CELEM GŁÓWNYM PROJEKTU jest zwiększenie wykrywalności cukrzycy wśród 2.700 mieszkańców (1.620K, 1.080M) m.st. Warszawa oraz powiatu płockiego poprzez prowadzenie działań edukacyjnych z zakresu diabetologii oraz badań przesiewowych w kierunku wykrywania cukrzycy do 31.12.2021r. Planowane zadania są zgodne z założeniami Regionalnego Programu Zdrowotnego Samorządu Woj.Mazowieckiego (RPZ) i obejmują: akcję informacyjną (moduł I), konsultacje lekarskie i działania edukacyjne (moduł II) i szkolenia personelu medycznego (moduł III) . GŁÓWNI UCZESTNICY PROJ. objęci wsparciem bezpośrednim to osoby pow. 60 r.ż. mieszkające w Warszawie i pow. płockim dotychczas niediagnozowane w kierunku cukrzycy, które podpiszą druk świadomej zgody na udział w projekcie. Ponadto działaniami zostaną objęci również lekarze/pielęgniarki, POZ. ZAKŁADANY WSKAŹNIK REZULTATU: liczba wspartych w programie miejsc świadczenia usług zdrowotnych, istniejących po zakończeniu projektu. ZAKŁADANE WSK.PRODUKTU: liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami zdrowotnymi w programie. Pozostałe wskaźniki określone w RPZ będą wykazane i raportowane zgodnie z §30 pkt 3 wzoru umowy ryczałtowej dla przedmiotowego konkursu. Projekt spełni KRYTERIUM MERYTOR. The main purpose of the project is to increase the detection of diabetes among 2.700 inhabitants (1.620K, 1.080M) of Warsaw and the Płock county by conducting educational activities in the field of diabetology and screening for the detection of diabetes by 31.12.2021. The planned tasks are in line with the objectives of the Regional Health Programme of the Mazowieckie Regional Government and include: information action (Module I), medical consultations and educational activities (Module II) and training of medical staff (Module III). The main PARTIES OF PROJ. covered by direct support are people area. 60 years old living in Warsaw and the area of Płock, not yet diagnosed in the direction of diabetes, who will sign a print of informed consent to participate in the project. In addition, doctors/nurses, POZ will also take action. EXPECTED RESULT INDICATOR: number of post-project health service providers supported in the programme. ESTABLISHED WSK.PRODUCT: number of people at risk of poverty or social exclusion covered by health services in the programme. L’objectif principal du projet est d’accroître la détection du diabète chez 2 700 habitants (1,620K, 1,080M) de Varsovie et du comté de Płock en menant des activités éducatives dans le domaine de la diabétologie et du dépistage du diabète d’ici au 31.12.2021. Les tâches prévues sont conformes aux objectifs du programme régional de santé du gouvernement régional de Mazowieckie et comprennent: action d’information (Module I), consultations médicales et activités éducatives (Module II) et formation du personnel médical (Module III). Les principales PARTIES DU PROJ. couverts par un soutien direct sont la zone des personnes. 60 ans vivant à Varsovie et dans la région de Płock, non encore diagnostiqué dans la direction du diabète, qui signera une impression de consentement éclairé à participer au projet. En outre, médecins/infirmières, POZ prendra également des mesures. INDICATEUR DE RÉSULTAT ATTENDU: nombre de prestataires de services de santé post-projet bénéficiant d’un soutien dans le cadre du programme. ÉTABLI WSK.PRODUCT: nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale couvertes par les services de santé dans le cadre du programme. Hauptzweck des Projekts ist es, den Nachweis von Diabetes auf 2.700 Einwohner (1,620K, 1.080M) in Warschau und im Landkreis Płock zu erhöhen, indem bis zum 31.12.2021 Bildungsaktivitäten im Bereich Diabetologie und Screening zur Diabeteserkennung durchgeführt werden. Die geplanten Aufgaben stehen im Einklang mit den Zielen des regionalen Gesundheitsprogramms der Regionalregierung Mazowieckie und umfassen: Informationsmaßnahme (Modul I), medizinische Beratung und Bildungsaktivitäten (Modul II) und Ausbildung des medizinischen Personals (Modul III). Die wichtigsten von der direkten Unterstützung abgedeckten PARTIES OF PROJ. sind Personenbereich. 60 Jahre alt, die in Warschau und dem Gebiet von Płock leben, noch nicht diagnostiziert in Richtung Diabetes, der einen Druck der Einwilligung nach Aufklärung zur Teilnahme am Projekt unterzeichnen wird. Darüber hinaus werden Ärzte/Krankenschwestern, POZ auch Maßnahmen ergreifen. ERWARTETER ERGEBNISINDIKATOR: Zahl der im Rahmen des Programms unterstützten Gesundheitsdienstleister nach dem Projekt. GEGRÜNDET WSK.PRODUCT: Zahl der von Armut oder sozialer Ausgrenzung bedrohten Personen, die im Rahmen des Programms von Gesundheitsdiensten abgedeckt sind. Die anderen im RP genannten Indikatoren werden gemäß § 30 Abs. 3 des Muster-Pauschalvertrags für den betreffenden Wettbewerb ausgewiesen und gemeldet. Het belangrijkste doel van het project is de opsporing van diabetes onder 2.700 inwoners (1.620K, 1.080M) in Warschau en de provincie Płock te verhogen door middel van educatieve activiteiten op het gebied van diabetologie en screening voor de opsporing van diabetes tegen 31.12.2021. De geplande taken zijn in overeenstemming met de doelstellingen van het regionale gezondheidsprogramma van de regionale regering van Mazowieckie en omvatten: voorlichtingsactie (module I), medische consultaties en educatieve activiteiten (module II) en opleiding van medisch personeel (module III). De belangrijkste PARTIES VAN PROJ. die door directe steun worden gedekt, zijn mensengebied. 60 jaar oud wonend in Warschau en het gebied van Płock, nog niet gediagnosticeerd in de richting van diabetes, die een afdruk van geïnformeerde toestemming om deel te nemen aan het project zal ondertekenen. Lo scopo principale del progetto è quello di aumentare la rilevazione del diabete tra 2.700 abitanti (1.620K, 1.080M) di Varsavia e della contea di Płock conducendo attività educative nel campo della diabetologia e screening per l'individuazione del diabete entro il 31.12.2021. I compiti previsti sono in linea con gli obiettivi del programma regionale per la salute del governo regionale di Mazowieckie e comprendono: azione d'informazione (modulo I), consultazioni mediche e attività educative (modulo II) e formazione del personale medico (modulo III). Le principali PARTI DI PROGETTO coperte da sostegno diretto sono l'area delle persone. 60 anni che vivono a Varsavia e nella zona di Płock, non ancora diagnosticati nella direzione del diabete, che firmerà una stampa di consenso informato per partecipare al progetto. Inoltre, medici/infermieri, POZ adotterà anche azioni. INDICATORE DI RISULTATO PREVISTO: numero di fornitori di servizi sanitari post-progetto sostenuti nel programma. WSK.PRODUCT ISTITUITO: numero di persone a rischio di povertà o di esclusione sociale coperte dai servizi sanitari nel programma. El objetivo principal del proyecto es aumentar la detección de diabetes en 2.700 habitantes (1.620K, 1.080 millones) de Varsovia y el condado de Płock mediante la realización de actividades educativas en el campo de la diabetología y el cribado para la detección de la diabetes antes del 31.12.2021. Las tareas previstas se ajustan a los objetivos del Programa Regional de Salud del Gobierno Regional de Mazowieckie e incluyen: actividades de información (módulo I), consultas médicas y actividades educativas (módulo II) y formación del personal médico (módulo III). Las principales PARTES DEL PROJ. cubiertos por el apoyo directo son las personas. 60 años viviendo en Varsovia y el área de Płock, aún no diagnosticado en la dirección de la diabetes, que firmará una impresión de consentimiento informado para participar en el proyecto. Además, los médicos/enfermeros, POZ también tomarán medidas. INDICADOR DE RESULTADOS ESPERADO: número de proveedores de servicios de salud posteriores al proyecto que reciben apoyo en el programa.
