    OBJETIVOS: 1. Conocer el cumplimiento de indicadores de calidad y resultados auditables relacionados con la calidad de la colonoscopia en una muestra de centros de nuestro país; 2. Conocer qué factores influyen sobre el cumplimiento de estos indicadores de calidad; 3. Evaluar la utilidad de un programa de mejora de la calidad; 4. Establecer un registro de pacientes con adenomas de colon y pólipos serrados que pueda servir de punto de partida a futuros estudios que mejoren las evidencias disponibles sobre vigilancia post-polipectomía; 5. Validar factores genéticos y epigenéticos de la mucosa sana adyacente a los pólipos de colon que puedan estar relacionados con la existencia de lesiones colónicas múltiples. METODOS: Estudio observacional prospectivo. CRITERIOS DE INCLUSION: Individuos entre 40-80 años a los que se realiza una colonoscopia en los centros participantes como procedimiento de cribado de CCR, vigilancia post-polipectomía o en el estudio de pacientes con síntomas digestivos. Se prevé incluir información de unas 70.000 colonoscopias. Se analizarán indicadores de calidad (tasa de detección de adenomas, tasa de adenomas y tasa de complicaciones, entre otros). Se estudiará qué factores de la colonoscopia (limpieza colónica, uso de sedación, etc), del equipamiento endoscópico y del endoscopista (experiencia, dedicación, formación, etc) son predictores independientes de cada uno de los indicadores de calidad investigados. Los endoscopistas participantes serán aleatorizados a participar en un programa de mejora de la calidad, con posterior evaluación del efecto de este programa. Se validarán factores genéticos y epigenéticos previamente seleccionados en mucosa sana. (Spanish)

Establish a registry of patients with colon adenomas and serrated polyps that can serve as a starting point for future studies to improve the evidence available on post-polypectomy surveillance; 5. Validate genetic and epigenetic factors of the healthy mucosa adjacent to colon polyps that may be related to the existence of multiple colonic lesions. METHODS: Prospective observational study. INCLUSION CRITERIA: Individuals aged 40-80 years who undergo colonoscopy in participating centers such as CRC screening procedure, post-polypectomy surveillance or in the study of patients with digestive symptoms. Some 70,000 colonoscopies are expected to be included. Quality indicators (adenoma detection rate, adenoma rate and complication rate, among others) will be analysed. The factors of colonoscopy (colonic cleaning, sedation use, etc.), endoscopic equipment and endoscopist (experience, dedication, training, etc.) are independent predictors of each of the quality indicators investigated. Participating endoscopists will be randomised to participate in a quality improvement programme, with subsequent evaluation of the effect of this programme. Previously selected genetic and epigenetic factors in healthy mucosa will be validated. (English)
