Choroby cywilizacyjne, w tym przede wszystkim nadmierna masa ciała i cukrzyca stanowią jeden z najbardziej rozpowszechnionych problemów związanych ze stylem życia w PL, w tym w woj. Opolskim. Celem nadrzędnym projektu jest poprawa stanu zdrowia mieszkańców WO poprzez przeprowadzenie kompleksowej interwencji edukacyjno-zdrowotnej oraz wdrożenie działań zapobiegających nadwadze, otyłości i cukrzycy. Cel główny projektu to zmniejszenie zachorowalności na choroby metaboliczne poprzez przebadanie pod kątem nadwagi/otyłości 36 428 (18 333 K) oraz pod kątem cukrzycy 18214 (9289K)osób zamieszkujących w rozumieniu KC teren subregionu południowego (powiaty:kędzierzyńsko-kozielski, głubczycki, krapkowicki i strzelecki), a także objęcie opieką multikomponentową 700 (304K) osób, w okresie 01.08.2017-31.12.2019. Ponadto WN oraz Partner przewidują przeprowadzenie szeregu działań informacyjno edukacyjnych, a także objęcie kompleksowym i dostosowanym do indywidualnych potrzeb wsparciem min. 418 (213K)osób (60%) zagrożonych ubóstwem lub wykluczeniem społecznym- będą one traktowane priorytetowo (współpraca z OPS na terenie subregionu południowego SP).Działania informacyjno-edukacyjne będą prezentowane podczas rozmaitych wydarzeń kulturalnych i sportowych organizowanych w WO (WN rozmieści namioty, w których dokonywać będzie badań, a także przeprowadzi urozmaicony program dietetyczno-aktywnościowy, w tym warsztaty kulinarne, szkolenia zdrowotne,zajęcia sportowe etc.). Ponadto WN powoła Radę odpowiedzialną za jakość merytoryczną realizowanych działań (w tym zgodność z procedurami medycznymi) oraz nawiąże współpracę z takimi podmiotami jak: POZ, OPS, PCPR, IPP, NGO. Projekt zakłada realizację wszystkich założeń Regionalnego Programu Zdrowotnego WO. WN przeprowadzi monitoring realizacji założeń projektu po 6 i 12 m-cach od zakończenia interwencji, a także po zakończeniu realizacji projektu (współpraca z Partneram). Civilisation diseases, including overweight and diabetes, are one of the most common problems related to lifestyle in PL, including wars. I'm an Opole. The overarching objective of the project is to improve the health of WO residents by conducting a comprehensive educational and health intervention and implementing measures to prevent overweight, obesity and diabetes. The main objective of the project is to reduce the incidence of metabolic diseases by testing for overweight/obesity 36 428 (18 333 K) and for diabetes 18214 (9289K) of people living within the meaning of the Central Committee of the Southern Subregion (powiats:Kdzierzyńsko-kozielski, Łubczycki, krapkowicki and shooting), as well as to take multi-component care of 700 (304K) people, in the period 01.08.2017-31. In addition, the WN and the Partner envisage carrying out a number of educational activities, including comprehensive and tailored support of at least 418 (213K)people (60 %) at risk of poverty or social exclusion – they will be prioritised (cooperation with the OPS in the southern subregion of the SP). In addition, the JN will establish a Council responsible for the substantive quality of the activities carried out (including compliance with medical procedures) and cooperate with such entities as: POS, OPS, PCPR, IPP, NGO. The project assumes the implementation of all the objectives of the Regional Health Programme. WN will monitor implementation of project objectives after 6 and 12 m from the end of the intervention, as well as after completion of the project (cooperation with Partneram).
