A operação 3C AL - Changing Complex Care on Alentejo Litoral pretende melhorar a prestação de cuidados de saúde aos utentes que frequentam a ULSLA, nomeadamente a doentes crónicos complexos com multimorbilidades. Para tal tenciona integrar numa plataforma a informação clínica partilhada pelas diversas equipas técnicas, de modo a otimizar o registo, acesso e gestão da informação e potenciar a personalização do tratamento a realizar ao utente. Operation 3C AL – Changing Complex Care on Alentejo Litoral aims to improve the provision of healthcare to users attending ULSLA, namely complex chronic patients with multimorbidities.
