Projekt realizuje Fundacja Elbląg w partnerstwie ze Stowarzyszeniem dla Europy. Projekt wdraża produkt finalny projektu innow: "Wybierz przyszłość-innowac.narzędzia wspom.interwencję socjalną". Cel: Zwiększenie aktywności i integracji 90 osób w wieku 15-64 lata zagroż. ryzykiem ubóstwa i/lub wykl. społecz. oraz ich rodzin i otoczenia z Elbląga i pow. elbląsk. poprzez dostępność do wysokiej jakości usług społ. w okresie 10.2017-01.2021 r. Zadania: Indywid. diagnoza - identyfikacja potrzeb, Aflatoun - program edukacji społ.-finan., poradnictwo specjalis. (prawne,psycholog.,porady dla osób niepełnospr. i aktyw. zaw.),terapia indyw. i grupowa,warsztaty aktywizacyjne (z treningu interpers.,rozwoju osobistego, przedsiębiorczości), warsztaty prozdrowotne (dot. profilaktyki zdrow. oraz indywid. poradnictwo dietet.). Kluczowym zad. projektu są działania prewencyjne. Gr.doc.: 90 (54K,36M) osób w wieku 15-64 z Ega i pow.elbl. (woj. warm.-maz.) zagroż. ryzykiem ubóstwa i/lub wykl. społ. w tym osoby bezrob.-(3 grupa tzw. oddalonych od rynku pracy), korzyst. z PO PŻ oraz rodziny/otoczenie osób wykl. społ.(w takim zakr., w jakim jest to niezbędne dla wsparcia UP).Każdy z UP (i ich rodziny/otoczenie) zostanie obj. min. 3 formami wsparcia. Efekty: Cel osiągniemy przez przeprowadz. 3 edycji wsparcia (I: 12.2017-11.2018;II: 01.2019-12.2019;III: 02.2020-01.2021; w edycji po 30 UP) zapewniając UP komplek dostęp do usług społ. z uwzgl. indywid. potrzeb.Efektem działań projektu będzie osiągnięcie wskaźników obligator.: 90 osób zagr.ubóstwem/wykl.społ zostanie objęt.usługami społ świadczonymi w interesie ogólnym; 90 osób zostanie wyposażonych w kompetencje umożliwiające samodzielne funkcjonowanie w życiu i uniezależnienie się od wsparcia instytucjonalnego. The project implements the final product of the innow project: Choose the future-innovate.Tools in support of social intervention. Objective: Increase the activity and integration of 90 people aged 15-64 years of age. the risk of poverty and/or social outreach and their families and surroundings from Elbląg and the elbląsk area through the availability of high-quality community services during the period 10.2017-01.2021 Tasks: Individ. Diagnosis – Needs Identification, Aflatoun – program of social-finan education, specialist counseling. (legal,psychologist., advice for disadvantaged and incl. people), Individual and Group therapy,activation workshops (interpers training, personal development, entrepreneurship), health-promoting workshops (health prevention and individual dietary counseling.). The key part of the project is preventive action. Gr.doc.: 90 (54K,36M) people aged 15-64 with Ega and pow.elbl. (Warm.-maz.) the risk of poverty and/or outreach, including those who are unemployed.-(3 a group of so-called remotes from the labour market), the benefits of PO PZ and the family/environmental population (in such a way as it is necessary for the support of UP).Each UP (and their family/environment) will be subject to a minimum of 3 forms of support. Effects: We'll reach the target by moving. 3 editions of support (I: 12.2017-11.2018;II: 01.2019-12.2019;III: 02,2020 TO 01,2021; in the edition after 30 UP) providing UP complete access to the services of the community with individual needs.The effect of the project activities will be to achieve the obligator indicators.: 90 people in charge of poverty/general group will be covered by community services of general interest; 90 people will be equipped with competences to function independently in life and independence from institutional support. Le projet est mis en œuvre par la Fondation Elbląg en partenariat avec l’Association pour l’Europe. Le projet met en œuvre le produit final du projet dansNow: Choisissez les outils d’innovation pour l’avenir à l’appui de l’intervention sociale. Objectif: Accroître l’activité et l’intégration de 90 personnes âgées de 15 à 64 ans exposées au risque de pauvreté et/ou d’évaluation sociale ainsi que de leurs familles et de leurs environs de la région d’Elblag et d’Elbląsk grâce à la disponibilité de services sociaux de qualité au cours de la période 10.2017-01.2021. Diagnostic individuel — identification des besoins, Aflatoun — programme d’éducation sociale. (juridique,psychologue,conseils pour les personnes ayant une activité partielle et incl.), thérapie indienne et de groupe, ateliers d’activation (interpers. formation, développement personnel, entreprenariat), ateliers de promotion de la santé (prévention de la santé et conseil individuel en nutrition). La prévention est un élément clé du projet. Gr.doc.: 90 (54K,36M) personnes âgées de 15 à 64 ans d’Ega et po.elbl. (chaleurs-Maz.) exposés au risque de pauvreté et/ou de probation, y compris les chômeurs — (3ème groupe de soi-disant éloignés du marché du travail), bénéficier du PO et de la famille/de l’entourage des personnes ayant des coupables (dans un cercle qui est nécessaire pour soutenir l’UP).Toute personne de l’UP (ainsi que leurs familles/entourages) sera couverte par au moins trois formes de soutien. Effets: Nous atteindrons l’objectif en nous déplaçant. 3 éditions de soutien (I: 12.2017-11.2018;II: 01.2019-12.2019;III: 02.2020-01.2021; dans l’édition de 30 UP) permettant à UP d’accéder à des services correspondant aux besoins individuels. L’effet des activités du projet sera la réalisation d’indicateurs d’obligation. 90 personnes seront couvertes par des services communautaires d’intérêt général; 90 personnes seront dotées de compétences leur permettant de vivre de manière autonome dans la vie et de devenir indépendantes du soutien institutionnel.
