Целта на проекта е подобряване на качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора от Община Ивайловград, чрез осигуряване на услуги в домашна среда. Интегрираната услуга предвижда мобилни здравно-социални услуги до 2 часа на ден за период от 12 месеца и включва: - асистентска почасова подкрепа в дома за дейности, в които потребителят среща трудности (почистване на дома, пазаруване, готвене, придружаване, административни дейности, общуване, др.);- професионална подкрепа от здравен асистент/рехабилитатор и социален работник (информиране, консултиране, рехабилитационна подкрепа, в зависимост от потребностите и възможностите в домашна среда);- дистанционна подкрепа в ситуация на риск (чрез подаване на сигнал чрез телекеър оборудване, което води до връзка с мониторинг център и/ или посещение в дома на потребителя от посочено от потребителя лице).Предвидените услуги ще бъда предоставени на 50 потребители от всички населени места в Община Ивайловград: - Възрастни хора над 65 г. с ограничения или в невъзможност за самообслужване - 50 лица, включваща и Хора с увреждания и техните семейства- 8 лица;Община Ивайловград ще възложи услугата на общинско социално предприятие "Социален патронаж". За организиране и предоставяне на предвидените услуги ще бъдат ангажирани 10 лица, които ще преминат специализирани обучения за предлагане на мобилните здравно -социални услуги. (Bulgarian)

The aim of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and elderly people from the Municipality of Ivaylovgrad by providing services in a home environment. The integrated service provides mobile health-social services up to 2 hours per day for a period of 12 months and includes: — hourly assistant support in the home for activities where the consumer encounters difficulties (cleaning the home, shopping, cooking, accompanying, administrative activities, communication, etc.);- professional support from a health assistant/rehabilitator and social worker (information, counselling, rehabilitation support, depending on the needs and opportunities in a home environment); — remote support in a situation of risk (by reporting via telecare equipment, which leads to a connection to a monitoring centre and/or a visit to the consumer’s home by a person designated by the user).The services provided will be provided to 50 users from all settlements in the Municipality of Ivaylovgrad: — Elderly people over 65 years old with restrictions or inability to self-service — 50 persons, including people with disabilities and their families — 8 persons; Municipality of Ivaylovgrad will entrust the service to the municipal social enterprise “Social patronage”. 10 persons will be involved in organising and providing the envisaged services, who will undergo specialised trainings for the provision of mobile health-social services. L’objectif du projet est d’améliorer la qualité de vie et les possibilités d’inclusion sociale des personnes handicapées et des personnes âgées de la municipalité d’Ivaylovgrad en fournissant des services dans un environnement familial. Le service intégré fournit des services sanitaires et sociaux mobiles jusqu’à 2 heures par jour pour une période de 12 mois et comprend: — assistance horaire à domicile pour les activités où le consommateur rencontre des difficultés (nettoyage de la maison, achats, cuisine, accompagnement, activités administratives, communication, etc.);- soutien professionnel d’un assistant de santé/réhabilitation et d’un travailleur social (information, conseil, aide à la réadaptation, en fonction des besoins et des possibilités dans un environnement familial); — soutien à distance dans une situation de risque (par rapport à l’équipement de télésoins, ce qui conduit à une connexion à un centre de surveillance et/ou à une visite au domicile du consommateur par une personne désignée par l’utilisateur).Les services fournis seront fournis à 50 utilisateurs de tous les établissements de la municipalité d’Ivaylovgrad: — Personnes âgées de plus de 65 ans avec restrictions ou incapacité de libre-service — 50 personnes, y compris les personnes handicapées et leur famille — 8 personnes; la municipalité d’Ivaylovgrad confiera le service à l’entreprise sociale municipale «patronage social». Dix personnes seront associées à l’organisation et à la prestation des services envisagés, qui suivront des formations spécialisées pour la fourniture de services mobiles de santé et de services sociaux.
