Celem projektu jest reintegracja społeczna osób zagrożonych wykluczeniem społecznym, zamieszkujących zdegradowany infrastrukturalnie obszar miasta Gniew.Projekt zakłada objęcie wsparciem 70os(34K i 36M), zamieszkujących zdegradowany obszar miasta Gniew.Objęcie wsparciem danej grupy wynika z diagnozy przeprowadzonej w ramach LPR dla Gminy Gniew na lata 2016-2023,zatwierdzonego uchwałą Rady Miejskiej w Gniewie nr XXXIII/245/17 z dn.12.04.2017r. Cele szczegółowe: -wzrost aktywności społ.u co najmniej70% uczestników proj.; -wzrost umiejętności rozwiązywania problemów wychowawczych u co najmniej50% rodziców objętych proj.; -wzrost wiedzy z zakresu funkcjonowania rodziny u co najmniej50%rodziców objętych proj.; -wzmocnienie więzi społecznych u co najmniej 50%uczestników; -rozwój społeczno-kulturalno-edukacyjny u co najmniej 80%dzieci i młodzieży objętej wsparciem; -rozwój usług opiekuńczych w obszarze rewitalizacji. Projekt zakłada realizację następujących zadań: -Utworzenie Centrum Wsparcia Rodzin:grupowe i indywidualne poradnictwo psychologiczne,warsztaty z pedagogiem,poradnictwo prawne,warsztaty i konsultacje indywidualne z zakresu terapii uzależnień,warsztaty grupowe i konsultacje indywidualne w zakresie umiejętności rodzicielskich,organizacja czasu wolnego dzieci i młodzieży,warsztaty komputerowe,warsztaty z zakresu integracji społ.,animacje działań w sferze kultury,warsztaty-profilaktyka zdrowia i higieny,warsztaty budowy swojego wizerunku, steetworking,zakup wyposażenia. -Placówka wsparcia dziennego prowadzona w formule mieszanej:organizacja zajęć w placówce wspierająca rozwój dzieci,animacje oraz zakup wyposażenia. -Wsparcie dla osób starszych i niesamodzielnych: zatrudnienie opiekunów,organizacja opieki,wolontariat opiekuńczy. Głównym efektem projektu będzie podniesienie aktywności społ.u co najmniej 70%uczestników(49os:24K i 25M)oraz zwiększenie liczby miejsc świadczenia usług społ.o 24 w stosunku do dotychczas realizowanych przez gminę. The aim of the project is to reintegrate people at risk of social exclusion living in the degraded infrastructure area of the city of Gniew.The project assumes the support of 70os (34K and 36M), living in the degraded area of the city of Gniew.The inclusion of the support group results from the diagnosis carried out within the framework of the LPR for the Gniew Municipality for the years 2016-2023, approved by resolution of the Gniew City Council no XXXIII/245/17 from 12.04.2017. Specific objectives: —increase in social activity of at least 70 % of participants in the project; —increase the ability to solve parental problems in at least 50 % of the projected parents; —increasing knowledge of the functioning of the family in at least 50 % of the projected parents; — strengthening social ties in at least 50 % of participants; — socio-cultural and educational development in at least 80 % of children and young people supported; —development of care services in the area of revitalisation. The project assumes the following tasks: —Creating a Family Support Centre: group and individual psychological counselling, workshops with educator, legal advice, workshops and individual consultations in the field of addiction therapy, group workshops and individual consultations in the field of parental skills, organisation of free time of children and young people, computer workshops, workshops on social integration, animation of activities in the sphere of culture, workshops-prophylaxis of health and hygiene. —Day support facility run in a mixed formula: organisation of activities in the institution supporting the development of children, animations and purchase of equipment. —Support for the elderly and dependants: hiring carers, care organisation, caring volunteering. The main effect of the project will be to increase the social activity of at least 70 % oftheparticipants(49os:24K and 25M)and an increase in the number of public service places by 24 compared to so-far implemented by the commune. L’objectif du projet est la réinsertion sociale des personnes exposées au risque d’exclusion sociale vivant dans une zone d’infrastructures dégradées de la ville de Gniew. Le projet suppose un soutien pour 70 personnes (34K et 36M), vivant dans la zone dégradée de la ville de Gniew. Le soutien du groupe est basé sur le diagnostic effectué dans le cadre du LPR pour la municipalité de Gniew pour les années 2016-2023, approuvé par la résolution du conseil municipal de Gniew no XXXIII/245/17 de décembre 2017. Objectifs spécifiques: —augmentation de l’activité sociale d’au moins 70 % des participants au projet; —accroître la capacité de résoudre les problèmes parentaux chez au moins 50 % des parents couverts par le projet; —améliorer la connaissance du fonctionnement de la famille chez au moins 50 % des parents concernés par le projet; —renforcer les liens sociaux chez au moins 50 % des participants; le développement socioculturel et éducatif d’au moins 80 % des enfants et des jeunes soutenus; —développement des services de soins dans le domaine de la revitalisation. Le projet assume les tâches suivantes: —Création du Centre d’appui Rodzin:grupowe et de conseils psychologiques individuels,ateliers avec l’éducateur,conseils juridiques,ateliers et consultations individuelles dans le domaine de la thérapie de toxicomanie,ateliers de groupe et consultations individuelles dans le domaine des compétences parentales,organisation de temps libre pour les enfants et les jeunes,ateliers d’informatique,ateliers d’intégration sociale,animation d’activités dans le domaine de la culture,ateliers-prophylaxie de la santé et de l’hygiène,ateliers de leur image, steetworking, achat d’équipements. —La plateforme d’assistance quotidienne fonctionne sous forme de cours mieszanej:organizacja dans une institution qui soutient le développement, l’animation et l’achat d’équipements pour les enfants. Soutien aux personnes âgées et dépendantes: emploi de soignants, organisation de soins,soins bénévoles. Le principal effet du projet sera d’accroître l’activité sociale d’au moins 70 % des participants (49 %: 24 K et 25 M) et d’augmenter le nombre de prestataires de services publics de 24 par rapport à ceux fournis par la municipalité jusqu’à présent. Ziel des Projekts ist die soziale Wiedereingliederung von Menschen, die von sozialer Ausgrenzung bedroht sind, die in einem degradierten Infrastrukturgebiet der Stadt Gniew leben. Das Projekt übernimmt Unterstützung für 70 Personen (34K und 36M), die im degradierten Bereich der Stadt Gniew leben. Die Unterstützung der Gruppe basiert auf der Diagnose, die im Rahmen der LPR für die Gemeinde Gniew für die Jahre 2016-2023 durchgeführt wird, die von der Gniew Stadtrat Resolution XXXIII/245/17 vom Dezember 2017 gebilligt wurde. Spezifische Ziele: Erhöhung der sozialen Aktivität um mindestens 70 % der Projektteilnehmer; die Fähigkeit zur Lösung von Elternproblemen in mindestens 50 % der vom Projekt erfassten Eltern zu erhöhen; Verbesserung der Kenntnisse über die Funktionsweise der Familie bei mindestens 50 % der von dem Projekt erfassten Eltern; Stärkung der sozialen Bindungen in mindestens 50 % der Teilnehmer; soziokulturelle und pädagogische Entwicklung bei mindestens 80 % der unterstützten Kinder und Jugendlichen; Entwicklung von Pflegediensten im Bereich der Revitalisierung. Das Projekt übernimmt folgende Aufgaben: Schaffung des Unterstützungszentrums Rodzin:grupowe und individuelle psychologische Beratung,Workshops mit Erzieher,Rechtsberatung,Workshops und individuelle Beratung im Bereich Suchttherapie, Gruppenworkshops und individuelle Beratung im Bereich der elterlichen Fähigkeiten,Freizeitorganisation für Kinder und Jugendliche,Computerworkshops,Workshops im Bereich der sozialen Integration,Animation von Aktivitäten im Bereich Kultur,Workshops-Prophylaxe von Gesundheit und Hygiene,Werkstätten ihres Bildes, Steetworking, Kauf von Geräten. —Tagesunterstützungsplattform läuft in der Formel von mieszanej:organizacja Klassen in einer Einrichtung, die die Entwicklung, Animation und den Kauf von Ausrüstung unterstützt. Unterstützung für ältere und abhängige Menschen: Beschäftigung von Pflegekräften, Pflegeorganisation, Pflegefreiwilligkeit. Die Hauptwirkung des Projekts wird darin bestehen, die soziale Aktivität von mindestens 70 % der Teilnehmer (49os:24K und 25M) zu erhöhen und die Zahl der öffentlichen Dienstleistungserbringer um 24 im Vergleich zu den bisher von der Gemeinde erbrachten Dienstleistungen zu erhöhen. Het doel van het project is de sociale reïntegratie van mensen die het risico lopen op sociale uitsluiting in een aangetast infrastructuurgebied van de stad Gniew. Het project gaat uit van steun voor 70 mensen (34K en 36M), wonend in het aangetaste gebied van de stad Gniew. De steun van de groep is gebaseerd op de diagnose die is uitgevoerd in het kader van de LPR voor de gemeente Gniew voor de jaren 2016-2023, goedgekeurd bij Resolutie nr. Het project heeft de volgende taken: —Oprichting van het ondersteuningscentrum Rodzin:grupowe en individuele psychologische begeleiding,workshops met opvoeder,juridisch advies, workshops en individuele raadplegingen op het gebied van verslavingstherapie, groepsworkshops en individuele raadplegingen op het gebied van ouderlijke vaardigheden, organisatie van vrije tijd voor kinderen en jongeren, computerworkshops, werkplaatsen op het gebied van sociale integratie,animatie van activiteiten op het gebied van cultuur, werkplaatsen-profylaxe van gezondheid en hygiëne,workshops van hun imago, steetworking, aankoop van apparatuur. —Day support platform wordt uitgevoerd in de formule van mieszanej:organizacja klassen in een instelling die de ontwikkeling, animatie en aankoop van apparatuur voor kinderen ondersteunt. Steun voor ouderen en afhankelijke personen: werkgelegenheid van mantelzorgers, zorgorganisatie, zorgvrijwilligers. Het belangrijkste effect van het project zal zijn de sociale activiteit van ten minste 70 % van de deelnemers (49os:24K en 25 miljoen) te verhogen en het aantal aanbieders van openbare diensten met 24 te verhogen ten opzichte van de tot dusver door de gemeente verstrekte diensten.
