    La calorimetría diferencial de barrido (DSC) se ha revelado recientemente como una interesante técnica que proporciona información sobre la composición e interacciones (proteínas y metabolitos) en el plasma/suero sanguíneo. Es posible diferenciar entre personas sanas y pacientes de determinadas patologías, ya que la composición del plasma/suero se encuentra alterada considerablemente como resultado de la enfermedad. En un estudio previo que incluía a 60 pacientes con cáncer gástrico, realizado en nuestro grupo y publicado recientemente, se han encontrado diferencias significativas entre las variables procedentes de los perfiles calorimétricos de individuos sanos y enfermos, así como entre enfermos en diferentes estadios de la enfermedad. En este proyecto se propone validar e implementar una metodología (DIGCAL) como herramienta útil en diagnóstico y de seguimiento de patologías digestivas tumorales relevantes (adenoma ductal de páncreas, lesiones quísticas pancreáticas preneoplásicas y cáncer gástrico), así como el aislamiento e identificación de potenciales marcadores tumorales. Se analizarán los perfiles calorimétricos del suero sanguíneo de 35 sujetos sanos y 360 pacientes en total (120 pacientes de cada patología) en diferentes estadios de la enfermedad (30 para cada estadio I, II, III y IV), antes y 6 meses después de su tratamiento clínico. Tras el análisis multiparamétrico de los perfiles pretendemos establecer un procedimiento a través del cual sea posible: 1/ Cribado de un proceso tumoral determinado; 2/ Clasificar a los pacientes según el estadio del mismo; 3/ Monitorizar el avance/control de la enfermedad; 4/ Identificar biomarcadores específicos de cada tipo de cáncer. DIGCAL podría implantarse a nivel clínico no sólo como técnica diagnóstica, sino como sistema de monitorización o seguimiento del paciente durante el tratamiento terapéutico, aportando valor pronóstico o de selección del tratamiento farmacológico más apropiado. It is possible to differentiate between healthy people and patients of certain pathologies, as the composition of the plasma/serum is considerably altered as a result of the disease. In a previous study involving 60 patients with gastric cancer, conducted in our group and recently published, significant differences have been found between the variables from the calorimetric profiles of healthy and sick individuals, as well as between patients at different stages of the disease. This project aims to validate and implement a methodology (DIGCAL) as a useful tool in the diagnosis and follow-up of relevant tumor digestive pathologies (pancreatic ductal adenoma, preneoplastic pancreatic cystic lesions and gastric cancer), as well as the isolation and identification of potential tumor markers. The calorimetric blood serum profiles of 35 healthy subjects and 360 patients in total (120 patients from each pathology) will be analysed at different stages of the disease (30 for each stage I, II, III and IV), before and 6 months after clinical treatment. After the multiparametric analysis of the profiles we intend to establish a procedure through which it is possible: 1/Criving of a given tumor process; 2/Classify patients according to the stage of the patient; 3/Monitorising disease progression/control; 4/Identify specific biomarkers for each type of cancer. DIGCAL could be implanted at a clinical level not only as a diagnostic technique, but as a monitoring or follow-up system of the patient during therapeutic treatment, providing prognostic value or selection of the most appropriate pharmacological treatment.
