Celem projektu jest rozwój deinstytucjonalizacji opieki nad osobami niesamodzielnymi w środowisku lokalnym w wyniku utworzenia Dziennego DomuOpieki Medycznej w Gminie Pałecznica dla 72 osób w tym dla 44 osób starszych 65 zamieszkałych na terenie powiatu proszowickiego jak również wsparcie 72 opiekunów nieformalnych osób niesamodzielnych;Projekt przyczynia się do realizacji celu poprzez zwiększenie dostępności usług medycznych oraz zmianę struktury zapewniania usług med. os. szczególnie powyżej 65 r.ż. tj. deinstytucjonalizację opieki nad os. zależnymi świadczonej w warunkach zbliżonych do domowych.Grupa docelowa:72 os, w tym 43 kobiet 59,5 korzystających z opieki w DDOM, dalej zwanymi pacjentami zamieszkujący w roz. KC na terenie obszaru pow.proszowickiegi, w tym co najmniej 44os. w wieku 65 29K,15M, które: są bezpośrednio po hospitalizacji i ich stan zdrowia wymaga wzmożonej opieki pielęgn, nadzoru nad terapią farmak, kompleksowych działań uspraw. I przeciwdziałających postępującej niesamodzielności oraz wsparcia i eduk. zdr. w okresie ostatnich 12 mc udzielone zostały świadczenia zdrowotne z zakresu leczenia szpitalnego.opiekunowie nieformalni osób niesamodzielnych;DDOMZadania1. The aim of the project is to develop the deinstitutionalisation of care for dependent persons in the local environment as a result of the creation of the Daily Healthcare House in the Pałecznica Municipality for 72 persons including 44 elderly persons 65 residing in the district of Proszowice as well as support of 72 non-formal care carers for dependent persons.The project contributes to achieving the goal by increasing the availability of medical services and changing the structure of the provision of medical services especially over 65. KC in the area of Proszowickiegi district, including at least 44 people at the age of 65 29K,15M, which: they are immediately after hospitalisation and their state of health requires increased care, supervision of Pharmac therapy, comprehensive care activities. L’objectif du projet est de développer la désinstitutionnalisation des soins aux personnes dépendantes dans l’environnement local grâce à la création du Centre de soins médicaux quotidiens dans la municipalité de Pałecznica pour 72 personnes, y compris pour 44 personnes âgées 65 résidents dans le district de proszowickie, ainsi qu’un soutien à 72 soignants informels de personnes dépendantes. Le projet contribue à la réalisation de l’objectif en augmentant la disponibilité des services médicaux et en modifiant la structure de la prestation des services médicaux, en particulier de plus de 65 ans, c’est-à-dire la désinstitutionnalisation des soins aux personnes dépendantes dans des conditions similaires à leur domicile.
