Celem proj. jest wzrost dostępności usług społecznych poprzez zwiększenie o 3 miejsca świadczenia usług społecznych i objęcie 10os. niesamodziel. (7K;3M) usługami społ., utworzenie wypożyczalni sprzętu wspomagająco-pielęgnacyjnego, zwiększającego samodzielność os. i niezbędnego do opieki nad os. niesamodzielnymi oraz wsparcie dla 10 opiek. faktycznych (5K;5M) poprzez realizacje szkoleń i poradnictwa specjalist. w okresie I.18-XII.19 na ter.Gm. Pełczyce (woj. zach-pom). W ramach proj. założone zostały działania: - Zawarcie indywid. kontraktów trójstronnych określających zakres wsparcia i wymiar godz. usług opiekuńczych. - Przygotowanie 1os. (1K) do świadczenia usług asyst. (AOON) i 1os. (1K) do świadczenia usług opiekuń. w miejscu zamieszk. poprzez udział w zaplanow. szkoleniach w ramach proj. zgodnie z Wytycznymi w zakresie realizacji przedsięwzięć w obszarze włączenia społ. i zwalczania ubóstwa z wykorzystaniem środków EFS i EFRR na lata 2014 -2020. - Usługi opiekuńcze świadczone w miejscu zamieszk. dla 5os. (4K;1M) niesamodzielnych i specjalist. usługi opiekuńcze świadczone w miejscu zamieszk. dla 4os. (3K;1M) oraz usługi asystenckie dla 5os. z niepełnospr.(3K;2M) na ter. Gm. Pełczyce w okresie I.18-XII.19. - Wsparcie 10 (5K;5M) opiek. faktycznych poprzez realizacje szkoleń oraz poradnictwa specjalist. w okresie I.18-XII.19. - utworzenie wypożyczalni sprzętu wspomagająco-pielęgnacyjnego w Gm. Pełczyce w okresie I-II.18r., która będzie funkcjonować przez okres realizacji proj. Wsparcie kierowane jest do 2 kandydatów na pełnienie usług społ., 10os. zagroż. ubóstwem i wyklucz. społ. (os. niesamodzielne) oraz 10 opiek. faktycznych os. The aim of the project is to increase the availability of social services by increasing the provision of social services by 3 and covering 10 people. (7K; 3M) community services, the creation of a rental equipment rental, increasing the independence of individuals and necessary for care of dependants and support for 10 actual care (5K;5M) through the implementation of training and guidance specialists during the period I.18-XII.19 on Ter.Gm. Pełczyce (Zach-pom). As part of the project, actions were set up: — Conclusion of individuals. tripartite contracts specifying the scope of support and the dimension of care services. — Preparation of 1os. (1K) for the provision of assistance services. (AOON) and 1os. (1K) to provide care services at the place of residence by participating in training plans under the project in accordance with the Guidelines for the implementation of projects in the area of social inclusion and combating poverty using ESF and ERDF funds for the period 2014-2020. — Care services provided at the place of residence. for 5persons. (4K;1M) dependants and specialists. care services provided at the place of residence. for 4 people. (3K; 1M) and Assistant Services for 5persons from a disadvantage (3K;2M) to ter. GM. Pełczyce in the period I.18-XII.19. — Support 10 (5K; 5M) actual care through the implementation of training and specialist guidance during the period I.18-XII.19. — the creation of a car rental facility in Gm. Pełczyce during the period I-II.18 which will function during the period of implementation of the project. Support is directed to 2 candidates for community service, 10 people. the threat of poverty and exclusion. (independent) and 10 carers of the actual wasps.
