    Objetivo general: Caracterizar el riesgo de transmisión del VIH en hombres que tienen relaciones sexuales con hombres (HSH) y que reciben tratamiento antirretroviral (TAR) efectivo (con supresión de la viremia plasmática) y su posible asociación con co-infecciones de transmisión sexual subclínicas. Diseño: Estudio transversal en el marco de una cohorte prospectiva de pacientes con infección por el VIH. Metodología: Se incluirán 114 HSH consecutivos en TAR con supresión virológica, que acepten participar en el estudio. Todos los pacientes rellenarán una encuesta sobre hábitos sexuales, se les recogerá una muestra de semen y se les realizará una anoscopia de alta resolución para recoger muestras rectales. Se cuantificará el RNA del VIH-1 en muestras rectales y en semen, y se determinará por PCR de la presencia de las siguientes copatógenos de transmisión sexual en secreciones rectales: Herpes simplex 1 y 2, Treponema pallidum, Haemophilus ducreyi, Chlamydia trachomatis incluyendo serotipos L, Neisseria gonorrhoeae, Micoplasma genitalium, Micoplasma hominis, Ureaplasma urealyticum, Ureaplasma parvum, Trichomonas vaginalis, y virus del papiloma humano (VPH). Se describirá el uso de correceptores (CCR5 vs CXCR4) del VIH-1 detectado en muestras rectales y semen. Se estudiará la asociación de la presencia del VIH-1 en las muestras genitales con las coinfecciones de transmisión sexual y con la conducta sexual ajustando por variables sociodemográficas, clínicas e inmunológicas.     General objective: Characterise the risk of HIV transmission in men who have sex with men (SHH) and who receive effective antiretroviral therapy (ART) (with plasma viremia suppression) and its possible association with subclinical sexually transmitted co-infections. Design: Cross-sectional study in a prospective cohort of patients with HIV infection. Methodology: 114 consecutive HSHs will be included in ART with virological suppression, which agrees to participate in the study. All patients will complete a survey on sexual habits, collect a semen sample and perform a high resolution anoscopy to collect rectal samples. HIV-1 RNA shall be quantified in rectal samples and semen and determined by PCR of the presence of the following sexually transmitted copatogens in rectal secretions: Herpes simplex 1 and 2, Treponema pallidum, Haemophilus ducreyi, Chlamydia trachomatis including L serotypes, Neisseria gonorrhoeae, Mycoplasma genitalium, Mycoplasma hominis, Ureaplasma urealyticum, Ureaplasma parvum, Trichomonas vaginalis, and human papillomavirus (HPV). The use of HIV-1 co-acceptors (CCR5 vs CXCR4) detected in rectal samples and semen shall be described. The association of the presence of HIV-1 in genital samples with sexually transmitted co-infections and sexual behavior will be studied, adjusting for sociodemographic, clinical and immunological variables.
