Projekt skierowany jest do 8 (6K,2 M) mieszkańców gminy Człopa, będących osobami zagrożonymi wykluczeniem społecznym i/lub ubóstwem. W ramach projektu zaplanowano realizację następujących form wsparcia: - utworzenie i wsparcie dla funkcjonowania 2 mieszkań chronionych - treningowych oferujących 4 miejsca; - świadczenie usług społecznych w lokalnej społeczności dla osób korzystających z mieszkań chronionych powstałych w ramach projektu. Grupa docelowa: Projekty jest skierowany bezpośrednio do następującej grupy odbiorców: - osoby zagrożone ubóstwem i/lub wykluczeniem społecznym, - osoby z niepełnosprawnościami i ich rodziny; Wsparciem w ramach projektu objętych zostanie 8 osób (6K,2M) kwalifikujących się do wsparcia. Zadania zaplanowane w projekcie: - stworzenie (adaptacja i wyposażenie) 2 mieszkań chronionych oferujących 4 miejsca wsparcia - zapewnienie funkcjonowania 2 mieszkań chronionych i kompleksowane wsparcie osób w nich przebywających. Rezultaty: Liczba utworzonych w programie miejsc świadczenia usług w mieszkaniach wspomaganych i chronionych istniejących po zakończeniu projektu: 4. W przypadku mieszkań chronionych, zapewnia się, że jest stosowany standard (m.in. rodzaj i zakres wsparcia świadczonego w mieszkaniu chronionym oraz standard lokalu przeznaczonego na mieszkanie chronione) dotyczący tej formy pomocy wynikający z ustawy z dnia 12 marca 2014 r. o pomocy społecznej i rozporządzenia Ministra Pracy i Polityki Społecznej z dnia 14 marca 2012 r. w sprawie mieszkań chronionych. Usługi w ramach mieszkań wspomaganych wykonują również specjaliści w danej dziedzinie (opiekun, asystent) posiadający stosowne uprawnienia i kwalifikacje, zatrudniani do realizowania określonych rodzajów usług społecznych zg z Załącznikiem nr 7.19 Minimalne wymagania świadczenia usług społecznych w społeczności lokalnej. The project will provide for the following forms of support: Creation and support for the operation of 2 sheltered housing, with 4 seats; — the provision of social services in the local community for the beneficiaries of the sheltered housing created by the project. Audience: Projects are directly addressed to the following audience: People at risk of poverty and/or social exclusion, persons with disabilities and their families; The project will support 8 people (6K, 2M) eligible for support. Tasks planned in the project: Creation (adaptation and equipment) of 2 protected housing offering 4 support sites — ensuring the functioning of 2 sheltered housing and comprehensive support for people residing there. Results: Number of services created in the programme for the provision of assisted and sheltered housing that exists after the end of the project: 4. In the case of sheltered housing, it is ensured that the standard is used (inter alia, the type and scope of support provided in a sheltered dwelling and the standard of housing reserved for sheltered housing) as regards this form of aid resulting from the Social Assistance Act of 12 March 2014 and the Regulation of the Minister for Labour and Social Policy of 14 March 2012 on sheltered housing. Assisted living accommodation services shall also be performed by specialists in the field (keeper, assistant) with appropriate qualifications and qualifications, employed to perform certain types of social services, of Annex 7.19, minimum requirements for the provision of social services in the local community.
