PITES PI15/00583 project seeks for the development of a technological collaborative and predictive solution based on applications, smart devices and decision support systems for the different providers involved in the chain of care in the home hospitalization. The project will permit to test a methodology of care, patient-centered and proactive, to promote an integrated care (social and health) on the home hospitalization with the aid of the information and communication technologies. Through this technological platform the aim is on the enhancement of the patient attention by sharing information between all actors, allowing the coordination of tasks, with the availability of decission support systems based on the generated information, integrated with the already existing information systems and looking for the optimization of resources. Reasons for collaboration between Health and Social Systems, enhacement of the integrated care of the patients, equity in the access to health resources, sustainability of the Welfare Systems and the pursuit of excellence in the quality of the services offered have inspired SALUD-Sector Barbastro in the design of this project. Involving the social partners in the provision of integrated care during home hospitalization. (Spanish)

Le projet Pites PI15/00583 vise le développement d’une solution technologique collaborative et prédictive basée sur des applications, des dispositifs intelligents et des systèmes d’aide à la décision pour les différents prestataires impliqués dans la chaîne de soins dans l’hospitalisation à domicile. Le projet permettra de tester une méthodologie de soins, centrée sur le patient et proactive, afin de promouvoir une prise en charge intégrée (sociale et santé) sur l’hospitalisation à domicile à l’aide des technologies de l’information et de la communication. Grâce à cette plate-forme technologique, l’objectif est d’accroître l’attention du patient en partageant des informations entre tous les acteurs, en permettant la coordination des tâches, avec la disponibilité de systèmes de soutien à la décission basés sur les informations générées, intégrés aux systèmes d’information existants et à la recherche d’une optimisation des ressources. Les raisons de la collaboration entre la santé et les systèmes sociaux, l’amélioration des soins intégrés des patients, l’équité dans l’accès aux ressources de santé, la durabilité des systèmes de protection sociale et la poursuite de l’excellence dans la qualité des services offerts ont inspiré SALUD-Sector Barbastro dans la conception de ce projet. Associer les partenaires sociaux à la fourniture de soins intégrés pendant l’hospitalisation à domicile. (French)
