Целта на проектa е да подобри качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора в общината, чрез осигуряване на мрежа от услуги в домашна среда и изграждане на подходящ материален и кадрови капацитет за предоставянето им чрез въвеждане на модел за патронажни грижи за възрастни хора и лица с увреждания, вкл. с хронични заболявания и трайни увреждания за осигуряване на почасови мобилни интегрирани здравно-социални услуги в техните домове. The aim of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly in the municipality, by providing a network of services in the home environment and building appropriate material and personnel capacity for their provision by introducing a model for patronage care for elderly people and persons with disabilities, including chronic diseases and permanent disabilities to provide hourly mobile integrated health-social services in their homes. L’objectif de ce projet est d’améliorer la qualité de vie et les possibilités d’inclusion sociale des personnes handicapées et des personnes âgées dans la municipalité, en mettant en place un réseau de services dans l’environnement familial et en mettant en place des capacités matérielles et humaines appropriées pour leur fourniture en introduisant un modèle de prise en charge des personnes âgées et des personnes handicapées, y compris les maladies chroniques et les incapacités permanentes, afin de fournir des services sanitaires et sociaux intégrés et mobiles à domicile.
