Поставените цели в проектното предложение на Община Исперих са фокусирани върху реализирането на целите на Оперативна програма "Развитие на човешките ресурси" 2014-2020 заложени в:Приоритетна ос № 2 „Намаляване на бедността и насърчаване на социалното включване“;Инвестиционен приоритет № 3 „Подобряване на достъпа до услуги, които са на достъпна цена, устойчиви и висококачествени, включително здравни и социални услуги от общ интерес“;Специфична цел № 2 към ИП 3 „Намаляване броя на възрастните и хората с увреждания, настанени в институции чрез предоставяне на социални и здравни услуги в общността, включително услуги за дългосрочна грижа“.Общата цел на проектното предложение е подобряване на качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора, чрез осигуряване на мрежа от услуги в домашна среда и изграждане на подходящ капацитет за предоставянето им.Специфичната цел на проектното предложение е да се изгради модел за патронажни грижи за възрастни хора и лица с увреждания, включително с хронични заболявания и трайни увреждания с цел осигуряване на почасови мобилни интегрирани здравно-социални услуги в техните домове.Целевите групи са:Възрастни хора над 65г. с ограничения или в невъзможност за самообслужване;Хора с увреждания и техните семейства.Дейностите заложени в проектното предложение гарантират постигането на целта на настоящата процедура на директно предоставяне на безвъзмездна финансова помощ.Те са насочени изцяло към възрастни хора над 65г. с ограничение или невъзможност за самообслужване, както и към хора с увреждания и техните семейства, хора, които нямат близки, и за които има социална необходимост от почасови услуги като по този начин се защитава обществения интерес и се отговаря на нуждите на населението.Изпълнението на дейностите ще доведе до подобряване на качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора, чрез осигуряване на мрежа от услуги в домашна среда.
