Celem projektu jest: Zwiększenie aktywności społecznej, zawodowej i edukacyjnej 40 (24K/16M) osób w wieku od 15 do 64 roku życia (w tym 50% osób niepełnosprawnych) zagrożonych ubóstwem lub wykluczeniem społecznym, w szczególności z powodu długotrwałego bezrobocia,niepełnosprawności, niesamodzielności ze względu na wiek lub powodu pochodzenia z rodzin zagrożonych wykluczeniem społecznym z obszarów wiejskich województwa lubuskiego w okresie od 01.03.2017 do 31.05.2018 w oparciu o ścieżkę reintegracji, stworzoną indywidualnie dla każdej osoby. ramach projektu objęte wsparciem zostaną grupy marginalizowane. Każdy UP otrzyma kompleksowe wsparcie m.in. w zakresie zdiagnozowania potrzeb, warsztatów kompetencji społecznych, wsparcia pracownika socjalnego/asystenta rodziny, szkoleń zawodowych lub staży i pośrednictwa pracy. Wsparcie kierowane do uUP będzie zakładać obowiązkowe przeprowadzenie indywidualnej diagnozy społeczno-zawodowej dla każdego UP i dostosowanie wsparcia do jej wyników. Wskaźnik efektywności społeczno-zatrudnieniowej wyniesie 56%, w tym 22% wsk.efekt.zatr. (Polish)

The aim of the project is to: Increase social, occupational and educational activity 40 (24K/16M) of people aged 15 to 64 (including 50 % of people with disabilities) at risk of poverty or social exclusion, in particular due to long-term unemployment, disability, disability due to age or origin from families at risk of social exclusion from the rural areas of Lubuskie voivodship in the period 01.03.2017 to 31.05.2018, based on the reintegration path, created individually for each person. Each UP will receive comprehensive support, among others, in diagnosing needs, workshops of social competences, support of a social worker/assistant of the family, vocational training or internships and job placements. Support addressed to UUPs will assume mandatory individual socio-occupational diagnosis for each UP and align the support with its results. The socio-employment efficiency ratio will be 56 %, including 22 % in the year. (English)
