinder und Jugendliche, die Umsetzung einer erneuerten und verbesserten Prävention chronischer Krankheiten, die Einbeziehung schutzbedürftiger Personen in die präventive Gesundheitsversorgung und die Einführung eines Modells des Gemeinschaftskonzepts für die Gesundheitsförderung und den Abbau gesundheitlicher Ungleichheiten im Bereich der Trebnje-Verwaltungseinheit ermöglichen. Alle oben genannten Maßnahmen werden durch die Einrichtung integrierter Gesundheitsförderungszentren (CKZ) als separate Einheiten im Gesundheitszentrum durchgeführt. Ziel des Projekts ist es, räumliche und materielle Bedingungen für die Durchführung der Aktivitäten des Zentrums für Gesundheitsförderung zu schaffen. (German)
