    Risks related to the use of drugs increase with age and can have serious consequences (hospitalizations, deaths). These risks, referred to as drug-related iatrogenia, are largely preventable thanks to better collaboration between health professionals on the correct use of the drug. With this in mind, OPTIMAGE aims to deploy in the cross-border region of the Pyrenees a project for the prevention of drug-related iatrogenia in the elderly. Developed in connection with the APTITUDE project on the prevention of dependence among seniors, OPTIMAGE will be integrated into the APTITUDE network by strengthening it through the integration of city and hospital pharmacists. 3 axes are planned: - Creation of the network of fight against drug-related iatrogenia (RELIM) which will be based on the existing devices in the territories. It will involve pharmacists, family doctors, nurses, geriatricians, hospitals... Structures located in isolated geographic areas will be included as a priority. - Training / information for health professionals, the general public, associations, pharmaceutical companies. - Animation of the network with deployment of 2 flagship actions, one concerns seniors at home, and the other hospitalized seniors. This cross-border project should help to standardize practices, promote exchanges and strengthen actions in terms of prevention and reduction of drug-related iatrogenia among seniors. (English)
