Основният фокус на проекта е насочен към осигуряването на качествена грижа за лицата с увреждания, включително с тежки множествени увреждания и техните семейства на територията на Община Пловдив. За постигане на тази цел ще бъде създаден ДЦЛУТМУ, който да осигури комплексна подкрепа за целевите групи чрез предоставяне на интегрирани социални услуги в общността. Ефективното функциониране на Центъра ще бъде осигурено от висококвалифицирания и обучен екип за управлението му, състоящ се от 20 специалиста с опит в различни сфери като здравно обслужване, социална и консултативна дейност, психологическа и терапевтична подкрепа и др. ДЦЛУТМУ ще се специализира в предоставянето на следните услуги: осигуряване на дневни и почасови услуги; мобилна работа, консултиране, информиране и подкрепа, включително и на лицата, полагащи грижи за лицата от целевата група; осигуряване на терапевтична и специализирана помощ, вкл. – рехабилитация и кинезитерапия; осигуряване на краткосрочни услуги по заместваща грижа чрез подкрепа за семействата, полагащи грижи за лица с увреждания; реализация на интегрирани подходи за мотивиране и насърчаване участието на пазара на труда на лица зависими от грижи и лица, полагащи грижи за тях. Очакваните резултати от реализацията на проектните дейности са: изграждане на постоянен достъп до интегрирани социални услуги за лицата от целевата група на територията на град Пловдив чрез създадения по Проекта ДЦЛУТМУ, което да осигури подобрение в начина и качеството им на живот, включително и да доведе до оптимизиране на условията за осъществяване на процесите по пълноценно включване в обществото и повишаване на социално-икономическата активност на лицата с увреждания, вкл. тежки такива, и техните семейства. The main focus of the project is to provide quality care for persons with disabilities, including severe multiple disabilities and their families on the territory of Plovdiv Municipality. To achieve this objective, a DCLUTS will be established to provide comprehensive support to target groups through the provision of integrated community-based social services. The effective functioning of the Centre will be ensured by the highly qualified and trained team for its management, consisting of 20 specialists with experience in different fields such as healthcare, social and advisory activities, psychological and therapeutic support, etc. DCSLUTHU will specialise in the provision of the following services: provision of daily and hourly services; mobile work, counselling, information and support, including the carers of the targeted persons; provision of therapeutic and specialised assistance, including rehabilitation and kinesitherapy; providing short-term replacement care services by supporting families caring for persons with disabilities; implementation of integrated approaches to motivating and encouraging the labour market participation of dependants and carers. The expected results of the project activities are: building permanent access to integrated social services for the targeted persons on the territory of the city of Plovdiv through the project created under the project “Tslutmu” to provide improvement in their way and quality of life, including to optimise the conditions for implementation of the processes of full inclusion in society and increase the socio-economic activity of persons with disabilities, including heavy ones, and their families.
