Projekt realizowany będzie w okresie 1.01.2021 30.06.2023 .Cel główny projektu: Aktywizacja społecznozawodowa 66 osób zagrożonych ubóstwem lub wykluczeniem społecznym na terenie Gminy Proszowice, w okresie realizacji projektu, w tym co najmniej 10 osób niepełnosprawnych. W ramach projektu planowana jest realizacja następujących działań: Identyfikacja indywidualnych potrzeb oraz potencjałów uczestnika, realizacja wsparcia w ramach aktywnej integracji poprzez aktywizację społeczno zawodową uczestników projektu, realizację pracy o charakterze socjalnym oraz aktywizację zawodową przy współpracy z wyspecjalizowanymi podmiotami. Główny rezultat projektu efektywność społecznozatrudnieniowa uczestników projektu 34 w wymiarze społecznym i 25 w wymiarze zatrudnieniowym. (Polish)

The project plans to carry out the following activities: Identification of individual needs and potentials of the participant, implementation of support as part of active inclusion through social activation of project participants, implementation of social work and professional activation in cooperation with specialised entities. Le projet sera mis en œuvre au cours de la période 1.01.2021 30.06.2023.L’objectif principal du projet: Activation socioprofessionnelle de 66 personnes menacées de pauvreté ou d’exclusion sociale dans la commune de Proszowice, pendant la période du projet, dont au moins 10 personnes handicapées. Le projet prévoit de mener à bien les activités suivantes: L’identification des besoins et des potentiels individuels du participant, la mise en œuvre d’un soutien dans le cadre de l’inclusion active par l’activation sociale des participants au projet, la mise en œuvre du travail social et l’activation professionnelle en coopération avec des entités spécialisées. Résultat principal de la performance socio-emploi des participants au projet 34 dans les dimensions sociale et 25 de l’emploi. (French)

Das Projekt wird im Zeitraum 1.01.2021 30.06.2023 durchgeführt.Das Hauptziel des Projekts: Sozioprofessionelle Aktivierung von 66 von Armut oder sozialer Ausgrenzung bedrohten Personen in der Gemeinde Proszowice während des Projektzeitraums, darunter mindestens 10 Menschen mit Behinderungen. Das Projekt plant die Durchführung folgender Tätigkeiten: Ermittlung individueller Bedürfnisse und Potenziale des Teilnehmers, Umsetzung der Unterstützung im Rahmen der aktiven Eingliederung durch soziale Aktivierung der Projektteilnehmer, Umsetzung der Sozialarbeit und berufliche Aktivierung in Zusammenarbeit mit spezialisierten Einrichtungen. Wichtigstes Ergebnis des Projekts Sozio-Beschäftigungsleistung der Projektteilnehmer 34 in der sozialen und 25 Beschäftigungsdimension. (German)

Het project zal worden uitgevoerd in de periode 1.01.2021 30.06.2023.De belangrijkste doelstelling van het project: Sociaal-professionele activering van 66 mensen die het risico lopen op armoede of sociale uitsluiting in de gemeente Proszowice tijdens de looptijd van het project, waaronder ten minste 10 personen met een handicap. Het project is van plan de volgende activiteiten uit te voeren: Identificatie van individuele behoeften en mogelijkheden van de deelnemer, uitvoering van steun als onderdeel van actieve inclusie door sociale activering van projectdeelnemers, uitvoering van maatschappelijk werk en professionele activering in samenwerking met gespecialiseerde entiteiten.
