SPZOZ w Sokołowie Podlaskim w którego strukturze znajduje się POZ, NPL, AOS,szpital, rehabilitacja dysponuje pod tym samym adresem wolnostojącym budynkiem parterowym o pow.300m2 przystosowanym dla osób niepełnosprawnych,który będzie przeznaczony na DDOM. Przedmiotem projektu jest pomoc pacjentowi w powrocie do samodzielności w domu, edukacja pacjenta i jego rodziny w zakresie stwierdzonych deficytów pielegnacyjnych, diety i rehabilitacji oraz przeciwdziałanie postępującej niesamodzielności i izolacji społecznej. Dzięki realizacji projektu możliwa będzie poprawa stanu zdrowia, przywrócenie do samodzielnego życia, zapobiegnięcie wykluczeniu społecznemu osób starszych i schorowanych. Grupą docelową w projekcie są mieszkańcy powiatu sokołowskiego, pacjenci bezpośrednio po przebytej hospitalizacji, których stan zdrowia wymaga wzmożonej opieki pielęgniarskiej, nadzoru nad terapią farmakologiczną, kompleksowych działań usprawniających i przeciwdziałających postępującej niesamodzielności oraz wsparcia i edukacji zdrowotnej, a także doradztwa w zakresie organizacji opieki i leczenia osoby niesamodzielnej, a także pacjenci, którym w okresie ostatnich 12 miesięcy udzielone zostały świadczenia zdrowotne z zakresu leczenia szpitalnego - łącznie 90 osób K i M, w tym min. 75% osób pow. 65 r. ż. (68 osób). Są to osoby, których stan zdrowia nie pozwala na pozostawanie wyłącznie pod opieką podstawowej opieki zdrowotnej i ambulatoryjnej opieki specjalistycznej, a jednocześnie nie wymagają całodobowego nadzoru lekarskiego i pielęgniarskiego realizowanego w trybie stacjonarnym, którzy w ocenie skalą poziomu samodzielności (skala Barthel) otrzymali 40-65 punktów. The subject of the project is to help the patient return to independence at home, educate the patient and his family in terms of identified care deficits, diet and rehabilitation, and counteract progressive independence and social isolation. Through the implementation of the project, it will be possible to improve health, restore to independent life, prevent social exclusion of elderly and ill people. The target group in the project are residents of the Sokołowski county, patients immediately after hospitalisation, whose health condition requires increased nursing care, supervision of pharmacological therapy, comprehensive measures to improve and counteract progressive dependency and support and health education, as well as advice on the organisation of care and treatment of dependent persons, as well as patients who in the last 12 months received health services in the scope of hospital treatment – including 90 people K and M. 65 (68 people). SPZOZ à Sokolow Podlaski, dont la structure comprend POZ, NPL, AOS, hôpital et réhabilitation, a à la même adresse un bâtiment au rez-de-chaussée indépendant d’une superficie de 300 m² adapté aux personnes handicapées, qui sera dédié à la DDOM. L’objectif du projet est d’aider le patient à retrouver son autonomie à la maison, d’éduquer le patient et sa famille en ce qui concerne les déficits de soins identifiés, l’alimentation et la réadaptation, et de lutter contre la dépendance progressive et l’isolement social. Grâce à la mise en œuvre du projet, il sera possible d’améliorer la santé, de rétablir l’autonomie, de prévenir l’exclusion sociale des personnes âgées et des malades. Le groupe cible du projet est constitué des résidents du district de Sokołowski, des patients immédiatement après l’hospitalisation, dont la santé nécessite des soins infirmiers accrus, une surveillance pharmacologique, des mesures globales pour améliorer et contrer la dépendance progressive, le soutien et l’éducation sanitaire, ainsi que des conseils sur l’organisation des soins et des traitements pour les personnes dépendantes, ainsi que des patients qui ont reçu des services de soins de santé au cours des 12 derniers mois dans le domaine des soins hospitaliers — un total de 90 personnes K et M, dont au moins 75 % des personnes dans le domaine des soins hospitaliers. 65 ans (68 personnes). SPZOZ in Sokolow Podlaski, deren Struktur umfasst POZ, NPL, AOS, Krankenhaus und Rehabilitation, hat an der gleichen Adresse ein freistehendes Erdgeschossgebäude mit einer Fläche von 300 m² für Behinderte angepasst, die dem DDOM gewidmet ist. Ziel des Projekts ist es, den Patienten bei der Rückkehr zur Selbstversorgung zu Hause zu unterstützen, den Patienten und seine Familie in Bezug auf festgestellte Pflegedefizite, Ernährung und Rehabilitation aufzuklären und die fortschreitende Abhängigkeit und soziale Isolation zu bekämpfen. Dank der Umsetzung des Projekts wird es möglich sein, die Gesundheit zu verbessern, ein unabhängiges Leben wiederherzustellen, die soziale Ausgrenzung älterer und kranker Menschen zu verhindern. Zielgruppe des Projekts sind Bewohner des Bezirks Sokołowski, Patienten unmittelbar nach dem Krankenhausaufenthalt, deren Gesundheit eine verstärkte Pflege, pharmakologische Überwachung, umfassende Maßnahmen zur Verbesserung und Bekämpfung der fortschreitenden Abhängigkeit, Unterstützung und Gesundheitserziehung sowie Beratung bei der Organisation der Pflege und Behandlung von abhängigen Personen erfordert, sowie Patienten, die in den letzten 12 Monaten im Bereich der Krankenhausbehandlung Gesundheitsdienstleistungen erhalten haben – insgesamt 90 Personen K und M, darunter mindestens 75 % der Menschen im Bereich der Krankenhausbehandlung. 65 Jahre alt (68 Personen). Dabei handelt es sich um Personen, deren Gesundheitszustand es ihnen nicht erlaubt, ausschließlich in der Primärversorgung und ambulanter Spezialversorgung zu bleiben, und gleichzeitig keine 24-Stunden-Gesundheits- und Pflegeaufsicht im stationären Modus erfordern, die bei der Beurteilung des Grads der Unabhängigkeit (die Barthel-Skala) 40-65 Punkte erhielt. SPZOZ in Sokolow Podlaski, waarvan de structuur bestaat uit POZ, NPL, AOS, ziekenhuis en revalidatie, heeft op hetzelfde adres een vrijstaand gebouw op de begane grond met een oppervlakte van 300 m² aangepast voor gehandicapten, die zal worden gewijd aan de DDOM. Het doel van het project is de patiënt te helpen terugkeren naar zelfredzaamheid thuis, de patiënt en zijn/haar gezin te onderwijzen in termen van vastgestelde zorgtekorten, dieet en revalidatie, en progressieve afhankelijkheid en sociaal isolement te bestrijden. SPZOZ a Sokolow Podlaski, la cui struttura comprende POZ, NPL, AOS, ospedale e riabilitazione, ha allo stesso indirizzo un edificio indipendente al piano terra con una superficie di 300 m² adattato ai disabili, che sarà dedicato al DDOM. I destinatari del progetto sono i residenti del distretto di Sokołowski, i pazienti immediatamente dopo il ricovero, la cui salute richiede un aumento delle cure infermieristiche, la supervisione farmacologica, misure globali per migliorare e contrastare la dipendenza progressiva, il sostegno e l'educazione sanitaria, nonché consigli sull'organizzazione delle cure e delle cure per le persone non autosufficienti, nonché i pazienti che hanno ricevuto servizi di assistenza sanitaria negli ultimi 12 mesi nel campo delle cure ospedaliere — per un totale di 90 persone K e M, tra cui almeno il 75 % delle persone nel settore delle cure ospedaliere. 65 anni (68 persone).
