Proj (typ 2) jest skierowany do 100 os (66K) zagrożonych ubóstwem i wykluczeniem społecznym, w tym 95 os niesamodzielnych i 5 os niepełnosprawnych, zamieszkałych na terenie woj. wlkp w Gminie Wieleń oraz ich 30 opiekunów faktycznych (25 K).Celem proj jest utworzenie 75 miejsc świadczenia usług opiek. i asyst., zwiększenie liczby os objętych usług społ. w Gm. Wieleń o 95 os niesam. (63K) i 5 os niepełnospr. (3K) mającego służyć zaspokojeniu potrzeb tych os wynik. z niesam. poprzez m.in. utworzenie mieszk wspomag.,usługi opiek. i asyst. świadczone w miejscu zamieszkania i Klub Seniora oraz zwiększenie kompetencji 30 opiekunów fakt.(25K) z zakr. opieki nad podopiecznymi w okr. 01.01.2021-31.12.2022.Proj odpowiada na problemy i potrzeby w świadczeniu usług społ., zidentyfik. na obszarze jego real. biorąc pod uwagę trendy demograf. i poziom dostępności usług społ. na tym obszarze. Proj będzie skier. do os niesamodz. i ON. Zadania w ramach proj: utworzenie mieszk wspomaganego, wsparcie opiekuńcze, asystenckie, opiekuńcze specjal., sąsiedzkie usługi opiek., wsparcie opiekunów faktycznych, Klub Seniora, co przyczyni się do zwiększenia autonomii UP, zwiększenie dostępności i bezp dla podopiecznych. Usługi będą świadcz w oparciu o wysokie standardy przez specj kadrę, przygotowaną do świadczenia tych usług, w zdecydowanej większości w miejscach zamieszkania. Usługi te będą służyć zaspokojeniu rosnących potrzeb wynikających z niesam. Dzięki proj 95 osób niesam. (63 K) i 5 os niepełn. (3 K) będzie mogło otrzymać niezbędne wsparcie poza systemem instytucjonalizowanej pomocy, przez co dłużej będą mogły być aktywne w społ. lokalnej.Rezultaty:1. Liczba miejsc świadczenia usług społ. istniejących po zakończeniu proj: 70 (3 miejsca w mieszkaniu wspomaganym, 2 opiekunki, 1 pielęgniarka, 1 AON, 1 rehabilitant, 1 logopeda, 1 psycholog, 60 miejsc w Klubie Seniora)2. Proj (type 2) is aimed at 100 people (66K) at risk of poverty and social exclusion, including 95 dependent and 5 persons with disabilities, residing in the province of Wieleń and their 30 de facto guardians (25 K). The aim of the project is to create 75 places of care and assistance, increasing the number of persons covered by the services in Wieleń by 95 people. (63K) and 5 persons incomplete. (3K) designed to meet the needs of these persons, the result of the lack of the same. by, among others, the creation of support follicles, care and assistance services provided in the place of residence and Senior Club and the increase in the competence of 30 carers fact.(25K) from the care of the children in the circle. 01.01.2021-31.12.2022.Proj responds to problems and needs in the provision of social services, identification. in the area of its real. taking into account demographic trends. and the level of availability of services in this area. The proj will be a pointer to the idiosyncrasy and ON. Project tasks: the creation of assisted bellows, care support, assistance, care specialty., neighbourhood care services., support for real carers, Senior Club, which will help to increase the autonomy of the UP, increase accessibility and free of charge for the caregivers. The services will be provided on the basis of high standards by a specialist staff prepared to provide these services, in the vast majority of places of residence. These services will serve to meet the growing needs arising from failure. Thanks to the probe, 95 people are gone. (63 K) and 5 people incomplete. (3 K) will be able to receive the necessary support outside the system of institutionalised aid, which will allow them to be active in the local community for longer.Results:1. Number of service providers existing after the end of the project: 70 (3 seats in an assisted apartment, 2 babysitters, 1 nurse, 1 AON, 1 rehabilitator, 1 speech therapist, 1 psychologist, 60 seats in the Senior Club)2.
