GRUPA DOCELOWA:60os.(36kobiet i 24mężczyzn)lub rodzin zagrożonych ubóstwem lub wykluczeniem społecznym tj.os.lub rodziny korzystające ze świadczeń pomocy społecznej zgodnie z ustawą z dn.12 marca 2004r.o pomocy społecznej lub kwalifikujące się do objęcia wsparciem pomocy społecznej,spełniające co najmniej jedną z przesłanek określonych w art.7 ustawy z dn.12 marca 2004r.o pomocy społecznej,z obszaru woj.mazowieckiego,wyłącznie z obszarów poniżej progu defaworyzacji(w przypadku os.fizycznych zamieszkują na obszarze woj.mazowieckiego w roz.Kodeksu Cywilnego),w tym: *min.80%uczestników-os.doświadczające wielokrotnego wykluczenia roz.jako wykluczenie z powodu więcej niż jednej przesłanki *min.5%uczestników-os.z niepełnosprawnościami Preferowane do objęcia wsparciem w ramach projektu są również os.korzystające z Programu Operacyjnego Pomoc Żywnościowa 2014-2020. CELEM GŁÓWNYM PROJEKTU jest zwiększenie aktywności społeczno-zawodowej 60os.(36kobiet i 24mężczyzn)lub rodzin zagrożonych ubóstwem lub wykluczeniem społecznym z obszaru woj.mazowieckiego,wyłącznie z obszarów poniżej progu defaworyzacji,w terminie: X 2018-V 2019r.poprzez zestaw kompleksowych i zindywidualizowanych form wsparcia,w wyniku których uczestnicy nabędą kwalifikacje zawodowe i/lub podejmą zatrudnienie. ZADANIA: 1.Diagnoza indywidualnych potrzeb i potencjałów uczestników wraz z opracowaniem Indywidualnej Ścieżki Reintegracji dla 60os. 2.Zintegrowane poradnictwo rodzinne dla 60os. 3.Warsztaty rozwoju osobistego dla 36os. 4.Coaching grupowy dla 24os. 5.Szkolenie zawodowe dla 60os. 6.Pośrednictwo pracy dla 20os. 7.3 miesięczny staż zawodowy dla 40os. WSKAŹNIKI: *Nabycie kwalifikacji przez min.22%uczestników projektu *Osiągnięcie minimalnego poziomu efektywności społecznej i zatrudnieniowej: -34% i 22% dla os.zagrożonych ubóstwem lub wykluczeniem społecznym -34% i 12% dla os.o znacznym stopniu niepełnosprawności,os.z niepełnosprawnością intelektualną,os.z niepełnosprawnościami sprzężonymi. (36women and 24men), or families at risk of poverty or social exclusion, i.e. family benefiting from social assistance benefits in accordance with the Act of 12 March 2004 on social assistance, or eligible for social assistance, meeting at least one of the conditions set out in Article 7 of the Social Assistance Act of 12 March 2004 on social assistance, in the area of war of Mazowieckie, exclusively in areas below the threshold of disadvantage (in the case of natural persons in the area of civil war; * min.80 % of the persons who have experienced multiple exclusions as an exclusion because of more than one rationale * min.5 % of the persons with disabilities who have been Preferred for support under the project are also os. using the Operational Programme Food Aid 2014-2020. The MAIN PROJECT is intended to increase the socio-professional activity of 60os (36women and 24men), or families at risk of poverty or social exclusion from the area of the war of Mazowieckie, only from areas below the threshold for disadvantage, on time: X 2018-V 2019through a set of comprehensive and personalised forms of support, resulting in participants gaining a professional qualification and/or taking up employment. TASKS: 1.Diagnosis of the individual needs and potential of the participants together with the development of the Personal Profile for the Reintegration for 60os. 2.Integrated family counselling for 60os. 3.Workshop personal development workshops for 36os. 4.Coaching group for 24os. 5.Vocational training for 60os. 6.Employment intermediation for 20os. 7.3 monthly traineeship for 40os. INDICATORS: * Acquisition of qualifications by min.22 % of project participants * Achievement of a minimum level of social and employment efficiency: 34 % and 22 % for people who are at risk of poverty or social exclusion -34 % and 12 % for those with severe disabilities, for persons with intellectual disabilities, for those with secondary disabilities.
