    El objetivo principal del proyecto One-Step ICTUS es el abordaje del proceso asistencial (PA) integral del paciente con ictus a través de las TIC y la implementación de las tecnologías digitales que permitan maximizar la seguridad, calidad y eficiencia en el diagnóstico y tratamiento._x000D_ La finalidad última del proyecto es maximizar la calidad y eficiencia de la atención asistencial, a través de la implementación de un novedoso modelo de atención urgente del paciente con ictus, el uso y fusión de diferentes tecnologías digitales, el fomento del apoderamiento del paciente y familiares para el manejo de la enfermedad, y la corresponsabilidad entre el Hospital Vall d¿Hebron y la industria médica, durante todo el ciclo de asistencial del paciente. _x000D_ El proyecto contempla un conjunto de actuaciones orientadas a optimizar el proceso asistencial:_x000D_ ¿ Desarrollo e implementación de una solución TIC que de soporte y agregue toda la información asociada al PA del ictus, facilite la comunicación e intercambio de información entre todos los profesionales involucrados y los pacientes y/o cuidadores, y sea integrable dentro del sistema de información hospitalario._x000D_ ¿ Implementación de un nuevo modelo de atención urgente al paciente con ictus que garantice contar con los recursos humanos y técnicos necesarios, y ofrecer un continuo asistencial de excelencia en el menor tiempo posible, optimizando los beneficios de la recanalización precoz del paciente. _x000D_ ¿ Desarrollo de un programa de rehabilitación que potencie el seguimiento del paciente a nivel domiciliario y mejore la información del paciente y familiares a través de la Plataforma TIC diseñada._x000D_ _x000D_ _x000D_ ¿ Mejora de la gestión integral del proceso asistencial con la implementación y uso de cuadros de mando e indicadores que permitan conocer los resultados alcanzados, vinculados a la Plataforma._x000D_ El proyecto One-Step ICTUS tiene una duración prevista de 48 meses, y requiere de una financiación de 10 millones de euros para cubrir todas las actuaciones planteadas._x000D_ Los resultados clínicos esperados supondrían una reducción del tiempo puerta-punción de 75 a 18 minutos, pasando de una tasa actual de recuperación clínica espectacular del 27% al 48%, y una ausencia de discapacidad a los 3 meses del 45% al 60% de los pacientes del nuevo proceso. Además, el proyecto permitirá monitorizar y evaluar los resultados en salud alcanzados, y gestionar mejor y optimizar la atención asistencial prestada a estos pacientes._x000D_ Finalmente, el proyecto representa una forma innovadora de dar respuesta a una necesidad asistencial compleja: el diagnóstico, tratamiento y seguimiento del paciente con ictus (Spanish)

    HUVH is one of the reference centers in Catalonia for the treatment of Ictus, and wants to continue leading the innovation and the improvement of the welfare processes for the reduction of response times and thus benefit the patient and society as a whole ._x000D_ The goal of the OneStep Ictus operation is the approach of the patient's stroke patient care process (PA). The use of ICT to make the digital transformation of the new process and the new technologies will maximize security, quality and efficiency in the diagnosis and treatment. However, it will promote the empowerment of the patient and relatives for the management of the disease during the entire care cycle, including follow-up in primary care and at home._x000D_ Objectives of the OneStep Ictus operation:_x000D_ -Monitor the integral cycle: detection, transfer, treatment and monitoring through a digital platform (INNOVICTUS), with the patient's empowerment_x000D_ -The design and implementation of a new comprehensive assistance model for the diagnosis, treatment, rehabilitation and follow-up of the patient with stroke, guaranteeing the co-responsibility of the patient_x000D_ -Respond to healthcare needs with a new paradigm of stroke care, which allows the reception of patients with stroke suspected in an "all-in-one Angio Suite"_x000D_ -Progress in the study and the search for better final results in time-dependent cerebral vascular pathology, either ischemic as hemorrhagic and act as a reference_x000D_ _x000D_ In Catalonia there are more than 21,000 stroke per year (287 cases / 100,000hab), of which 15,000 request urgent assistance. Of these, 12,500 are ischemic and 2,500 haemorrhagic. The type and severity of the sequels of a stroke are mainly determined by the speed of their diagnosis and treatment. The Ictus Code is an emergency code for the identification and urgent transfer to the most appropriate hospital._x000D_ _x000D_ At our Hospital, we receive 1,200 patients each year with stroke code, of which 960 are ischemic. Of these, 450 receive treatment (300 mechanical thrombectomy and 150 pharmacological treatment, rTPA). Of the 240 haemorrhagic stroke, about 50 patients have subarachnoid hemorrhage (HSA) and are treated with endovascular embolization, and about 50 have intraparenchymal bleeding and are treated with medication (Prothomplex)._x000D_ _x000D_ The main challenge in all these patients is to shorten the time of diagnosis and treatment. To achieve this, One Step Ictus is based on two fundamental components:_x000D_ -Digital platform ("INNOVICTUS")_x000D_ -Reengineering of the care process ("ALL-IN-ONE ANGIO SUITE")._x000D_ _x000D_ The clinical results expected in the main group of the project, acute ischemic stroke of a mechanical thrombectomy, would lead to a reduction in the time of administration of 75 to 18 minutes, from a current clinical recovery rate of 27% to 48%, and an absence of disability at 3 months from 45% to 60% with the new process._x000D_ _x000D_ In the case of other stroke, there is an undeniable beneficial effect both in the HSA in poor clinical condition, as well as in the rapid pharmacological treatment with rtPA in ischemic stroke and Prothromplex in intraparenchyma femoral bleeds._x000D_ _x000D_ The results of this project would be easily extrapolated to the rest of the hospitals in Catalonia, and they would provide the Primary Care of follow-up tools for the high-value post-patient patient. In addition, the platform would facilitate the extraction, comparison and exploitation of data for its subsequent study and improvement of the general processes._x000D_ _x000D_ Main results:_x000D_ _x000D_ ¿ INNOVICTUS platform_x000D_ -Early detection of stroke in Primary Care and immediate referral to the Hospital_x000D_ -Direct communication with the transfer of clinical data between SEM and Emergency Hospital or Empowerment of the patient and his relatives_x000D_ -Implementation of telemedicine with the rehabilitation at the patient's home_x000D_ -Digital transformation of the process with the collection of transverse indicators during all phases_x000D_ -Reengineering PA ("ALL-IN-ONE ANGIO SUITE")_x000D_ -Reduction of the time of treatment (English)
