CEL GŁÓWNY: Poprawa funkcjonowania 25 ON (10 K i 15 M) w środowisku szkolnym, rodzinnym z 25 rodzin (23 K, 2 M) zagroż. ubóstwem i wyklucz. społ. zamieszkujących na terenie PS poprzez utworzenie PWD w formie opiekuńczo-specjalistycznej tj. Świetlicy terapeutycznej, współpracującej z rodzinami ON od 07.2020-04.2023. GD stanowią 25 R przeżywających trudności w wypełnianiu funkcji opiek.-wychow. ze względu na ON, korzyst. lub kwalifik.ze świadczeń z pomocy. GŁÓWNE DZIAŁANIA: • pomoc w opiece i wychowaniu dziecka poprzez usługi PWD w formie OPIEKUŃCZEJ (pomoc w nauce, organizacja czasu wolnego, zajęcia z 3 kompet. kluczowych, arteterapii) i SPECJALISTYCZNEJ (zajęcia logopedyczne i terapeutyczne, socjoterapia, terapia psycholog. i pedagogiczna); • Zespół ds. pracy z rodziną (konsultacje, poradnictwo specjalistyczne, prawne, psychologiczne, realizacja gr wsparcia). MAIN OBJECTIVE: Improvement of the functioning of 25 ONs (10 K and 15 M) in the school environment, family environment of 25 families (23 K, 2 M) at risk of poverty and exclusion of the population living in the territory of the PS by creating a PWD in the form of a caring and specialised form, i.e. Rehabilitation centre, working with ON families since 07.2020-04.2023. GDs are 25 Rs experiencing difficulties in fulfilling the functions of childcare. due to the ON, advantage or eligibility of assistance benefits. MAIN ACTIONS: • assistance in the care and upbringing of the child through PWD services in the form of care (help in science, organisation of leisure time, classes with 3 key subjects, artetherapy) and specialised (logic and therapeutic classes, sociotherapy, psychologist therapy and pedagogical therapy); • Family work team (consultation, specialist, legal, psychological counselling, implementation of a support game). OBJECTIF PRINCIPAL: Amélioration du fonctionnement de 25 ON (10 K et 15 M) dans le milieu scolaire, environnement familial de 25 familles (23 K, 2 M) menacées de pauvreté et d’exclusion de la population vivant sur le territoire du PS par la création d’une personne handicapée sous la forme d’une forme soignée et spécialisée, c’est-à-dire Centre de réadaptation, en collaboration avec les familles de l’Ontario depuis le 07.2020-04.2023. Les SMD sont 25 R qui éprouvent des difficultés à remplir les fonctions de garde d’enfants en raison de l’ON, de l’avantage ou de l’admissibilité des prestations d’aide. PRINCIPALES ACTIONS: • aide à la prise en charge et à l’éducation de l’enfant par le biais de services de soins aux personnes handicapées (aide à la science, organisation du temps libre, cours de 3 sujets clés, artéthérapie) et spécialisés (classes logiques et thérapeutiques, sociothérapie, psychologues et thérapies pédagogiques); • Équipe de travail familial (consultation, spécialiste, conseil juridique, psychologique, mise en œuvre d’un jeu de soutien). HAUPTZIEL: Verbesserung der Funktionsweise von 25 ONs (10 K und 15 M) im Schulumfeld, Familienumfeld von 25 Familien (23 K, 2 M), die von Armut und Ausgrenzung der Bevölkerung bedroht sind, die auf dem Gebiet der PS lebt, indem eine PWD in Form einer Betreuungs- und Fachform geschaffen wird, d. h. Rehabilitationszentrum, arbeiten mit ON Familien seit 07.2020-04.2023. GDS sind 25 Rs, die Schwierigkeiten bei der Erfüllung der Aufgaben der Kinderbetreuung haben. Aufgrund des ON, des Vorteils oder der Förderfähigkeit von Unterstützungsleistungen. WICHTIGSTE MASSNAHMEN: • Unterstützung bei der Betreuung und Erziehung des Kindes durch PWD-Dienstleistungen in Form von Betreuung (Hilfe in der Wissenschaft, Organisation der Freizeit, Klassen mit 3 Schlüsselfächern, Artetherapie) und spezialisierte (Logik- und Therapieklassen, Soziotherapie, Psychologentherapie und pädagogische Therapie); • Familienarbeitsteam (Beratung, Facharzt, Rechtsberatung, psychologische Beratung, Umsetzung eines Support-Spiels). HOOFDDOELSTELLING: Verbetering van de werking van 25 ONs (10 K en 15 miljoen) in de schoolomgeving, familiale omgeving van 25 gezinnen (23 K, 2 miljoen) die risico lopen op armoede en uitsluiting van de bevolking die op het grondgebied van de PS woont, door het creëren van een gehandicaptenrichtlijn in de vorm van een zorgzame en gespecialiseerde vorm, d.w.z. Revalidatiecentrum, werkend met ON families sinds 07.2020-04.2023.
