CELEM GŁÓWNYM PROJEKTU jest wzrost świadomości wśród 2.350 mieszkańców m.st. Warszawy i powiatu piaseczyńskiego gm.: Góra Kalwaria, Konstancin-Jeziorna, Piaseczno, Tarczyn, Lesznowola, Prażmów; m.: Góra Kalwaria, Konstancin-Jeziorna, Piaseczno i Tarczyn (w tym 940 M i 1.410 K) związanych z zagrożeniami jakie niesie cukrzyca i objęcie ich konsultacjami lekarskimi w kierunku wykrywania cukrzycy do 30.08.2019r. Planowane zadania są zgodne z założeniami Regionalnego Programu Zdrowotnego Samorządu Woj.Mazowieckiego (RPZ) i obejmują: akcję informacyjną (moduł I), konsultacje lekarskie i działania edukacyjne (moduł II) i szkolenia personelu medycznego (moduł III) . GŁÓWNI UCZESTNICY PROJ. objęci wsparciem bezpośrednim to osoby pow. 60 r.ż. mieszkające w Warszawie i pow.piaseczyńskim dotychczas niediagnozowane w kierunku cukrzycy, które podpiszą druk świadomej zgody na udział w projekcie. Ponadto działaniami zostaną objęci również lekarze/pielęgniarki, personel placówek prowadzących działalność leczniczą. ZAKŁADANY WSKAŹNIK REZULTATU: liczba wspartych w programie miejsc świadczenia usług zdrowotnych, istniejących po zakończeniu projektu. ZAKŁADANE WSK.PRODUKTU: liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami zdrowotnymi w programie, liczba personelu medycznego uczestniczącego w szkoleniach oraz liczba wykładów dla pacjentów dotyczące w szczególności diety i aktywności fizycznej. Pozostałe wskaźniki określone w RPZ będą wykazane i raportowane zgodnie z §30 pkt 3 wzoru umowy ryczałtowej dla przedmiotowego konkursu. Wnioskodawca we wsk.rezultatu nr 2 oraz wskaźnikach produktu nr 3, 4, 5, 6, 7, i 8 wskazał wartości docelowe na poziomie "0" - ich oszacowanie jest możliwe wyłącznie na etapie realizacji proj. i będzie raportowane zgodnie z zapisami umowy o dofinasowanie - inform.zostaną przekazane w terminie określonym w §30 pkt 3 wzoru umowy ryczałtowej. The main aim of the project is to increase awareness among the 2,350 inhabitants of Warsaw and Piaseczyński district: Mount Kalwaria, Konstancin-Jeziorna, Piaseczno, Tarczyn, Lesznowola, Prażmów; m.: Mount Kalwaria, Konstancin-Jeziorna, Piaseczno and Tarczyn (including 940 M and 1.410 K) related to the risks of diabetes and their medical consultation on diabetes detection by 30.08.2019. The planned tasks are consistent with the assumptions of the Regional Health Programme of the Mazowieckie Voivodship (RPZ) and include: information campaign (Module I), medical consultations and educational activities (module II) and training of medical staff (module III). The main participants of the project are persons covered by direct support. 60 years old, living in Warsaw and Pot. Piaseczyński not yet diagnosed with diabetes, who will sign the printing of informed consent to participate in the project. In addition, the activities will also include doctors/nurses, staff of medical establishments. EXPECTED RESULT INDICATOR: number of post-project health services supported by the programme. ASSUMED WSK.PRODUCT: the number of people at risk of poverty or social exclusion covered by health services in the programme, the number of medical staff participating in the training and the number of lectures for patients on diet and physical activity in particular. Other indicators specified in RPZ will be shown and reported in accordance with §30 point 3 of the model flat-rate agreement for the contest. The applicant in the result No 2 and the Product Indicators No 3, 4, 5, 6, 7, and 8 indicated the target values at the level “0” – their estimation is possible only at the stage of the project implementation and will be reported in accordance with the provisions of the financing agreement – information will be provided within the deadline specified in §30 point 3 of the model flat-rate agreement. L’objectif principal du projet est de sensibiliser les 2 350 habitants du district de Varsovie et de Piaseczyński: Mont Calvary, Konstancin-Jeziorna, Piaseczno, Tarczyn, Lesznowola, Prażmów; M.: Mount Kalwaria, Konstancin-Jeziorna, Piaseczno et Tarczyn (y compris 940 M et 1.410 K) liés aux risques de diabète sucré et faisant l’objet d’une consultation médicale pour la détection du diabète d’ici au 30 août 2019. Les tâches prévues sont conformes aux objectifs du programme régional de santé du gouvernement régional de Mazowieckie et comprennent: action d’information (Module I), consultations médicales et activités éducatives (Module II) et formation du personnel médical (Module III). Les principales PARTIES DU PROJ. couverts par un soutien direct sont la zone des personnes. 60 ans vivant à Varsovie et dans la région de Piaseczyński non encore diagnostiqué dans la direction du diabète, qui signera une impression de consentement éclairé à participer au projet. En outre, les médecins/infirmières et le personnel des établissements médicaux seront également ciblés. INDICATEUR DE RÉSULTAT ATTENDU: nombre de prestataires de services de santé post-projet bénéficiant d’un soutien dans le cadre du programme. ÉTABLI WSK.PRODUCT: le nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale couvertes par les services de santé dans le cadre du programme, le nombre de membres du personnel médical participant à la formation et le nombre de conférences à l’intention des patients sur l’alimentation et l’activité physique en particulier. Les autres indicateurs spécifiés dans le RP seront indiqués et communiqués conformément à l’article 30, paragraphe 3, du modèle de contrat forfaitaire pour le concours en question. Dans le résumé no 2 et les indicateurs de réalisation nos 3, 4, 5, 6, 7 et 8, le demandeur a indiqué les valeurs cibles au niveau «0» — leur estimation n’est possible qu’au stade de la mise en œuvre du projet et sera communiquée conformément aux dispositions de la convention de subvention — informations.Ils seront fournis dans le délai spécifié au paragraphe 30, paragraphe 3, du modèle de convention forfaitaire.
